A global outlook to the interruption of education due to COVID-19 Pandemic: Navigating in a time of uncertainty and crisis
Uncertain times require prompt reflexes to survive and this study is a collaborative reflex to
better understand uncertainty and navigate through it. The Coronavirus (Covid-19) pandemic hit hard
and interrupted many dimensions of our lives, particularly education. As a response to interruption of
education due to the Covid-19 pandemic, this study is a collaborative reaction that narrates the overall
view, reflections from the K12 and higher educational landscape, lessons learned and suggestions from
a total of 31 countries across the world with a representation of 62.7% of the whole world population. In
addition to the value of each case by country, the synthesis of this research suggests that the current
practices can be defined as emergency remote education and this practice is different from planned
practices such as distance education, online learning or other derivations. Above all, this study points
out how social injustice, inequity and the digital divide have been exacerbated during the pandemic and
need unique and targeted measures if they are to be addressed. While there are support communities
and mechanisms, parents are overburdened between regular daily/professional duties and emerging
educational roles, and all parties are experiencing trauma, psychological pressure and anxiety to various
degrees, which necessitates a pedagogy of care, affection and empathy. In terms of educational
processes, the interruption of education signifies the importance of openness in education and highlights
issues that should be taken into consideration such as using alternative assessment and evaluation
methods as well as concerns about surveillance, ethics, and data privacy resulting from nearly exclusive
dependency on online solutions

OCT-Angiography in Early Choroidal Melanoma and Choroidal Nevi
Purpose: To study the features of small choroidal melanoma and choroidal nevi angioarchitectonics using the optical coherence tomography angiography (OCTA).Patients and methods. We examined 128 patients with small choroidal neoplasms: 41 β with small choroidal melanoma (group 1), 42 β with suspicious nevi (group 2) and 45 β with benign nevi (group 3). All patients underwent general ophthalmic examinations and special instrumental diagnostic methods (ultrasound examination (US), fluorescence angiography (FA), spectral optical coherence tomography (SOCT), OCTA).Results. OCTA in all patients with small choroidal melanoma showed neovascular network under retinal pigment epithelium. We found a looped, cranked-twisted, heterogeneous vascular network with uneven clearance and with numerous bends and weaves, located under the vessels of retina. The limiting avascular zone corresponding to the tumor slope was determined in 19 (46.3 %) of 41 cases. There was a rim of dilated hyperreflective choriocapillaries on the periphery of the tumor. We identified hyperreflective homogeneous enlarged choriocapillaries in the focus area with a brighter glow than the surrounding choriocapillaries in 39 (92.9 %) of 42 patients with suspicious choroidal nevus. We detected an avascular zone with surrounding extended hyperreflective choriocapillaries in the center of the nevus in 3 (7.1 %) of 42 cases. We diagnosed homogeneous isoreflective choriocapillaries similar in brightness to the surrounding vessels in all 45 patients with benign nevi.Conclusions. Thus, the complex of clinical and instrumental methods, including OCTA, makes it possible to establish the diagnosis of malignant tumor of the choroid in early stages. At the same time, OCTA: 1) allows to visualize of the tumor vessels in the choroidal layer in 100 % of cases of in small choroidal melanoma; 2) makes it possible to distinguish the newly formed tumor vessels from the choriocapillaries in case of small choroidal melanoma; 3) provides an opportunity to establish the correct diagnosis and provide timely assistance to patients with benign and malignant choroidal tumors
The effect of prolonged oxygen therapy on the clinical course of the disease and diaphragm function in patients with chronic obstructive pulmonary disease and hypoxemic respiratory failure (three-year follow-up)
