    Photoacoustic emission from Au nanoparticles arrayed on thermal insulation layer

    Efficient photoacoustic emission from Au nanoparticles on a porous SiO2 layer was investigated experimentally and theoretically. The Au nanoparticle arrays/porous SiO2/SiO2/Ag mirror sandwiches, namely, local plasmon resonators, were prepared by dynamic oblique deposition (DOD). Photoacoustic measurements were performed on the local plasmon resonators, whose optical absorption was varied from 0.03 (3%) to 0.95 by varying the thickness of the dielectric SiO2 layer. The sample with high absorption (0.95) emitted a sound that was eight times stronger than that emitted by graphite (0.94) and three times stronger than that emitted by the sample without the porous SiO2 layer (0.93). The contribution of the porous SiO2 layer to the efficient photoacoustic emission was analyzed by means of a numerical method based on a one-dimensional heat transfer model. The result suggested that the low thermal conductivity of the underlying porous layer reduces the amount of heat escaping from the substrate and contributes to the efficient photoacoustic emission from Au nanoparticle arrays. Because both the thermal conductivity and the spatial distribution of the heat generation can be controlled by DOD, the local plasmon resonators produced by DOD are suitable for the spatio-temporal modulation of the local temperature

    Photothermally controlled Marangoni flow around a micro bubble

    We have experimentally investigated the control of Marangoni flow around a micro bubble using photothermal conversion. Using a focused laser spot acting as a highly localized heat source on Au nanoparticles/dielectric/Ag mirror thin film enables us to create a micro bubble and to control the temperature gradient around the bubble at a micrometer scale. When we irradiate the laser next to the bubble, a strong main flow towards the bubble and two symmetric rotation flows on either side of it develop. The shape of this rotation flow shows a significant transformation depending on the relative position of the bubble and the laser spot. Using this controllable rotation flow, we have demonstrated sorting of the polystyrene spheres with diameters of 2 μm and 0.75 μm according to their size

    An Adult Case of Kawasaki Disease in a Pregnant Japanese Woman: A Case Report

    Kawasaki disease is an acute febrile disease predominantly seen in young children. We report a case of Kawasaki disease in a 32-year-old pregnant woman. She developed a generalized erythematous skin rash accompanied by high fever. Bilateral conjunctival congestion, tender cervical lymphadenopathy, an edematous lower lip and peripheral edema followed by desquamation were observed. She was successfully treated with aspirin and intravenous gammaglobulin (1 g/kg/day). Her course was not complicated by coronary artery aneurysm and she delivered a healthy baby. To the best of our knowledge, this is the first case of Kawasaki disease in a pregnant woman. We suggest that Kawasaki disease should be included in the differential diagnosis of a generalized, erythematous skin rash accompanied by high fever in adults

    Gravitational fermion creation during an anisotropic phase of cosmological expansion

    The free Dirac equation is solved in a Bianchi Type I space-time, which represents a homogeneous but anisotropic universe, to show the creation of fermionic particles. It is found that unlike in the isotropic case, massless fermion production is possible. An estimate of the energy density of massless particles created during an early anisotropic phase of cosmological expansion is shown to cause substantial back-reaction on the gravitational field. The potential relevance to dark matter particle production, primordial magnetogenesis, and early universe cosmology is discussed briefly.Comment: Accepted for publication in Physical Review

    Prognostic significance of cribriform adenocarcinoma of the lung: validation analysis of 1,057 Japanese patients with resected lung adenocarcinoma and a review of the literature

