SpliceMiner: a high-throughput database implementation of the NCBI Evidence Viewer for microarray splice variant analysis
BACKGROUND: There are many fewer genes in the human genome than there are expressed transcripts. Alternative splicing is the reason. Alternatively spliced transcripts are often specific to tissue type, developmental stage, environmental condition, or disease state. Accurate analysis of microarray expression data and design of new arrays for alternative splicing require assessment of probes at the sequence and exon levels. DESCRIPTION: SpliceMiner is a web interface for querying Evidence Viewer Database (EVDB). EVDB is a comprehensive, non-redundant compendium of splice variant data for human genes. We constructed EVDB as a queryable implementation of the NCBI Evidence Viewer (EV). EVDB is based on data obtained from NCBI Entrez Gene and EV. The automated EVDB build process uses only complete coding sequences, which may or may not include partial or complete 5' and 3' UTRs, and filters redundant splice variants. Unlike EV, which supports only one-at-a-time queries, SpliceMiner supports high-throughput batch queries and provides results in an easily parsable format. SpliceMiner maps probes to splice variants, effectively delineating the variants identified by a probe. CONCLUSION: EVDB can be queried by gene symbol, genomic coordinates, or probe sequence via a user-friendly web-based tool we call SpliceMiner (). The EVDB/SpliceMiner combination provides an interface with human splice variant information and, going beyond the very valuable NCBI Evidence Viewer, supports fluent, high-throughput analysis. Integration of EVDB information into microarray analysis and design pipelines has the potential to improve the analysis and bioinformatic interpretation of gene expression data, for both batch and interactive processing. For example, whenever a gene expression value is recognized as important or appears anomalous in a microarray experiment, the interactive mode of SpliceMiner can be used quickly and easily to check for possible splice variant issues
Amenability of algebras of approximable operators
We give a necessary and sufficient condition for amenability of the Banach
algebra of approximable operators on a Banach space. We further investigate the
relationship between amenability of this algebra and factorization of
operators, strengthening known results and developing new techniques to
determine whether or not a given Banach space carries an amenable algebra of
approximable operators. Using these techniques, we are able to show, among
other things, the non-amenability of the algebra of approximable operators on

Disclosure of Maternal HIV Status to Children: To Tell or Not To Tell . . . That Is the Question
HIV-infected mothers face the challenging decision of whether to disclose their serostatus to their children. From the perspective of both mother and child, we explored the process of disclosure, providing descriptive information and examining the relationships among disclosure, demographic variables, and child adjustment. Participants were 23 mothers and one of their noninfected children (9 to 16 years of age). Sixty-one percent of mothers disclosed. Consistent with previous research, disclosure was not related to child functioning. However, children sworn to secrecy demonstrated lower social competence and more externalizing problems. Differential disclosure, which occurred in one-third of the families, was associated with higher levels of depressive and anxiety symptoms. Finally, knowing more than mothers had themselves disclosed was related to child maladjustment across multiple domains. Clinical implications and the need for future research are considered

The Schroedinger Problem, Levy Processes Noise in Relativistic Quantum Mechanics
The main purpose of the paper is an essentially probabilistic analysis of
relativistic quantum mechanics. It is based on the assumption that whenever
probability distributions arise, there exists a stochastic process that is
either responsible for temporal evolution of a given measure or preserves the
measure in the stationary case. Our departure point is the so-called
Schr\"{o}dinger problem of probabilistic evolution, which provides for a unique
Markov stochastic interpolation between any given pair of boundary probability
densities for a process covering a fixed, finite duration of time, provided we
have decided a priori what kind of primordial dynamical semigroup transition
mechanism is involved. In the nonrelativistic theory, including quantum
mechanics, Feyman-Kac-like kernels are the building blocks for suitable
transition probability densities of the process. In the standard "free" case
(Feynman-Kac potential equal to zero) the familiar Wiener noise is recovered.
In the framework of the Schr\"{o}dinger problem, the "free noise" can also be
extended to any infinitely divisible probability law, as covered by the
L\'{e}vy-Khintchine formula. Since the relativistic Hamiltonians
and are known to generate such laws, we focus on
them for the analysis of probabilistic phenomena, which are shown to be
associated with the relativistic wave (D'Alembert) and matter-wave
(Klein-Gordon) equations, respectively. We show that such stochastic processes
exist and are spatial jump processes. In general, in the presence of external
potentials, they do not share the Markov property, except for stationary
situations. A concrete example of the pseudodifferential Cauchy-Schr\"{o}dinger

Natural boundaries for the Smoluchowski equation and affiliated diffusion processes
The Schr\"{o}dinger problem of deducing the microscopic dynamics from the
input-output statistics data is known to admit a solution in terms of Markov
diffusions. The uniqueness of solution is found linked to the natural
boundaries respected by the underlying random motion. By choosing a reference
Smoluchowski diffusion process, we automatically fix the Feynman-Kac potential
and the field of local accelerations it induces. We generate the family of
affiliated diffusions with the same local dynamics, but different inaccessible
boundaries on finite, semi-infinite and infinite domains. For each diffusion
process a unique Feynman-Kac kernel is obtained by the constrained (Dirichlet
boundary data) Wiener path integration.As a by-product of the discussion, we
give an overview of the problem of inaccessible boundaries for the diffusion
and bring together (sometimes viewed from unexpected angles) results which are
little known, and dispersed in publications from scarcely communicating areas

