Mean age of participents was 64.2 yr and the mean diabetes duration was 13.3 yr. The mean HbA1c va1ue in the who1e popu1ation was 7.41 土 1.43% (means土 SD)，and 137 subjects (41.9%) had HbA1c be10w 7.0%. The mean LDL cho1estero1 and trig1ycerides 1eve1s were 2.75土 0.70and 1. 22 :t O. .67 mmo1/1， respective1y， and 193 subjects (59.0%) had LDL cho1estero1 be10w 3.1 and trig1ycerides be-10w 1. 7 mmol/l. The mean systolic and diasto1ic b100d pressure were 133.0 :t 17.8 and 71.9 :t 9.5 mmHg， respective1y， and 125 subjects (38.2%) had systo1ic b100d pressure be-10w 130 and diastolic b100d pressure be10w 85 mmHg. Thus， in the who1e popu1ation， 39 patients (11.9%) had good contro1 of hyperg1ycemia， hyperlipidemia， and b100d pressure. The present resu1ts will serve as a basis for future efforts to improve the qua1ity of dia-betes care and to reach one of the main goa1s in diabetes care. はじめに糖尿病患者のQOLを阻害する代表的な慢性合併症として細小血管症と大血管障害が重要である.長期にわたる良好な血糖コントロールが1型および2型糖尿病患者の細小血管症の発症を予防するとともに，その進展を阻止することはよく知られた事実であるl引.脂質代謝異常も大血管障害を促進し，高コレステロール血症や高トリグリセリド血症は冠動脈疾患の主要な原因であり，糖糖尿病における脂質および血圧 219 尿病患者においても冠動脈疾患の発症や進展を促進し死亡率を高めることが知られているか8) いくつかの大規模なスタディー，すなわちHe1sinkiHeart Study9)， Scandinavian Simvastatin Sur-viva1 StudyIO-I1l，そしてCho1estero1and Recur-rent Events Studyl2)は，糖尿病患者において血清指質をコントロールすることにより冠動脈疾患の2次予訪が可能なことを示した.同様に長期の大規模なスタディーにより厳密な血圧コントロールにより2型糖尿病患者の縮小血管症および大血管障害いずれの進展も姐止されることが，最近明らかにされてきた13) このような事実の蓄積により，糖尿病患者においては血糖のコントロールはいうまでもなく，同時に血清脂質や血圧の管理が重要視されるようになってきた.しかし，現在のところ糖尿病患者において血糖，血清脂質，車圧すべてについてのコントロール状態について検討された報告はほとんどみられない.そこで今回，われわれが管理している糖尿病患者について血糖，血清脂質，虫圧の管理状況について検討を加えた.対象および方法対象 擢病歴ならびに治療歴がl年以上である2型糖尿病患者327名(男性149名，女性178名)を対象とした.平均年齢は64.2蔵で糖尿病の平均擢病歴は13.3年であった.80名が食事療法(年齢は62.6:t 11.2歳で擢病歴は7.2:t5.7年)， 132名が経口血糖降下剤療法(年齢は66.2:t 10.5歳で躍病歴は13.9こと 8.3年)， 115名がインスリン治療(年齢は 63.1:t 11.1歳で躍病歴は16.8ごと9.0年)であった.最近測定した3田のHbA1cの平均値を血糖コントロールの指標とした.81名が抗脂血症剤 (57名がスタチン製剤， 9名がフィブラート製剤投与，その他は併用療法)， 91名が降圧剤 (39名がカルシウム桔抗剤， 16名がACE阻害剤，その他は併用療法)の投薬を受けていた.測定血清指質空腹時に採血し，総コレステロール， トリグリセリド， HDLコレステローlレを酵素法にて測定した.LDLコレステロールはFriedewa1dの計算式にて算出した14) 脂質レベルにより， 1群(LDLコレステロールが3.1mmo1/1未満，およびトリグリセリドが1.7mmo1/1未満)， I群 (LDLコレステロールが3.1-3.6mmol/l，およびトリグリセリドが1.7-2.3mmo1/1)，およびIII群(LDLコレステロールが3.6mmo1/1以上および，またはトリグリセリドが2.3mmo1/1以上)の3群に分類した.血圧 5分間の安静後，座位血圧を2回測定しその平均値を用いた.血圧レベルによりA群(収縮期温圧が130mm豆g未満，および拡張期血庄が85mmHg未満)， B群(収縮期血庄が130-139mmHg および拡張期臨庄が85-89mmHg) ， C群(収縮期血圧が140mmHg以上および，または拡張期血圧が90mmHg以上)の3群に分類した.データはすべて平均::tSDで表した.結果由糖のコントロール全仔U327例における平均耳bA1cは7.41:t 1. 43 %であり，そのうち137例 (41.9%)でHbA1cが7.0%未満であった.食事療法群の平均HbA1cは6.43土 0.990/0で，そのうち61開 (76.3%)がHbA1c7.0%未満，経口血糖降下剤治療群の平均HbA1cは7.57:t1.22%で，そのうち44例(33.3%)が7.0%未満，インスリン治療群の平均HbA1cは7.91:t1.41%で，そのうち32例(27.8%)が7.0%未満であった.血清指質のコント口ール(図1)全例における平均凪清LDLコレステロール，トリグリセリド{直は2.75土 0.70および1.22 :t 0.67 mmo1/1であり，そのうち193例 (59.0%)がI群であった.