    Background: Cribriform-predominant adenocarcinoma of the lung (Cribri-ADC) is a recently described tumor growth pattern. However, its prognostic impact has not been clearly determined. We analyzed the data of a series of 1, 057 Japanese patients with resected lung adenocarcinoma to identify the clinical significance of Cribri-ADC. Methods: Cribriform pattern (Cribri-p) is defined as invasive back-to-back fused tumor glands with poorly formed glandular spaces or invasive tumor nests comprising tumors cells that produced glandular lumina. We investigated the correlations of Cribri-p and Cribri-ADC with clinicopathological factors as well as disease-free survival (DFS) and overall survival (OS). Results: Cribri-p was present in 217 patients (20.5%) and Cribri-ADC was determined in 25 patients (2.4%). Cribri-p was associated with larger tumor size, pleural invasion, vascular invasion, lymphatic invasion, and spreading through air spaces (STAS) (all, P<0.0001). Cribri-ADC was associated with younger age (P=0.019), vascular invasion (P=0.0025), STAS (P<0.0001), and ALK rearrangement (P=0.012). The DFS curve of patients with Cribri-ADC was identical to that of patients with solid adenocarcinoma; however, the OS curve was located between that of patients with papillary and acinar adenocarcinoma. Of the 10 patients who had tumor recurrences, eight had EGFR mutations or ALK rearrangement, six of whom achieved relatively long survival (median, 64.6, range, 37.4–113 months) following treatment with tyrosine kinase inhibitors (TKIs). In multivariate analysis, Cribri-ADC was not an independent prognostic factor of either recurrence or death. Conclusions: Cribri-ADC is associated with a higher risk of recurrence; however, most patients can be successfully treated with TKIs

    Endoscopic therapy using an endoscopic variceal ligation for minute cancer of the esophagogastric junction complicated with esophageal varices: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Standard endoscopic mucosal resection or endoscopic submucosal dissection is a procedure for patients with minute cancers, complicated with esophageal varices that puts them at high risk of bleeding.</p> <p>Case presentation</p> <p>We present the case of a 77-year-old Japanese man with alcoholic cirrhosis who underwent a routine endoscopy examination as a screening procedure for esophageal varices and was incidentally diagnosed as having minute cancer of the esophagogastric junction with esophageal varices. Endoscopic ultrasonography findings suggested that the minute cancer was a non-invasive carcinoma (carcinoma <it>in situ</it>) and a 2 mm in diameter blood vessel, feeding the esophageal varices, pierced the lesion. Following the examination, we carried out endoscopic treatment of the minute cancer and esophageal varices. Endoscopic variceal ligation was performed using a pneumo-activated device (Sumitomo Bakelite, Tokyo, Japan). Two years after the treatment, during the follow-up endoscopic examination on the patient, recurrence of carcinoma was not detected endoscopically or histologically.</p> <p>Conclusion</p> <p>Endoscopic therapy using an endoscopic variceal ligation device for minute cancer of the esophagogastric junction, complicated with esophageal varices, may be an acceptable and easily applicable method.</p

    キャリア成熟は、Super,D.E.(1957)が提唱している職業的発達 vocational development の概念を発展的に継承したものであり、「発達課題に取り組もうとする個人の態度的、認知的レディネス」(Crites、1978)と定義されている。本研究においては、坂柳(1990)が開発した「成人キャリア成熟尺度(Adult Career Maturity Scales : ACMS)」を基礎に、看護師508名のデータを用いて項目反応理論による項目選定と構造方程式モデリングによる確証的因子分析を行い、看護師用の職業キャリア成熟の測定に寄与する適切な項目配置を検討した。その結果、あらかじめ仮定した3因子斜交モデルのデータへの適合性は、CFIが0.921、RMSEAが0.081であり、また看護職者の職業キャリア成熟における「関心性」「自律性」「計画性」の3因子各4項目で測定できることを明らかにした。本研究においては、看護職者における「職業キャリア成熟」の測定尺度が開発できたが、今後はさらに、前記因子モデルの不変性について検討を継続すると同時に、「職業キャリア成熟」の影響要因やインパクトについての検討が必要なことが推察された。The theory of career maturity advanced by Super (1957) developed out of the concept of occupational development. It is defined as an individuals\u27 attitudinal and cognitive read-iness for developmental challenges (Crites,1978). The present study reviewed the Adult Career Maturity Scales: ACMS developed by Sakayanagi (1990) and examined its construct validity using item response thelry. A confirmatory factor analysis (structural equation modeling) was performed based on the data of 508 nurses. The research questions checkd the number of reliable and interpretable factors in the data set, and clarified the nature of the factors. The fitness of the model was confirmed (CFI = 0.91; SRMR = 0.081) and a three factor model of "concern","autonomy",and "planning" was determined. Future research will test the reliability of this model and examine the causes and effects of career maturity
