BackgroundChylothorax is the accumulation of chyle in the pleural cavity, usually develops after disruption of the thoracic duct along its intra-thoracic route. In the majority of cases this rupture is secondary to trauma (including cardio thoracic surgeries). Chylothorax is a potentially serious complication after cardiovascular surgeries that require early diagnosis and adequate management.MethodsA retrospective study of all cases complicated with chylothorax after pediatric cardiac surgery in King Abdulaziz Cardiac Center between January 2007 and December 2009 was conducted. The study aim to determine the risk factors, and the impact on the post operative course.ResultsWe have 1135 cases operated during the study period, 57 cases (5%) were complicated with chylothorax in the post operative period. The most common surgeries complicated with chylothorax were the single ventricle repair surgeries (Glenn-Fontan) 15 cases (27%), followed by the arch repair cases 10 cases (18%), the ventricular septal defect cases 10 cases (18%), the Atrioventricular septal defect cases 7 cases (12%), the arterial switch cases 6 cases (11%), and others 8 cases (14%). The ICU stay, the length of hospital stay and the bypass time were significantly longer in the chylothorax group, also the ventilation time, the inotropes duration and number were higher in the chylothorax group.ConclusionChylothorax after pediatric open heart surgery is not an uncommon complication, it occurs more commonly with single ventricle repair and aortic arch repair surgeries, it has a significant impact on the post operative course and post operative morbidity

Cerebrovascular circulation; Ischemia; Ischemic strokeCirculació cerebrovascular; Isquèmia; Ictus isquèmicCirculación cerebrovascular; Isquemia; Ictus isquémicoBackground and Purpose Various imaging paradigms are used for endovascular treatment (EVT) decision-making and outcome estimation in acute ischemic stroke (AIS). We aim to compare how these imaging paradigms perform for EVT patient selection and outcome estimation. Methods Prospective multi-center cohort study of patients with AIS symptoms with multi-phase computed tomography angiography (mCTA) and computed tomography perfusion (CTP) baseline imaging. mCTA-based EVT-eligibility was defined as presence of large vessel occlusion (LVO) and moderate-to-good collaterals on mCTA. CTP-based eligibility was defined as presence of LVO, ischemic core (defined on relative cerebral blood flow, absolute cerebral blood flow, and cerebral blood volume maps) 1.8, absolute mismatch >15 mL. EVT-eligibility and adjusted rates of good outcome (modified Rankin Scale 0-2) based on these imaging paradigms were compared. Results Of 289/464 patients with LVO, 263 (91%) were EVT-eligible by mCTA-criteria versus 63 (22%), 19 (7%) and 103 (36%) by rCBF, aCBF, and CBV-CTP-criteria. CTP and mCTA-criteria were discordant in 40% to 53%. Estimated outcomes were best in patients who met both mCTA and CTP eligibility-criteria and were treated with EVT (62% to 87% good outcome). Patients eligible for EVT by mCTA-criteria and not by CTP-criteria receiving EVT achieved good outcome rates of 53% to 57%. Few patients met CTP-criteria and not mCTA-criteria for EVT. Conclusions Simpler imaging selection criteria that rely on little else than detection of the occluded blood vessel may be more sensitive and less specific, thus resulting in more patients being offered EVT and arguably benefiting from it.This study was supported by a grant from the Canadian Institute of Health Research. Dates have been recorded as part of human diet since antiquity and throughout major human civilizations including the biblical traditions. Dates are a robust, flexible and long lasting plant that can survive in a harsh environment. In Arabic, it is known as al-nakhl, which etymologically means “good, nourishing and nice smelling”. Dates are mentioned in the Qur’an for 27 times as nakhl, nakhla, alnakhlah, nakhil, and others; ascertaining its significance. Therefore, this study attempts to analyze the interpretations of the verses according to significant books of commentary. By applying textual analysis method, the study found that Allah has revealed that the date palm has soft spadix producing clusters of dates hanging low and near; being arranged and layered one above another. It also has tough trunk; most suitable for crucifixion and to be attached to during childbirth. Allah uses the hollow and uprooted trunks as parables and proclaims that some of the date palms come from a single stem root. Allah conveys that the date palm produces sheathed fruit-stalks; enclosing dates as well as fruits of different taste. He also admits that strong drink i.e. liquor could be derived from dates i.e. before its prohibition and describes the date palm stone in three different places. Simultaneously, He sets forth parables with three small parts hidden in the nucleus of dates known as al-fatil, al-naqir and al-qitmir. Allah has also described the date palm using different terms known as shajarah tayyibah, kull hin and min linah

