    New contributions to the palaeoenvironmental framework of the Los Molles Formation (Early-to-Middle Jurassic), Neuquén Basin, based on palynological data

    Being the main oil-bearing basin of Argentina, the Neuquén Basin contains a well-documented stratigraphic record of continental and marine sedimentation during the Jurassic and Cretaceous in the western margin of Gondwana. Marine sedimentation started in the Early Jurassic with the deposition of the ofshore to prodelta shales of the Los Molles Formation, the basal unit of the Cuyo Group. A palynological study of outcrop samples of the Los Molles Formation at two localities,Puente Picún Leufú, southern Neuquén Basin, and Cordillera del Viento, central basin area, is presented. The palynological evidence allows inferring two diferent palaeoceanographic contexts during the deposition of the Los Molles Fm. At Puente Picún Leufú and the lower part of the Cordillera del Viento localities, the record of acritarchs and prasinophytes suggests a stratiied water column, suboxic-to-anoxic bottom conditions, and a reduced salinity within the photic zone, associated with a marginal marine environment under restricted oceanic circulation. These conditions would have last at least until the Early Bajocian. Conversely, at the middle and mainly the upper part of Cordillera del Viento locality, the predominance of dinocysts in the assemblages indicates a hydrographically unstable shelf (non-stratiied water mass column) with welloxygenatedbottom waters developed under open-marine settings with non-restricted oceanic circulation. The abundance and diversity of dinocyst assemblages are comparable with those observed in the Late Callovian Lotena Formation. These evidences suggest an open oceanic circulation due to the establishment of diferent seaways in the Neuquén Basin, during the inal accumulation of the Los Molles Formation (Early Callovian).Fil: Olivera, Daniela Elizabeth.     Detecção de anticorpos anti-Leishmania infantum syn chagasi em cães de São José do Rio Preto, São Paulo

    A leishmaniose visceral (LV) é uma zoonose que vem se expandido por todo o território paulista desde 1998, quando foi identificado o primeiro caso canino autóctone, no município de Araçatuba. O objetivo do presente estudo foi determinar a ocorrência de anticorpos anti-Leishmania infantum syn chagasi em amostras de soro de 584 cães de São José do Rio Preto, São Paulo, área não endêmica para a doença. Cinco cães (0,86%) foram soro reagentes pela técnica de ELISA e um (0,17%) por imunocromatografia. A reação de imunofluorescência indireta, realizada em 138 animais que possuíam densidades ópticas acima ou próximas ao ponto de corte do ELISA evidenciou dois cães (1,45%) com títulos acima de 1:40. Apenas um animal foi sororeagente nas três técnicas sorológicas. Apesar deste cão não apresentar histórico de deslocamento para áreas endêmicas, havia sido adquirido em região com casos caninos e humanos de LV. Os resultados obtidos no presente estudo fazem supor que não existiam casos autóctones de LV canina na população estudada.Visceral leishmaniasis (VL) has been a widespread zoonosis in São Paulo since 1998, when the first autochthonous canine case was identified in Araçatuba. The aim of this study was to determine the occurrence of anti-Leishmania infantum syn chagasi antibodies in serum samples of 584 dogs from São José do Rio Preto, São Paulo, a non endemic area for the disease. Five dogs (0.86%) seroconverted by ELISA and one (0.17%) by immunochromatography. The indirect immunofluorescent reaction, carried out in 138 animals whose optical densities were above or close to ELISA's cutt-off point, evidenced two dogs (1.45%) with titers above 1:40. Only one dog was serum-reactive on the three techniques. Although there was not a history of displacing this animal to endemic areas, the dog had been acquired in a region with canine and human cases of VL. These results suggests that there were no autochthonous cases of canine VL in this population

    En el entorno de la Laguna de Rocha se ha visto incrementada la superficie dedicada a las actividades agrícolas con un mayor uso de plaguicidas, entre ellos el herbicida glifosato, usado en cultivos de soja principalmente. Mediante la utilización de técnicas de inmunoensayo enzimático (ELISA), se investigó la presencia de glifosato en la Laguna y algunos de sus afluentes en dos momentos específicos de tiempo. Se detectó glifosato en 27 de las 28 muestras estudiadas. Muestras positivas por ELISA fueron confirmadas por cromatografía iónica. El método ELISA demostró ser una herramienta de screening adecuada para determinar la presencia de glifosato en agua

