    The Effects of Font Type on Reading Accuracy and Fluency in Japanese Children with Developmental Dyslexia

    We administered rapid reading tasks in Japanese children (32 with typical development and 24 with developmental dyslexia), and investigated the effects of two different font types: Rounded-Gothic and Mincho style font. In the experiment, we used four kinds of stimuli: two scripts (paragraphs and random kana character non-words) in two font types (Rounded-Gothic and Mincho style font). In this experiment, the duration time, the number of errors and the number of self-corrections were measured during reading. Participants were asked which font type was easier to read. There was no significant difference in the duration time, the number of errors and the number of self-corrections between two types of fonts among the 56 participants. On the other hand, regarding subjective readability, the developmental dyslexia group reported that the Rounded-Gothic font was easier to read. There was a difference between objective and subjective readability. In this study, there was no difference in reading performance of Rounded-Gothic and Mincho style fonts in Japanese children with developmental dyslexia

    A Case of Severe Esophageal Intramural Pseudodiverticulosis Whose Symptoms Were Ameliorated by Oral Administration of Anti-Fungal Medicine

    Esophageal intramural pseudodiverticulosis (EIPD) is a rare disease of unknown etiology that displays multiple pseudodiverticula radiologically, leading to benign esophageal stricture. Dysphagia, which sometimes slowly progresses, is the main symptom in the majority of cases. We here report a 59-year-old male EIPD patient who suffered from severe dysphagia. Radiography and endoscopy of this patient disclosed a severe constriction in the upper thoracic esophagus. Although we tried several endoscopic procedures including frequent endoscopic balloon dilatation (EBD), the effect was very limited and his dysphagia relapsed shortly after the treatments. During the procedures, we noticed some white, thick, creamy liquid emerging from the orifices of EIPD, and PAS staining of biopsy specimens revealed infection with Candida albicans. Hence, the patient was given anti-fungal medicine in addition to EBD. The additional treatment with anti-fungal medicine dramatically improved his symptoms and the esophageal constriction. This case suggests that anti-fungal treatment is an effective first-line therapy even against a severe form of esophageal constriction in EIPD

    奈良県の地域住民と生活環境におけるピロリ菌の蔓延実態

    Background: Chronic infection with Helicobacter pylori, specifically cagA-positive strains, is associated with gastric cancer. Thus, measures to prevent H. pylori infection are required. This study was conducted to clarify the prevalence of H. pylori in the community to identify the infection source and comprehensively assess the risk of H. pylori infection. Methods: We collected 90 human faecal samples and 73 environmental samples (water, vegetable, and animal faecal samples) from the residents in an area with a high incidence of gastric cancer in Japan. Polymerase chain reaction assay was performed to detect the glmM housekeeping gene and the cagA virulence gene of H. pylori. A questionnaire survey was conducted, and the responses were analyzed statistically. Results: The glmM gene was detected in 18 of 90 (20%) faecal samples obtained from residents; among them, the cagA gene was detected in 33.3% (6/18), and in all who had undergone eradication therapy. H. pylori was not detected in environmental samples. However, contact with dogs (OR 3.89, 95% CI 1.15-13.15, P < 0.05) was associated with higher odds for glmM gene positivity in the questionnaire survey. Conclusions: The prevalence of H. pylori and cagA-positive strains among the residents was low. However, the study results suggest a correlation between recurrent infection and cagA-positive H. pylori strains. Although H. pylori genes were not detected in living environments, an association between contact with dogs and a glmM positive status was revealed. Further investigations targeting community-dwelling healthy people and their living environments would be required for H. pylori infection control.博士(医学)・甲第774号・令和3年3月15日© 2020 The Authors.     Evolution of the public opinion on COVID-19 vaccination in Japan

