Impact of two rounds of praziquantel mass drug administration on Schistosoma mansoni infection prevalence and intensity: a comparison between community wide treatment and school based treatment in western Kenya
AbstractThis study compared the effectiveness of the community-wide treatment and school-based treatment approaches in the control of Schistosoma mansoni infections in villages with ⩾25% prevalence in western Kenya. Stool samples from first year students, 9–12year olds and adults (20–55years) were analyzed by the Kato–Katz technique for S. mansoni eggs. After two rounds of treatment, S. mansoni prevalence and intensity levels significantly declined in both treatment approaches. Prevalence comparisons between the two approaches did not show any significant differences following treatment. However, infection intensity levels in the 9–12year old school-attending pupils were significantly higher in the community-wide treatment arm than in the school-based treatment arm. Nevertheless, significant reductions in S. mansoni infection prevalence and intensity levels were achieved among school-age children regardless of the treatment approach used

High temperatures can affect the survival, establishment and symbiotic properties of Rhizobium strains. Bean nodulating Rhizobium strains are considered particularly sensitive because on this strains genetic recombinations and/or deletions occur frequently, thus compromising the use of these bacteria as inoculants. In this study R. tropici and R. leguminosarum bv. phaseoli strains isolated from Cerrado soils were exposed to thermal stress and the strains’ growth, survival and symbiotic relationships as well as alterations in their genotypic and phenotypic characteristics were analyzed. After successive thermal shocks at 45ÂşC for four hours, survival capacity appeared to be strain-specific, independent of thermo-tolerance and was more apparent in R. tropici strains. Certain R. leguminosarum bv. phaseoli strains had significant alterations in plant dry weight and DNA patterns obtained by AP-PCR method. R. tropici strains (with the exception of FJ2.21) were more stable than R. leguminosarum bv. phaseoli strains because no significant phenotypic alterations were observed following thermal treatments and they maintained their original genotypic pattern after inoculation in plants.<br>Altas temperaturas podem afetar a sobrevivĂŞncia, estabelecimento e as propriedades simbiĂłticas em estirpes de Rhizobium. As estirpes capazes de nodular o feijoeiro tĂŞm sido consideradas particularmente sensĂveis, porque nessas estirpes Ă© comum a ocorrĂŞncia de recombinações e/ou deleções genĂ´micas comprometendo, muitas vezes, a sua utilização como inoculantes. Neste trabalho, procurou-se avaliar a capacidade de crescimento e sobrevivĂŞncia em temperaturas elevadas de estirpes de Rhizobium efetivas na fixação de nitrogĂŞnio no feijoeiro isoladas dos cerrados, bem como avaliar suas caracterĂsticas fenotĂpicas e genotĂpicas apĂłs choque tĂ©rmico. A capacidade de sobrevivĂŞncia Ă temperaturas elevadas, avaliada apĂłs choques tĂ©rmicos sucessivos (45ÂşC por 4 horas) mostrou ser uma caracterĂstica prĂłpria de cada estirpe, independente de sua termotolerância, que aparentemente foi mais acentuada nas estirpes de R. tropici. Algumas estirpes de R. leguminosarum bv. phaseoli mostraram alterações significativas (Duncan 5% de probabilidade) nas suas caracterĂsticas fenotĂpicas (produção de matĂ©ria seca) apĂłs choques tĂ©rmicos e nos seus padrões genĂ´micos evidenciados pela tĂ©cnica de AP-PCR. The ASOS Surgical Risk Calculator: development and validation of a tool for identifying African surgical patients at risk of severe postoperative complications
Background:
The African Surgical Outcomes Study (ASOS) showed that surgical patients in Africa have a mortality twice the global average. Existing risk assessment tools are not valid for use in this population because the pattern of risk for poor outcomes differs from high-income countries. The objective of this study was to derive and validate a simple, preoperative risk stratification tool to identify African surgical patients at risk for in-hospital postoperative mortality and severe complications.
Methods:
ASOS was a 7-day prospective cohort study of adult patients undergoing surgery in Africa. The ASOS Surgical Risk Calculator was constructed with a multivariable logistic regression model for the outcome of in-hospital mortality and severe postoperative complications. The following preoperative risk factors were entered into the model; age, sex, smoking status, ASA physical status, preoperative chronic comorbid conditions, indication for surgery, urgency, severity, and type of surgery.
Results:
The model was derived from 8799 patients from 168 African hospitals. The composite outcome of severe postoperative complications and death occurred in 423/8799 (4.8%) patients. The ASOS Surgical Risk Calculator includes the following risk factors: age, ASA physical status, indication for surgery, urgency, severity, and type of surgery. The model showed good discrimination with an area under the receiver operating characteristic curve of 0.805 and good calibration with c-statistic corrected for optimism of 0.784.
Conclusions:
This simple preoperative risk calculator could be used to identify high-risk surgical patients in African hospitals and facilitate increased postoperative surveillance.
