    Results of surgical treatment of thymomas with special reference to the involved organs

    AbstractObjective: The purpose of this study is to clarify the significance of the particular involved organ as a prognostic factor and its relation to other previously reported factors. Methods: The prognoses of 194 consecutive patients with thymoma who had undergone complete or subtotal resection were reviewed retrospectively. Survival was evaluated as actuarial freedom from tumor death. Analysis of prognostic factors was performed by the Kaplan-Meier method with the log rank test and Cox's proportional hazards model. Results: The Masaoka staging system and involvement of the great vessels were the independent prognostic factors in the entire study group; age, sex, histologic subtype, completeness of resection, association of myasthenia gravis, or involvement of other organs were not factors. The 10-year and 20-year survivals were 99% and 90% in stage I, 94% and 90% in stage II, 88% and 56% in stage III, 30% and 15% in stage IVa, 0% and 0% in stage IVb, 93% and 83% in the absence of involvement of the great vessels, and 54% and 20% in the presence of it. Involvement of the great vessels was also the single independent prognostic factor in the patients with stage III disease although completeness of resection or involvement of other organs were not. The 10-year and 20-year survivals in patients with stage III disease were 97% and 75% in the absence of involvement of the great vessels, and 70% and 29% in the presence of it. Conclusion: Although the Masaoka staging system is a valuable prognostic factor, the category of stage III is heterogeneous and consists of 2 groups with distinct prognoses depending on involvement of the great vessels.     The Efficacy of Reboxetine as an Antidepressant, a Meta-analysis of Both Continuous (Mean HAM-D Score) and Dichotomous (Response Rate) Outcomes

    Reboxetine is the first selective Norepinephrine Reuptake Inhibitor (NRI). Thereare limited numbers of quantitative synthesis studies of the efficacy of this drug intreating depressive disorders. We have meta-analyzed the efficacy of the reboxetineusing both continuous and dichotomous outcome measures. Data was collected from thePubmed search of English-language studies published from 1997 to 2007 and manualsearch of retrieved articles. We have searched for controlled clinical trials of reboxetinewith any other antidepressant comparator or placebo in adults with depressivedisorders using HAM-D scale for the outcome measure. After 11 studies were selected,separate meta-analyses for the active drug and for the placebo were performed usingrandom effect model. The overall effect size compared with the other antidepressantswas -0.06 (95%CI: -0.19; 0.08), with placebo -1.54 (95%CI: -2.23; -0.85). It wascalculated using the final mean HAM-D score (continuous outcome). The pooled SDwas used when the variance was not available. Pooled odds ratios for the response rates(dichotomous outcome) were 1.04 (95%CI: 0.75; 1.46) and 2.85 (95%CI: 1.88; 4.31) forthe active drug and placebo comparisons accordingly. These results suggest that theefficacy of the reboxetine and the other antidepressants (SSRI, TCA and SNRI) on bothmeasures does not differ while it is significantly superior to placebo

    Pneumothorax during Pemetrexed Treatment in a Patient with Non-Small Cell Lung Cancer: A Case Report and Literature Review

    Pemetrexed is a multitargeted antifolate that has demonstrated antitumor activity in non-small cell lung cancer. A 70-year-old male presented with a stage IV non-small cell lung cancer. The patient was treated with pemetrexed as third-line chemotherapy. However, a pneumothorax occurred 16 days after the administration of the second cycle of pemetrexed. The pneumothorax was slight and the patient was observed without undergoing any additional treatment. Twenty-four days after its initial occurrence, the pneumothorax had improved. This is the first case of pneumothorax that has been observed during pemetrexed treatment. Pneumothorax during chemotherapy is rare; however, it is a life-threatening complication and should not be overlooked

    Current Situation and Factors of Farmland Diversion around New Road Construction in Rural Area

    In general, it is supposed that the construction of the new roads in rural areas is a major factor to induce the landuse conversion of farms located adjacent to the construction sites, which influences the change of green landscape. However, the substantial factors of farm conversion and its relationship of road construction are not yet clarified. This study aims to analyze the current trends of the farmland conversion around the new road construction sites and to realize the influence of road construction through the case study in Chikugo Region, Fukuoka Prefecture. As the results of the statistic analysis on the previous examples of farm conversion, we clarify the relationship between increasing trend of farm conversion and road construction factor and that the influence multiplied with the location factor strengthens the trend of farm conversion

    A lot of the rural and suburban areas in Japan are confronted with the severe problems of worsened landscape and environment caused by the increasing urban developments and sprawl. It is needed to adopt the effective land use control for preservation of natural and historical landscape. In this context, the study focuses on rural road landscapes, and aims to identify the characteristics by analyzing the composing landscape elements, and to propose the useful methods and tools for planning and design based on the overview of previous studies. As results, we specified eight characteristics to compose the rural road landscape, and figure out the systematic use of tools for prediction, analysis and evaluation which can be applicable for rural road landscape planning and design process