The objective: to determine the eο¬ect of prolonged oxygen therapy (POT) on changes in the clinical and functional status of patients with chronic obstructive pulmonary disease (COPD) and severe hypoxemic chronic respiratory failure.Subjects and methods. 30 patients with COPD were examined during the period of remission of the disease, the average age was 64.8 Β± 6.9 years. After the initial examination, patients were prescribed POT at home for 16 hours/day. As a basic therapy, all patients received a combined bronchodilator of long-acting indacaterol/glycopyrronium at a dose of 110/50 ΞΌg once a day in combination with beclomethasone dipropionate (BDP) at the dose of 200-400 ΞΌg/day. The patients were examined repeatedly 3 times with a 1-year interval. Clinical indicators, gas composition of arterial blood, results of spirometry and ultrasound examination of the diaphragm were evaluated.Results. Initially, disorders of the functional state of the diaphragm were detected in COPD patients with hypoxemia (decrease in the thickening fraction and relaxation rate). After the ο¬rst year of using POT, an increase in the rates of contraction and relaxation of the diaphragm muscle under tidal respiration was observed. On the 2nd and 3rd year of using POT, it was noted that indicators of the functional state of the diaphragm stabilized. As a result of POT, the number of exacerbations went down and mortality in COPD patients with hypoxemia decreased. Against the background of basic therapy with indacaterol/glycopyrronium in combination with low doses of BDP, no adverse events were observed for the entire period of follow-up.Conclusions. POT in combination with a double long-acting bronchodilator and low-dose inhaled glucocorticosteroids in COPD patients with hypoxemic chronic respiratory failure helps to reduce dyspnea, the number of severe exacerbations, as well as improve pulmonary ventilation functions, blood gas content, and the diaphragm function during the ο¬rst year of its use. Further, the indicators continued to stabilize
Simultaneous topography-guided photorefractive keratectomy with accelerated collagen cross-linking in the treatment of stage I keratoconus
Purpose: to evaluate the efficiency of simultaneous topography-guided photorefractive keratectomy (PRK) with accelerated collagen cross-linking in the treatment of stage I keratoconus.Material and methods. 38 patients (49 eyes) aged 18 to 44 with stage I keratoconus were treated with simultaneous topography-guided PRK combined with accelerated corneal collagen crosslinking. Along with standard ophthalmological testing, patients underwent aberrometry, confocal microscopy, optical coherence tomography of the cornea, imaging of the anterior eye section on a Scheimpflug analyzer, and examination of the biomechanical properties of the cornea. The follow-up period was 2 years.Results. Two years after surgery, uncorrected visual acuity which originally was 0.23 Β± 0.03 showed a statistically significant increase and reached 0.72 Β± 0.02; best corrected visual acuity improved from 0.44 Β± 0.03 to 0.89 Β± 0.02; the refractive power of the cornea fell from 45.03 Β± 0.28 to 42.55 Β± 0.31 D; the spherical component of refraction reduced from -2.00 Β± 0.19 to -0.73 Β± 0.09 D; the cylinder component reduced from 2.46 Β± 0.14 to 0.79 Β± 0.07 mm, the thinnest point of the cornea reduced from 486.30 Β± 4.78 to 406.80 Β± 6.51 ΞΌm. The highest order aberrations showed the following results: Coma index reduced from 0.27 Β± 0.12 to 0.08 Β± 0.07, Tilt reduced from 0.53 Β± 0.11 to 0.10 Β± 0,09, and Trefoil reduced from 0.13 Β± 0.04 to 0.05 Β± 0.02.Conclusion. The analysis of clinical and functional results, biomechanical properties and structures of the cornea confirmed the efficacy and safety of the combined treatment of patients with stage I keratoconus

Correction of microbiocenosis of female reproductive tract during the late stage of pregnancy
Pobedi str., 85, Belgorod, Russia, 308024 Bacteriological examination of cervical canal at 34 women in 38β40 weeks of gestation has been made before and after intravaginal usage of Gynoflor (βMedinovaβ, Switzerland), containing 50 mg of Lactobacillus acidophilius and 0,03 mg of estriol. It is established, that after usage of Gynoflor significant decrease of number of patients with Staphylococcus epidermidis and Peptococcus has been observed. Furthermore, lowering of number of women with other opportunistic bacteria and yeasts ( Staphylococcus aureus , Klebsiella pneumonia , Enterococcus , Enterobacter , Streptococcus , Bacteroides , Candida albicans ) has been appeared. It was accompanied with decrease of colony forming units number among all opportunistic species
