Introduction: Infected diabetic foot ulcers may lead to serious complications if not recognised in the early stage. Diagnosis of infection is particularly challenging at that stage; thus, a sensitive inflammatory biomarker may be helpful. We aimed to evaluate the role of procalcitonin (PCT) as an early biomarker for infected diabetic foot ulcers (IDFU). Materials and method: This cross-sectional study was conducted at Klinik Rawatan Keluarga (KRK), Orthopedic clinic and wards in Hospital Universiti Sains Malaysia (USM) from May 2020 to December 2020. A total of 264 participants were recruited and divided into three groups: 50 diabetic patients with no ulcers (control), 107 patients with non-infected diabetic foot ulcers (NIDFU), and 107 patients with infected diabetic foot ulcers (IDFU). The level of PCT was taken for all patients. Total white count (TWC) and C- reactive protein (CRP) were taken only for IDFU patients. Diagnosis of infection was based on the Infectious Disease Society of America-International Working Group of Diabetic Foot (IDSA-IMWGDF), and the severity of infection was graded according to the Wagner Classification. Results: The level of PCT was higher in IDFU than in NIDFU and diabetic patient, with a median (IQR) of 0.355 (0.63) ng/mL, 0.077 (0.15) ng/mL and 0.028 (0.02) ng/mL, respectively. PCT and CRP showed moderate positive correlations in IDFU patients (p<0.001). The sensitivity and specificity were 63.6% and 83.2%, respectively, at the best cut-off at 0.25 ng/mL. Conclusion: PCT is a valuable biomarker for the diagnosis of infection; however, it adds little value in the early diagnosis of IDFU in view of its low sensitivity

The use of advance computer-based technology is becoming necessary to address the growing complexity of human problems and enhance effective communication. The recent pandemic COVID-19 not only induces many morbidities and mortalities but also intensifies mental health problem worldwide. Due to the increasing benefits of virtual reality (VR) in addressing medical condition, it is believed that VR can be used as a diagnostic tool to assess numerous medical conditions and psychiatric disorders. To date, there is still scarce evidence of VR as a diagnostic tool to assess obsessive compulsive disorder (OCD). In this study, we had conducted a systematic review to investigate the use of VR as a diagnostic tool for OCD and assess its benefits and weaknesses in comparison to computer-assisted tools. Comprehensive searches of electronic databases including PubMed and Google Scholar were undertaken to discover peer review evidence of computer-based simulation tasks in detecting OCD symptoms. Twelve out of 9325 papers were screened and reviewed. Five articles reported on computerised tools and seven articles described VR tools. In comparison to computer-based tasks, VR is a promising assessment tool due to specific virtual environments and high resolutions which are able to induce anxiety symptoms. Despite numerous shortcomings, assessment can be utilised in computerised form to detect and generate a variety of psychiatry diagnoses among the general population. Although computerised assessment task and VR show promising results, the finding are uneven due to study design differences, wide variability content task use, small sample size, several methodological issue with the computerised tasks and lack of appropriate control groups. In conclusion, the choice to use computerisation or VR for OCD assessment will depend on aim, content, technical equipment and budget. More in-depth studies of these issues are required

Addressing triggering post zancolli lasso procedure

A claw hand causes disability as kinematics are affected due to hyperextension at the metacarpophalangeal joints. Zancolli lasso procedure is a simple tenodesis procedure which effectively lessens clawing to allow better grip. We present a56-year-old lady who had a history of trauma with progressive clawing of her left hand. She has been diagnosed with partially recovered incomplete lower trunk brachial plexus injury. She underwent successful Zancolli lasso procedures for all of her fingers but 8 months later, the patient developed triggering of the index and middle fingers. We experimented by releasing the adhesions in one finger and releasing the whole A1 pulley together with the lasso-ed flexor digitorum superficialis (FDS) in the other finger and the latter worked. We repeated the procedure in the index finger and the triggering resolved. Although both her index and middle fingers now have a flexor digitorum profundus (FDP) only (the FDS having retracted proximally), she did not have a recurrence of her clawing. We attributed the triggering due to increasing A1 pulley volume as well as contractures causing post-release functional positions

Background. Spontaneous labor (normal delivery) is a process to pull out aterm conception product that consist of the baby itself, placental, amnion sac with head presentation (back of the head position), from uterus through birth canal ( either soft or rogged canal) with the power of mother itself (without any intervention from outside). After experiencing a tense and exhausting events. Mother who have labor need to get the best possible care especially in this periode (post partum). The Objective of the experiment is to occupy and maintain a rest full of sleep after normal labor in post partum periode to restourage enough energy for daily activities. The Author conduct result on fabruary 2017 in Gedongan Baki Sukaharjo. Method chosen by the author is observation, interview, and documentation. Result of the research find a disruption of sleep patterns assosiated with responsibility to provide infant care and anxiety because breast milk has not come out after delivery until the second days. Interventions, identification about sleep disosder and etiology of sleep disosder. Explain the medication aggaints the sleep pattern, discuss about technique of sleep pattern that can reduce anxiety, observation and note sleep requirment ofNy. S everyday. After 3 days of nursing intervention, disruption of sleep patterns have been solved, so the intervention need to terminated. The next plan of intervention is to provide support and motivation for her to get enough sleep and taking some rest. And also recommend Ny. S to preserve itensity of rest in order to maintain sleep patterns

CpG-free plasmids confer reduced inflammation and sustained pulmonary gene expression.

Pulmonary delivery of plasmid DNA (pDNA)/cationic liposome complexes is associated with an acute unmethylated CG dinucleotide (CpG)-mediated inflammatory response and brief duration of transgene expression. We demonstrate that retention of even a single CpG in pDNA is sufficient to elicit an inflammatory response, whereas CpG-free pDNA vectors do not. Using a CpG-free pDNA expression vector, we achieved sustained (≥56 d) in vivo transgene expression in the absence of lung inflammation