Non-pharmacological interventions including mobility restriction have been developed to curb transmission of SARS-CoV-2. We provided precise estimates of disease burden and examined the impact of mobility restriction on reducing the COVID-19 effective reproduction number in the Kingdom of Saudi Arabia. This study involved secondary analysis of open-access COVID-19 data obtained from different sources between 2 March and 26 December 2020. The dependent and main independent variables of interest were the effective reproduction number and anonymized mobility indices, respectively. Multiple linear regression was used to investigate the relationship between the community mobility change and the effective reproduction number for COVID-19. By 26 December 2020, the total number of COVID-19 cases in Saudi Arabia reached 360,690, with a cumulative incidence rate of 105.41/10,000 population. Al Jouf, Northern Border, and Jazan regions were ≥2.5 times (OR = 2.93; 95% CI: 1.29–6.64), (OR = 2.50; 95% CI: 1.08–5.81), and (OR = 2.51; 95% CI: 1.09–5.79) more likely to have a higher case fatality rate than Riyadh, the capital. Mobility changes in public and residential areas were significant predictors of the COVID-19 effective reproduction number. This study demonstrated that community mobility restrictions effectively control transmission of the COVID-19 virus

Date palm (Phoenix dactylifera) fruit is an important food in the Middle East. Some date palm cultivars like the Ajwa is revered for its health-promoting properties. Phytochemicals like phenolic compounds are in part responsible for such health benefits. However, detailed information on these compounds is lacking. The present work aimed to investigate phytochemical components of Ajwa cultivar using UHPLC-ESI-QTOF-MS/MS in three different extraction solvents (aqueous, methanol–water, acetone–water) and three different extraction durations (2, 5 and 24 h). The proposed method provided tentative identification of 169 bioactive compounds out of which 44 (polyphenols and other phytochemical compounds) were successfully identified from three different extracts and three different extraction durations. Twenty-one compounds never previously reported in the Ajwa cultivar were identified. Aqueous-based extraction solvent and 24-h extraction duration yielded most phytochemical compounds

Te exposure of concrete to elevated temperatures is known to cause diverse severe damages in concrete composites. Hence, measures to improve the performance of concrete during exposure to fre are continually proposed. Te present study investigated the postfre residual strength and morphology of concrete incorporating natural rubber latex exposed to elevated temperature. Four diferent concrete mixes were considered for the investigation, namely, a control sample made without natural rubber latex, the second sample containing 1% natural rubber latex, the third sample containing 1.5% natural rubber latex, and the fourth sample containing 3% of natural rubber latex. Te concrete samples (150 mm cubes and 100 × 200 mm cylinders) were exposed to varying temperatures 300°C, 800°C, and 1000°C, after the curing process. Nondestructive tests using Schmidt rebound hammer and ultrasonic pulse tester were carried out on samples. Te compressive strength and split-tensile strength of concrete cubes and cylinders, respectively, were determined. Micrographs and elemental distribution in the sample were studied using the scanning electron microscopy (SEM-EDX) apparatus. It could be seen from the results that the concrete strength properties reduced as the exposure temperature increased. Te results also showed that NRL could be sparingly utilized as a concrete admixture, at 1% content. Te performance of concrete was not stable at over 300°C when NRL addition was above 1%
