The culture of stromal stem cells was incubated under constant and dynamic conditions. The incubation period was lasting 48 h. Intracellular enzyme activity of stromal stem cells were carried out with cytochemical methods. Alkaline phosphatase activity was increased under dynamic conditions, differentiation of stromal stem cells being accelerated comparatively steady-state conditions.Проведено культивирование стромальных клеток в динамических и стационарных условиях в течение 24-48 ч. Исследована их внутриклеточная ферментативная активность с помощью цитохимических методов - окраски на щелочную фосфатазу и неспецифическую эстеразу. В условиях динамического культивирования активность щелочной фосфатазы была выше по сравнению со стационарными условиями, что свидетельствует об ускорении дифференцировки и созревания стромальных клеток человека

The paper presents the results of the Osteoarthritis (OA) Expert Council held on September 8, 2019, which was attended by Russian and foreign specialists. The experts considered pharmacological treatment options for OA. The expert meeting resolution states that the treatment of patients with OA should be based on an individual assessment of the patient and on a modern evidence base of therapy efficacy.Treatment of patients with OA is based on the principles of evidence-based medicine that requires an integrated approach and the need of SYSADOAs prescription. Combined drugs with therapeutic dosages of chondroitin sulfate and glucosamine in the early stages of the disease are available as basic agents. The place of paracetamol in the anesthetic therapy algorithm in OA needs to be clarified. It is also noted that when choosing nonsteroidal anti-inflammatory drugs for OA treatment, it is important to take into account individual patient characteristics and the presence of comorbidities.Представлены результаты Экспертного совета по остеоартриту (ОА), проходившего 8 сентября 2019 г., в котором приняли участие российские и зарубежные специалисты. Рассматривались возможности фармакологического лечения ОА. В резолюции совещания указано, что лечение больных ОА должно быть основано на индивидуальной оценке состояния пациента и современных доказательствах эффективности терапии. Лечение больных ОА на основании принципов доказательной медицины предполагает комплексный подход и назначение SYSADOA. Комбинированные препараты с терапевтическими дозами хондроитина сульфата и глюкозамина уже на ранних стадиях заболевания рассматриваются в качестве базисных средств. Место парацетамола в алгоритме обезболивающей терапии при ОА требует уточнения. Отмечено также, что при выборе нестероидных противовоспалительных препаратов для лечения ОА важно учитывать индивидуальные особенности пациента и наличие коморбидных состояний

Cell technologies in the treatment of rheumatic diseases

Rheumatic diseases are caused by chronic autoimmune processes that require long-term therapy, which leads to the formation of resistance to the drugs used. The search for new approaches to their treatment in recent years has been enriched with the use of cell therapy methods. They are considered in this review of the literature and include the transplantation of hematopoietic stem cells, mesenchymal stem (stromal) cells, the induction or administration of T-regulatory cells, tolerogenic dendritic cells. The paper discusses methods of obtaining cell products, their safety and clinical use in patients with systemic lupus erythematosus, systemic sclerosis, rheumatoid arthritis, and other rheumatic diseases, and treatment complications. Based on the analysis, the authors give the comparative characteristics of various types of tolerogenic cell therapy for rheumatic diseases and point out their advantages, disadvantages, and the specific features of clinical application

Спектрометрический и морфометрический анализ кристаллов высушенных биологических жидкостей при термоингаляционной травме

Character of free and 0,9% sodium chloride solution initiated crystals yielding in saliva and blood serum of the 14 thermo-inhalation traumatized patients was studied. Spectrometric characteristics of the biofluids newgenied crystals at the 300-400 nm wave-lengths were estimated. We disposed the teziocrystalloscopic «pattern» of human biosubstratum for the studied pathological state .Произведен анализ активности и кинетических характеристик лактатдегидрогеназы печени крыс линии Вистар до и по окончании дозированной термомодификации (15 мин.; 60оС). Установлено, что выполненная термомодификация фермента приводит к активации прямой и ингибированию обратной лактатдегидрогеназной реакции, что подтверждается как по динамике уровня активности энзима, так и по кинетическим показателям

Results of clinic-morphological studies of sclera after surgical diathermy (experimental study)

The inflammatory-reparative reactions of sclera after different injury were studied. In animals of the 1st group carried out conjunctivotomy and surgical diathermy. In animals of the 2nd group carried out conjunctivotomy. Biomicroscopy, histologic and morphometrical studies were carried out. The results demonstrate surgical diathermy of sclera leads to intensification of local inflammatory reaction and is a risk factor of excessive regeneration in glaucoma filtration surgery

ASSESSMENT OF RISK FOR GASTROINTESTINAL AND CARDIOVASCULAR COMPLICATIONS ASSOCIATED WITH THE USE OF NONSTEROIDAL ANTI-INFLAMMATORY DRUGS IN THE CIS POPULATION: PRELIMINARY DATA OF THE CORONA-2 EPIDEMIOLOGICAL SURVEY

Nonsteroidal anti-inflammatory drugs (NSAIDs) are able to effectively control the major symptoms of rheumatic diseases and widely used in real clinical practice. However, they may cause serious gastrointestinal (GI) and cardiovascular (CV) events. The prevention of these events is based on the estimation of whether risk factors (RFs) are present.Objective: to estimate the presence of RFs in patients needing NSAIDs.Subjects and methods. A cross-sectional epidemiological survey was performed, during which 2021 physicians from 9 CIS countries questioned for 2 weeks at least 10 patients needing NSAIDs. The inclusion criterion was severe musculoskeletal pain (>40 mm on a 100-mm visual analogue scale (VAS)) or use of NSAIDs at the examination. Data were obtained on 21,185 patients (57.5% women and 42.5% men) (mean age 50.5±14.1 years) who had predominantly dorsalgia (56.6%) and osteoarthritis (23.5%). The mean pain value was 62.2±25.2 mm.Results. 1.7, 11.3, and 25.3% of patients had history of gastrointestinal bleeding, ulcer, or dyspepsia, respectively; people over 65 years of age constituted 16.8%; those who took low-dose aspirin (LDA) – 20.0%. The total number of patients at high risk for GI events was 29.0%. There were also common CV RFs: myocardial infarction or stroke (7.8%), coronary heart disease (17.8%), hypertension (37.7%), and diabetes mellitus (8.1%). The total number of patients at high risk for CV events (without SCOR assessment) was 23.0%. Many high-risk patients who has already used NSAIDs received no effective prevention. Thus, 62.2% of the patients at high GI risk took gastroprotective drugs; 53.2% of those at high CV risk used LDA.Conclusion. A large number of patients needing active analgesic therapy have a serious risk for drug-induced complications. This limits the possibility of using NSAIDs and determines the need for effective prevention or use of alternative methods for analgesia