食事療法群の平均値は2.82土0.65および1.35 :t 0.71 mmo1/1で，そのうち43例 (53.8%)がI群であり，経口血糖降下剤治療群の平均値は2.83土 0.69および1.21 :t 0.68 mmo1/1で，そのうち77例 (58.3%)がI群であり，インスリン治療群の平均値は2.60:t mmHgで，そのうち50例 (37.9%)がA群であり，インスリン治療群の平均値は135.9土 17.0および72.6:t 9.3 mmHgで、あり，そのうち38例 (33.0%)がA群であった.自標歪IJ達率慢性合併症の予防または進展阻止に必要と考えられる良好な糖尿病のコントロール，すなわち糖尿病における脂質および血圧 221 HbA1cが7.0%未満， LDLコレステロールが3.1 mmo1/1未満， トリグリセリドが1.7mmo1/1未満，収縮期血圧が130mmHg未満，および拡張期血圧85 mmHg未満のすべてを満たす糖尿病患者は39例 (11.9%)であった.この39例中5例は， HDL コレステロールが1.0mmo1/1未満であった.考察今国のわれわれの糖尿病症例における平均HbA1cは7.41土 1.43%であり，そのうち137例(41. 9%)が，良好な血糖コントロールの指標とされる7.0%未満を達成していた.フィンランドのVal1eら15)による多施設についての症例をまとめた報告では，平均HbA1cが8.6二と 1.9%であり約25%の症例においてHbA1cが7.3%未満であったという.この成績と比較しでも，われわれの症例の血糖コントロールはかなり良好なものであった.糖尿病においては血庇のコントロールも重要であり，縮小血管症や大血管障害を予防するための車圧管理に関しては， 1993年のConsensusState倫ment for Treatment of Hypertension in Diabetes および:'AmericanDiabetes Associationならびに1994年の Nationa1High B100d Pressure Educa-tion Program W orking Groupは，糖尿病患者における血圧は収縮期血圧140mmHg以上および，または拡張期血圧90mmHg以上であれば治療介入するべきで， 目標レベルは収縮期血圧は130mmHg未満および拡張期血庄85mmHg未満16-17)と勧告している.EURODIAB IDDM Comp1ica-tions StudylS)によれば， 1型糖尿病において高血圧が十分にコントロールされていたものはわずかに11，3%であったと報告されているが，今回の症例において，インスリン治療群における血圧コントロールの目標値到達率 (33.0%)は食事療法群のそれ (46.3%)に比較してやや低い傾向にあったものの，全例における平均収縮期血圧および拡張期血在は，それぞれ133.0土 17，8および71. 9土 9.5mmHgであり，そのうち125例(38，2%)において目標血圧レベルである収縮期血圧130mm狂g未満および拡張期温圧85mmHg 未満を達成していた.今回の成績は2型糖尿病患者におけるものでり，単純にEURODIABStudy と比較するわけにはし、かないが，かなり良好な血圧管理が行われていたことを示す成績であった.糖尿病患者における大血管揮害を予防または進展を阻止するための血清脂費レベルに関しては，LDLコレステロールが2.6mmo1/1未満， トリグリセリドが1，7mmo1/1未満，およびHDLコレステロールが男性では0.9mmo1/1以上，女性では1. 1 mmo1/1以上という勧告値が， National Cho1estero1 Education Program191およびAmeri-can Diabetes Association20)によって示されている. 日本においては臼本糖尿病学会および臼本動脈硬化学会により，糖尿病患者における血管合併症の予防および進展阻止のための嵐清指質レベルは， LDLコレステロールが3，1mmo1/1未満，トリグリセリドが1，7mmo1未満，およびHDLコレステロールが1.1mmo1/1以上のすべてを満たすことが望ましいと勧告されている.今回の症例において，平均血清LDLコレステロールおよびトリグリセ J)ド値はそれぞれ106.5:t 27，2 mg/dl および 108，9:t 59.1 mg/dlであり，全例中の193例 (59.0%)が目標到達レベル，すなわちLDLコレステロール3.1mmo1/1未満およびトリグリセリド 1.7mmol/l未満を満たしていた.Kantersら21)は，高脂血症を合併したl型および2型糖尿病患者95例に対して食事療法と抗脂血症剤投与により積極的に血清脂質レベルの正常化を目指した.その結果，多くの症例では抗脂血症剤の2剤ないし3剤による多剤併用療法を必要としたが，それだけやっても目標値到達率は66%にしか達しなかったと報告している.この報告を参考にすれば，今回の症例にみられた目標到達率59%という数字は比較的良好な結果であったと考えられる.以上より，糖尿病症例の40%以上において良好な血糖コントロール，すなわちHbA1c7.0%未満が達成されており，また欧米の成績に比較してそれぞれ単独でみた場合でも高脂血症および高血圧の目標値到達度は比較的良好なものであった.それにもかかわらず血糖，脂質，血圧のすべてにおいて良好なコントロールが得られている症例はわずかに10%であった.今回の成績はl施設における眼られた範囲での検討であり，今後より多くの施設についての詳細な検討が必要であると思われるが，糖尿病の予後改善を今まで以上に目指すためには，血清脂質および血圧レベルのより一層厳格な管理が必要である.222 池間匡他6名結 圭五回口2型糖尿病患者327例について血糖，血清脂質，血圧の自標値到達率について検討したが，これらすべての管理目標値を満たしているものはわずかに100/0であり，今後より一層厳格な血清脂質および血圧の管理が必要と考えられた.文献1) The Diabetes Contro1 and Complications Tria1 Research Group.
