    Efficacy of micronized progesterone-containing transdermal gel in the treatment of fibrocystic mastopathy: Results of a Russian study

    Objective. To evaluate the effect of progestogel as monotherapy on quality of life and magnitude of pain syndrome in fibrocystic mastopathy (FMP). Subjects and methods. The time course of changes in its symptoms was estimated using the pain visual analogue scale (VAS), the SF-6 health survey questionnaire, and ultrasonography in 798 patients diagnosed with FMP. Results. The pain VAS scores were 5.7 (5.0; 7.0) at baseline and statistically significantly lower [(2.4 (1.0; 4.0)] and [1.0 (0.0; 2.0)] after 3 and 6 months, respectively (p<0.05). The rate of diffuse and nodular changes was decreased by 44.3 and 60.4%, respectively. That of cystic masses ≥10 mm or less was reduced by 61.8 and 4.3%, respectively. There were physical and emotional improvements. Conclusion. Three-month progestogel monotherapy for FMP contributes to a decline in the number and extent of cystic changes, significantly reduces the magnitude of pain syndrome, and improves the quality of physical and mental health. © Bionika Media Ltd

    Medical library. medications – common over the counter medications the impact of a topical glucocorticoid flutikazon on the dynamics of the local immunity indices and microbiocenosis condition of the nasal mucosa was studied in the patients with the confirmed diagnosis of all the year round allergic rhinitis. In a year all patients demonstrated a sustained control of all the year round allergic rhinitis symptoms. A statistically significant increase of lysozyme, secretory and interferon γ concentration in the nasal and mouth mucous secretion as well as the decrease of tumor necrosis factor (TNF) α concentration and the frequency of yeast like fungi and gram negative bacteria growth were revealed. Thus, long term flutikazon therapy has brought about a sustained remission of all the year round allergic rhinitis clinical manifestations as well as a significant improvement of microbiocenosis indices of the upper respiratory mucous membranes and local immunity indices.Key words: allergic rhinitis, local immunity, mucous membranes microbiocenosis, topical glucocorticoids

    Shell-type SNRs as sources of cosmic rays

    Investigations of VHE gamma-ray sources by any methods, including mirror Cherenkov telescopes, touch on the problem of the cosmic ray origin and, accordingly, the role of the Galaxy in their generation. SHALON observations have yielded results on Galactic supernova remnants (SNR) of different ages. Among them are: the shell-type SNRs Tycho's SNR (1572y), Cas A (1680y), IC 443 (age ∼ (3 ÷ 30) × 103 y), Cygni SNR (age ∼ (5 ÷ 7) × 103 y), G166.0 + 4.3 (age ∼ 24 × 103 y) and the classical nova GK Per (Nova 1901). Observation results are presented for each of the SNRs with spectral energy distributions by SHALON in comparison with other experiment data and images by SHALON together with data from X-rays by Chandra and radio-data by CGPS. The collected experimental data have confirmed the prediction of the theory about the hadronic generation mechanism of very high energy 800 GeV–100 TeV gamma-rays in Tycho's SNR, Cas A and IC443. For the first time, unique data on GK Per (Nova1901) TeV gamma-ray emission were obtained with the SHALON experiment. The X-ray data shows that the nova remnant of GK Per could be a younger remnant that will resemble older SNRs like IC 443 which interact with molecular clouds. GK Per is supposed to be a candidate for TeV gamma-ray emission due to accelerated particles in the reverse shock region