    Vaccines are promising tools to control the spread of COVID-19. An effective vaccination campaign requires government policies and community engagement, sharing experiences for social support, and voicing concerns to vaccine safety and efficiency. The increasing use of online social platforms allows us to trace large-scale communication and infer public opinion in real-time. We collected more than 100 million vaccine-related tweets posted by 8 million users and used the Latent Dirichlet Allocation model to perform automated topic modeling of tweet texts during the vaccination campaign in Japan. We identified 15 topics grouped into 4 themes on Personal issue, Breaking news, Politics, and Conspiracy and humour. The evolution of the popularity of themes revealed a shift in public opinion, initially sharing the attention over personal issues (individual aspect), collecting information from the news (knowledge acquisition), and government criticisms, towards personal experiences once confidence in the vaccination campaign was established. An interrupted time series regression analysis showed that the Tokyo Olympic Games affected public opinion more than other critical events but not the course of the vaccination. Public opinion on politics was significantly affected by various events, positively shifting the attention in the early stages of the vaccination campaign and negatively later. Tweets about personal issues were mostly retweeted when the vaccination reached the younger population. The associations between the vaccination campaign stages and tweet themes suggest that the public engagement in the social platform contributed to speedup vaccine uptake by reducing anxiety via social learning and support

    Involvement of the accumbal osteopontin-interacting transmembrane protein 168 in methamphetamine-induced place preference and hyperlocomotion in mice

    Chronic exposure to methamphetamine causes adaptive changes in brain, which underlie dependence symptoms. We have found that the transmembrane protein 168 (TMEM168) is overexpressed in the nucleus accumbens of mice upon repeated methamphetamine administration. Here, we firstly demonstrate the inhibitory effect of TMEM168 on methamphetamine-induced behavioral changes in mice, and attempt to elucidate the mechanism of this inhibition. We overexpressed TMEM168 in the nucleus accumbens of mice by using an adeno-associated virus vector (NAc-TMEM mice). Methamphetamine-induced hyperlocomotion and conditioned place preference were attenuated in NAc-TMEM mice. Additionally, methamphetamine-induced extracellular dopamine elevation was suppressed in the nucleus accumbens of NAc-TMEM mice. Next, we identified extracellular matrix protein osteopontin as an interacting partner of TMEM168, by conducting immunoprecipitation in cultured COS-7 cells. TMEM168 overexpression in COS-7 cells induced the enhancement of extracellular and intracellular osteopontin. Similarly, osteopontin enhancement was also observed in the nucleus accumbens of NAc-TMEM mice, in in vivo studies. Furthermore, the infusion of osteopontin proteins into the nucleus accumbens of mice was found to inhibit methamphetamine-induced hyperlocomotion and conditioned place preference. Our studies suggest that the TMEM168-regulated osteopontin system is a novel target pathway for the therapy of methamphetamine dependence, via regulating the dopaminergic function in the nucleus accumbens

    Molecular epidemiology of Clostridioides difficile and risk factors for the detection of toxin gene-positive strains

    In this study, we investigated all Clostridioides difficile strains isolated from stool samples in Nagasaki University Hospital between January 2012 and December 2014. Toxin genes (tcdA, tcdB and cdtA/cdtB) were analyzed for multiplex PCR in a total of 213 strains. In the toxin gene-positive strain, PCR ribotyping was conducted using capillary gel electrophoresis-based PCR and the Webribo database. Patients’ backgrounds were analyzed by departments, disorders, antimicrobials, and clinical dates. The positive rates of tcdA, tcdB, and cdtA/cdtB genes were 62.9%, 63.4%, and 2.8%, respectively. The most frequent PCR ribotype was 047 (14.1%), followed by 014/0 (11.1%) and 002/0 (8.2%). In univariate analysis, the risk factors for the detection of toxin gene-positive strains in patients were older age (p = 0.0036), over ≥ 65 years old (p = 0.0175), the patients hospitalized at Department of Digestive Surgery (P = 0.0059), higher CRP level (P = 0.0395), and lower albumin level (p = 0.0014). In the multivariate analysis, the risk factor for detection of toxin gene-positive strains was the patients hospitalized at Department of Digestive Surgery (OR; 4.62, 95% CI; 1.18–18.0, p = 0.0274). In this study, the percentage of toxin gene-positive and cdtA/cdtB gene-positive strains was almost the same as that reported in previous studies, but the ribotype was different. In addition, we revealed that the risk factor associated with the detection of toxin gene-positive strains was the patients hospitalized at Department of digestive surgery