Psychosocial factors and their predictive value in chiropractic patients with low back pain: a prospective inception cohort study
BACKGROUND: Being able to estimate the likelihood of poor recovery from episodes of back pain is important for care. Studies of psychosocial factors in inception cohorts in general practice and occupational populations have begun to make inroads to these problems. However, no studies have yet investigated this in chiropractic patients. METHODS: A prospective inception cohort study of patients presenting to a UK chiropractic practice for new episodes of non-specific low back pain (LBP) was conducted. Baseline questionnaires asked about age, gender, occupation, work status, duration of current episode, chronicity, aggravating features and bothersomeness using Deyo's 'Core Set'. Psychological factors (fear-avoidance beliefs, inevitability, anxiety/distress and coping, and co-morbidity were also assessed at baseline. Satisfaction with care, number of attendances and pain impact were determined at 6 weeks. Predictors of poor outcome were sought by the calculation of relative risk ratios. RESULTS: Most patients presented within 4 weeks of onset. Of 158 eligible and willing patients, 130 completed both baseline and 6-week follow-up questionnaires. Greatest improvements at 6 weeks were in interference with normal work (ES 1.12) and LBP bothersomeness (ES 1.37). Although most patients began with moderate-high back pain bothersomeness scores, few had high psychometric ones. Co-morbidity was a risk for high-moderate interference with normal work at 6 weeks (RR 2.37; 95% C.I. 1.15–4.74). An episode duration of >4 weeks was associated with moderate to high bothersomeness at 6 weeks (RR 2.07; 95% C.I. 1.19 – 3.38) and negative outlook (inevitability) with moderate to high interference with normal work (RR 2.56; 95% C.I. 1.08 – 5.08). CONCLUSION: Patients attending a private UK chiropractic clinic for new episodes of non-specific LBP exhibited few psychosocial predictors of poor outcome, unlike other patient populations that have been studied. Despite considerable bothersomeness at baseline, scores were low at follow-up. In this independent health sector back pain population, general health and duration of episode before consulting appeared more important to outcome than psychosocial factors
Substantial and sustained reduction in under-5 mortality, diarrhea, and pneumonia in Oshikhandass, Pakistan: Evidence from two longitudinal cohort studies 15 years apart
Background: Oshikhandass is a rural village in northern Pakistan where a 1989-1991 verbal autopsy study showed that diarrhea and pneumonia were the top causes of under-5 mortality. Intensive surveillance, active community health education and child health interventions were delivered in 1989-1996; here we assess improvements in under-5 mortality, diarrhea, and pneumonia over this period and 15 years later.Methods: Two prospective open-cohort studies in Oshikhandass from 1989 to 1996 (Study 1) and 2011-2014 (Study 2) enrolled all children under age 60 months. Study staff trained using WHO guidelines, conducted weekly household surveillance and promoted knowledge on causes and management of diarrhea and pneumonia. Information about household characteristics and socioeconomic status was collected. Hurdle models were constructed to examine putative risk factors for diarrhea and pneumonia.Results: Against a backdrop of considerable change in the socioeconomic status of the community, under-5 mortality, which declined over the course of Study 1 (from 114.3 to 79.5 deaths/1000 live births (LB) between 1989 and 1996), exceeded Sustainable Development Goal 3 by Study 2 (19.8 deaths/ 1000 LB). Reductions in diarrhea prevalence (20.3 to 2.2 days/ Child Year [CY]), incidence (2.1 to 0.5 episodes/ CY), and number of bloody diarrhea episodes (18.6 to 5.2%) seen during Study 1, were sustained in Study 2. Pneumonia incidence was 0.5 episodes /CY in Study 1 and 0.2/CY in Study 2; only 5% of episodes were categorized as severe or very severe in both studies. While no individual factors predicted a statistically significant difference in diarrhea or pneumonia episodes, the combined effect of water, toilet and housing materials was associated with a significant decrease in diarrhea; higher household income was the most protective factor for pneumonia in Study 1.Conclusions: We report a 4-fold decrease in overall childhood mortality, and a 2-fold decrease in childhood morbidity from diarrhea and pneumonia in a remote rural village in Pakistan between 1989 and 2014. We conclude that significant, sustainable improvements in child health may be achieved through improved socioeconomic status and promoting interactions between locally engaged health workers and the community, but that continued efforts are needed to improve health worker training, supervision, and the rational use of medications
Arts practices in unreasonable doubt? Reflections on understandings of arts practices in healthcare contexts
This article suggests that the discourse on arts and health encompass contemporary arts practices as an active and engaged analytical activity. Distinctions between arts therapy and arts practice are made to suggest that clinical evidence-based evaluation, while appropriate for arts therapy, is not appropriate for arts practice and in effect cast them in unreasonable doubt. Themes in current discourse on “arts” and “health” are broadly sketched to provide a context for discussion of arts practices. Approaches to knowledge validation in relation to each domain are discussed. These discourses are applied to the Irish healthcare context, offering a reading of three different art projects; it suggests a multiplicity of analyses beyond causal positive health gains. It is suggested that the social turn in medicine and the social turn in arts practices share some similar pre-occupations that warrant further attention
