Diagnostics of pneumoconiosis at workers of thermal power stations by a method of a computer tomography

It is spent the analysis of results of a computer tomography of the high permission of lungs at workers of the thermal power stations which are exposed to influence of a coal dust and ashes, included in a suspicion group of pneumoconiosis. The method of a computer tomography has allowed to establish a presence of pathological changes in lungs, characteristic for pneumoconiosis, in more quantity of cases, than at the radiological research. Results of Kt-research have shown a prevalence of nodular formations over interstitial changes. At comparison of the risk group and patients with pneumoconiosis, the increase in the sizes of nodular formations is revealed. The present research has allowed to confirm the high diagnostic importance of a computer tomography of the high permission and to define features of pneumoconiosis at workers of the thermal power stations using coal in a technological process.Проведен анализа результатов компьютерной томографии высокого разрешения легких у работников тепловых электростанций, подвергающихся воздействию каменноугольной пыли и золы, включенных в группу подозрения на пневмокониоз. Метод компьютерной томографии позволил установить наличие патологических изменений в легких, характерных для пневмокониоза, в большем количестве случаев, чем при рентгенологическом исследовании. Результаты КТ-исследования показали преобладание узелковых образований над интерстициальными изменениями. При сравнении группы риска и больных пневмокониозом, выявлено увеличение размеров узелковых образований. Настоящее исследование позволило подтвердить высокую диагностическую значимость компьютерной томографии высокого разрешения и определить особенности пневмокониоза у работников тепловых электростанций, использующих в технологическом процессе каменный уголь

Significance of Social and Psychological Preconditions in Successful Therapy of Tuberculosis on the Background of HIV–Infection

The article considers the socio-psychological prerequisites for the successful therapy of tuberculosis in the context of HIV infection. The dependence of positive results is also considered — the increase in the number of CD4 cells from the conditions in which patients live, the conditions of the environment and awareness of these diseases, the conduct of a healthy lifestyle, adherence to therapy prescribed by a doctor. To achieve the effects of therapy, patients’ informed consent, absence of bad habits, simultaneous administration of antituberculous therapy and antiretroviral therapy, and the possibility of staying at a sanatorium-and-spa treatment are necessary. The research revealed unsatisfactory internal and external factors, as well as the prerequisites for the development of tuberculosis against HIV infection (adverse social and environmental conditions, alcohol, smoking, and drug abuse). During the analysis of the data, it was found that the presence of chronic diseases (such as diabetes, peptic ulcer or duodenal ulcer, pulmonary diseases) lead to a longer treatment of the disease and contribute to the chronic process.В статье рассмотрены социально-психологические предпосылки успешной терапии туберкулеза на фоне ВИЧ-инфекции, а также зависимость положительных результатов (увеличение числа CD4‑клеток) от условий, в которых проживают пациенты, внешней среды и информированности о данных заболеваниях, ведении здорового образа жизни, приверженности терапии, назначенной врачом. Для достижения эффектов от терапии необходимо информированное согласие пациентов, отсутствие вредных привычек, одновременное назначение противотуберкулезной терапии и антиретровирусной терапии, возможность пребывания на санаторно-курортном лечении. В ходе исследования выявлены неудовлетворительные внутренние и внешние факторы, а также предпосылки, способствующие развитию туберкулеза на фоне ВИЧ-инфекции (неблагоприятные социальные и экологические условия, злоупотребление алкоголем, курением, наркотиками). В процессе анализа данных было выявлено, что наличие хронических заболеваний (диабет, язвенная болезнь желудка или 12‑перстной кишки, заболевания легких) приводят к более длительной терапии заболевания и способствуют хронизации процесса

The article presents an analysis of satisfaction with the quality of medical services provided on an outpatient basis at the Megionskaya City Hospital N1. 120 respondents who underwent outpatient treatment in the department of general practice, therapeutic department of the polyclinic, medical advisory department, center of outpatient surgery and antenatal clinic took part in the survey.В статье представлен анализ удовлетворенности качеством предоставления медицинских услуг в амбулаторных условиях в БУ «Мегионская городская больница №1».
