    Effects of Activity Participation of the Elderly on Quality of Life

    Quality of life (QOL) and personality were examined in 2 groups of elderly subjects with and without activity participation (AP). A survey was conducted with 321 elderly subjects over 65 years of age using a 24-item questionnaire regarding personality and depressive inclination and the visual analogue scale-happiness to measure QOL. The AP group was involved in 5 types of activity: community center activity course, learning and lecture participation, club activity, elderly manpower service activity and other activities. The QOL of the AP group was significantly higher than the non-AP group as expected. The perceptual difference between the 2 groups obtained by the correspondence and cluster analyses was that although the elderly of the AP group were satisfied and not bored with their current life, this trend was not clear for the non-AP group. Among the 5 activity types, other activities, characterized as activities adhered to by participants over a long period, showed the highest QOL compared with the 4 other types. In conclusion, the AP of the elderly should be encouraged, and continuing AP might be an important factor in improving QOL of the elderly

    Effect of fentanyl for preterm infants on mechanical ventilation: a systematic review and meta-analysis

    Introduction: Because excessive physical stress is harmful, reducing pain and discomfort in premature neonates during mechanical ventilation is a major challenge for physicians. There are no consensus and systematic review on the use of fentanyl, the most commonly used pain reliever in preterm neonates during mechanical ventilation. We aim to compare the benefits and harms of fentanyl versus placebo or no drug for preterm neonates receiving mechanical ventilation. Methods: A systematic review of randomized controlled trials (RCTs) was conducted according to the Cochrane Handbook for Systematic Reviews of Interventions. The systematic review was reported using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. Scientific databases such as MEDLINE, Embase, CENTRAL, and CINAHL were searched. All preterm infants on mechanical ventilation and enrolled in an RCT of fentanyl versus control were included. Results: Of 256 reports initially retrieved, 4 reports met the eligibility criteria. Fentanyl was not associated with mortality risk compared to the control (risk ratio: 0.72, 95% confidence intervals [CIs]: 0.36–1.44). No increase in ventilation duration (mean difference [MD]: 0.04, 95% CIs: −0.63–0.71) and no effect on hospital stay length (MD: 4.00, 95% CIs: −7.12–15.12) were found. Fentanyl intervention does not affect any other morbidities, including bronchopulmonary dysplasia, periventricular leukomalacia, patent ductus arteriosus, intraventricular hemorrhage (IVH), severe IVH, sepsis, and necrotizing enterocolitis. Conclusion: The present systematic review and meta-analysis failed to demonstrate the benefit of administering fentanyl to preterm infants on mechanical ventilation in mortality and morbidities. Follow-up studies are required to investigate the long-term neurodevelopment of the children

    Stress Tolerance in Subjects with Myocardial Infarction

    In our course of surveys to study the relationship between stress and myocardial infarction (MI), we have investigated whether stress tolerance may play a significant role when MI develops. To elucidate the possible role of stress tolerance, we examined 96 patients with MI (68 men and 28 women, age 40-90 years) with a stress tolerance check list in which stress tolerance was quantified as total points for 20 self-evaluated items, each scored between 1 and 4 points. Their scores were compared with control values obtained from persons not suffering from MI, and further analyzed by dividing them into 3 groups according to their grade of stress tolerance; that is, low (39 or less in score), moderate (40-49) and high (50-80). The mean stress tolerance score (± SD) was significantly lower in the 96 MI patients (49.5 ± 8.90 points) than in the controls (60.8 ± 7.3 points). The stress tolerance grade was low in 13 of the 96 (13.5%), moderate in 34 (35.5%) and high in 49 (51.0%). In MI patients, lowly or moderately stress-tolerant persons were significantly higher in percentage (49.0%) than in the controls (6.3%). These results clearly indicate that low stress tolerance is a risk factor in the development of MI

    電子レンジによるじゃがいもの調理性と食味について

    電子レンジを用いて加熱したじゃがいもの調理性や食味について調べ従来の加熱法と比較検討し,二次的調理操作とのつながりをもとに電子レンジの効果的使用法についても考察を加えた。(1)じゃがいもを電子レンジで加熱する場合の形状は加熱時間および食味に関係し,丸のままの加熱が食味良く,形状が細分化されるにつれ加熱時間は短縮されるが食味の低下をまねく。(2)電子レンジ加熱では水煮形式(B)において従来法(A)と同程度の粉ふき状態のものが得られた。しかし塩化ビニリデンフィルム被覆(C)は水分蒸発現象が表面を硬化させ粉ふき状態は著しく劣った。官能検査ではA-C, B-C間に有意差を認めA-B間には差は認められず,(C)は粉ふきいもの前処理としては不適であった。(3)マッシュ操作の難易度をその仕事量(W)で比較すると従来法(A)1に対し,電子レンジ加熱の(B)は1.72,(C)は2.97を要した。(C")の過剰加熱となるとマッシュ操作は更に困難となり残査量も多い。マッシュ時に大きな力が加わると細胞膜の破壊も進み粘性も増大する。(4)マッシュポテトの粘性を懸濁液の沈降体積から推測するとA<B<Cの順に高く,塩化ビニリデンフィルム被覆の電子レンジ加熱が最も高い粘性を示した。(5)マッシュポテトの官能検査では,水っぽさ,色の項目についてA-C, B-C間に有意差が認められ,いずれも水煮形式のものが低い評価となった。総合評価では塩化ビニリデンフィルムで被覆した電子レンジ加熱(C)が最も高く評価された。前記の粘性が食味の上に影響を与えていない結果となったが今回は漉されたままのマッシュポテトを試料としたためと考えられる。マッシュポテトの場合は更につぎの段階で混ぜる,練る,液状にのばす等の調理操作が加わることが多いが,加熱法別のマッシュポテトの調理性を知りこれらの調理条件にあった前処理加熱法を行うことが望まれる