    COVID-19 PICU guidelines: for high- and limited-resource settings

    BACKGROUND: Fewer children than adults have been affected by the COVID-19 pandemic, and the clinical manifestations are distinct from those of adults. Some children particularly those with acute or chronic co-morbidities are likely to develop critical illness. Recently, a multisystem inflammatory syndrome (MIS-C) has been described in children with some of these patients requiring care in the pediatric ICU. METHODS: An international collaboration was formed to review the available evidence and develop evidence-based guidelines for the care of critically ill children with SARS-CoV-2 infection. Where the evidence was lacking, those gaps were replaced with consensus-based guidelines. RESULTS: This process has generated 44 recommendations related to pediatric COVID-19 patients presenting with respiratory distress or failure, sepsis or septic shock, cardiopulmonary arrest, MIS-C, those requiring adjuvant therapies, or ECMO. Evidence to explain the milder disease patterns in children and the potential to use repurposed anti-viral drugs, anti-inflammatory or antithrombotic therapies are also described. CONCLUSION: Brief summaries of pediatric SARS-CoV-2 infection in different regions of the world are included since few registries are capturing this data globally.     Independent lung ventilation in a newborn with asymmetric acute lung injury due to respiratory syncytial virus: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Independent lung ventilation is a form of protective ventilation strategy used in adult asymmetric acute lung injury, where the application of conventional mechanical ventilation can produce ventilator-induced lung injury and ventilation-perfusion mismatch. Only a few experiences have been published on the use of independent lung ventilation in newborn patients.</p> <p>Case presentation</p> <p>We present a case of independent lung ventilation in a 16-day-old infant of 3.5 kg body weight who had an asymmetric lung injury due to respiratory syncytial virus bronchiolitis. We used independent lung ventilation applying conventional protective pressure controlled ventilation to the less-compromised lung, with a respiratory frequency proportional to the age of the patient, and a pressure controlled high-frequency ventilation to the atelectatic lung. This was done because a single tube conventional ventilation protective strategy would have exposed the less-compromised lung to a high mean airways pressure. The target of independent lung ventilation is to provide adequate gas exchange at a safe mean airways pressure level and to expand the atelectatic lung. Independent lung ventilation was accomplished for 24 hours. Daily chest radiograph and gas exchange were used to evaluate the efficacy of independent lung ventilation. Extubation was performed after 48 hours of conventional single-tube mechanical ventilation following independent lung ventilation.</p> <p>Conclusion</p> <p>This case report demonstrates the feasibility of independent lung ventilation with two separate tubes in neonates as a treatment of an asymmetric acute lung injury.</p

    Transitional Care for Young People with Movement Disorders: Consensus-Based Recommendations from the MDS Task Force on Pediatrics

    Background: The International Parkinson and Movement Disorders Society (MDS) set up a working group on pediatric movement disorders (MDS Task Force on Pediatrics) to generate recommendations to guide the transition process from pediatrics to adult health care systems in patients with childhood-onset movement disorders. / Methods: To develop recommendations for transitional care for childhood onset movement disorders, we used a formal consensus development process, using a multi-round, web-based Delphi survey. The Delphi survey was based on the results of the scoping review of the literature and the results of a survey of MDS members on transition practices. Through iterative discussions, we generated the recommendations included in the survey. The MDS Task Force on Pediatrics were the voting members for the Delphi survey. The task force members comprise 23 child and adult neurologists with expertise in the field of movement disorders and from all regions of the world. / Results: Fifteen recommendations divided across four different areas were made pertaining to: (1) team composition and structure, (2) planning and readiness, (3) goals of care, and (4) administration and research. All recommendations achieved consensus with a median score of 7 or greater. / Conclusion: Recommendations on providing transitional care for patients with childhood onset movement disorders are provided. Nevertheless several challenges remain in the implementation of these recommendations, related to health infrastructure and the distribution of health resources, and the availability of knowledgeable and interested practitioners.     ACIDENTES CAUSADOS POR SERPENTES DO GÊNERO Bothrops EM PEQUENOS ANIMAIS – REVISÃO DE LITERATURA

    As serpentes do gênero Bothrops são responsáveis por cerca de 90% dos casos de acidentes ofídicos em seres humanos na América do Sul, sendo que deste valor, 89% dos casos ocorrem no Brasil, isso ocorre devido ao fato de que o Brasil possui as características necessárias para o habitat das serpentes botrópicas. O acidente botrópico apresenta como características as marcas das presas da serpente no local da picada, edema, sangramento local e até necrose; sendo assim o veneno possui ações proteolíticas, vasculotóxicas e coagulantes. Para o diagnóstico do acidente botrópico deve-se levar em consideração o histórico do paciente, achados no exame físico e nos exames laboratoriais, que sugiram o acidente. Sempre que houver suspeita de acidente botrópico deve ser avaliado o tempo de coagulação sanguínea (TC), que geralmente encontra-se de aumentado (maior que 10 minutos) a incoagulável (maior que 30 minutos). O tratamento baseia-se na soroterapia devendo ser realizado o mais rápido possível, podendo ser utilizado o soro antibotrópico ou soro polivalente. Considerando a importância dos acidentes botrópicos no nosso país, objetivou-se com esta revisão bibliográfica abordar o acidente botrópico, tanto pelas suas propriedades tóxicas quanto farmacológicas, bem como as medidas terapêuticas na condição deste tipo de acidente
