    Current situation in providing medical care for burn injuries in Ukraine: public opinion

    The results of assessment of specialized medical assistance to burn victims are presented in the article, they being based not on standard statistical figures from burn units and centers but on anonymous online survey of 486 respondents (60.5% of them - the residents of Vinnytsya region). The aim of this questionnaire-based survey was to study the level of awareness of Ukrainian population regarding the principles of providing first aid for burn injuries, as well as the availability, quality and shortcomings of current burn care in medical institutions based on the assessment of discharged patients. For this purpose, a 37-item questionnaire was developed, being divided into some blocks: general information about respondents, pre-medical aid, first medical aid, qualified medical aid and specialized medical care. According to the questionnaire, 76.9% of respondents rated the quality of medical care 5 scores by 5-score system, 17.9% - 4 scores, 5.2% - 3 scores, being indicative of rather high level of burn care, even in conditions of limited material, technical and financial resources, as well as rapid decrease of confidence in doctors in general. Online survey conducted in every region of Ukraine seems to be reasonable to make adequate assessment of study problem in the country in general

    The aim of the work was a comparative analysis of clinical effectiveness of existing materials for temporary closure of burn wounds. The advantages of biological wound dressings over gauze or synthetic materials are obvious today. However, the issue of comparing the clinical effectiveness of allo- and xenografts remains debatable. The latter provide rapid reepithelialization, analgesia, wound protection and, as a rule, a favorable treatment outcome. In terms of personal experience, over the past 20 years, with the transition to active surgical tactics for the treatment of patients with burns, enough evidence has been accumulated to confirm the high efficiency of using xenodermografts from domestic pigs to close postoperative wounds. Regarding the advantages of animal derived materials, we can highlight the safety, affordable price, unlimited raw materials for their manufacture, as well as the avoidance of various moral, ethical and legal restrictions. Thus, closure of burn wounds is a major issue in the treatment of thermal injuries, which is especially critical for deep and extensive burns.. Xenoderm grafts of our own design, which are widely used in clinical practice and significantly improve the course of burn disease are among the existing and most effective dressings in Ukraine.     Improving the effectiveness of the skin integrity restoring after deep burns by platelet-enriched plasma application

    Objective. To study the effectiveness of platelet-enriched plasma application in patients with deep burns at the stage of wounds preparation for autodermoplasty. Materials and methods. The study was performed on 23 patients with burns IIA, IIB and III degrees taking 25 - 40% of the body surface. Comprehensive local treatment of patients of the main group included injection of platelet-enriched plasma at the stage of preparation of postoperative wounds for autodermoplasty. In patients of the control group, local treatment was limited by dressings with antiseptic solution. Examination of patients included visual inspection of the damaged area, microbiological examination of wound contents and histological examination of damaged tissues. Results. Applying the suggested method of burn wounds local treatment resulted in reducing time of wounds preparation for closure by autologous skin grafts, reducing of their healing period, decreasing of tissues inflammatory activity, blood flow improvement, and stimulating processes of tissue granulation, proliferation, epithelialization. Conclusions. The application of the autologous platelet-enriched plasma can be considered as an effective biotechnology that can increase the effectiveness of the skin integrity restoring in patients with burns

    The article outlines the determinants of the state fiscal policy at the present stage of global transformations. Using the principles of financial science it is determined that regulation of financial flows within the fiscal sphere, namely centralization and redistribution of the GDP, which results in the regulation of the financial capacity of economic agents, is of importance. It is emphasized that the urgent measure for improving the tax model is re-considering the provision of fiscal incentives, which are used to stimulate the accumulation of capital, investment activity, innovation, increase of the competitiveness of national products, expansion of exports, increase of the level of the population employment. The necessity of applying the instruments of fiscal regulation of financial flows, which should take place on the basis of institutional economics emphasizing the analysis of institutional changes, the evolution of institutions and their impact on the behavior of participants of economic relations. At the same time it is determined that the maximum effect of fiscal regulation of financial flows is ensured when application of fiscal instruments is aimed not only at achieving the target values of parameters of financial flows but at overcoming institutional deformations as well. It is determined that the optimal movement of financial flows enables creating favorable conditions for development and maintenance of financial balance in the society and achievement of the necessary level of competitiveness of the national economy
