Randomized, open-label, phase 1/2a study to determine the maximum tolerated dose of intraventricular sustained release nimodipine for subarachnoid hemorrhage (NEWTON [Nimodipine Microparticles to Enhance Recovery While Reducing Toxicity After Subarachnoid Hemorrhage])
BACKGROUND AND PURPOSEâ: We conducted a randomized, open-label, phase 1/2a, dose-escalation study of intraventricular sustained-release nimodipine (EG-1962) to determine safety, tolerability, pharmacokinetics, and clinical effects in aneurysmal subarachnoid hemorrhage. METHODSâ: Subjects with aneurysmal subarachnoid hemorrhage repaired by clipping or coiling were randomized to EG-1962 or enteral nimodipine. Subjects were World Federation of Neurological Surgeons grade 2 to 4 and had an external ventricular drain. Cohorts of 12 subjects received 100 to 1200 mg EG-1962 (9 per cohort) or enteral nimodipine (3 per cohort). The primary objective was to determine the maximum tolerated dose. RESULTSâ: Fifty-four subjects in North America were randomized to EG-1962, and 18 subjects were randomized to enteral nimodipine. The maximum tolerated dose was 800 mg. One serious adverse event related to EG-1962 (400 mg) and 2 EG-1962 dose-limiting toxicities were without clinical sequelae. There was no EG-1962-related hypotension compared with 17% (3/18) with enteral nimodipine. Favorable outcome at 90 days on the extended Glasgow outcome scale occurred in 27/45 (60%, 95% confidence interval 46%â74%) EG-1962 subjects (5/9 with 100, 6/9 with 200, 7/9 with 400, 4/9 with 600, and 5/9 with 800 mg) and 5/18 (28%, 95% confidence interval 7%â48%, relative risk reduction of unfavorable outcome; 1.45, 95% confidence interval 1.04â2.03; P=0.027) enteral nimodipine subjects. EG-1962 reduced delayed cerebral ischemia (14/45 [31%] EG-1962 versus 11/18 [61%] enteral nimodipine) and rescue therapy (11/45 [24%] versus 10/18 [56%]). CONCLUSIONSâ: EG-1962 was safe and tolerable to 800 mg, and in this, aneurysmal subarachnoid hemorrhage population was associated with reduced delayed cerebral ischemia and rescue therapy. Fe XVII X-ray Line Ratios for Accurate Astrophysical Plasma Diagnostics
New laboratory measurements using an Electron Beam Ion Trap (EBIT) and an
x-ray microcalorimeter are presented for the n=3 to n=2 Fe XVII emission lines
in the 15 {\AA} to 17 {\AA} range, along with new theoretical predictions for a
variety of electron energy distributions. This work improves upon our earlier
work on these lines by providing measurements at more electron impact energies
(seven values from 846 to 1185 eV), performing an in situ determination of the
x-ray window transmission, taking steps to minimize the ion impurity
concentrations, correcting the electron energies for space charge shifts, and
estimating the residual electron energy uncertainties. The results for the
3C/3D and 3s/3C line ratios are generally in agreement with the closest theory
to within 10%, and in agreement with previous measurements from an independent
group to within 20%. Better consistency between the two experimental groups is
obtained at the lowest electron energies by using theory to interpolate, taking
into account the significantly different electron energy distributions.
Evidence for resonance collision effects in the spectra is discussed.
Renormalized values for the absolute cross sections of the 3C and 3D lines are
obtained by combining previously published results, and shown to be in
agreement with the predictions of converged R-matrix theory. This work
establishes consistency between results from independent laboratories and
improves the reliability of these lines for astrophysical diagnostics. Factors
that should be taken into account for accurate diagnostics are discussed,
including electron energy distribution, polarization, absorption/scattering,

We demonstrate, in an elementary manner, that given a partition of the single
particle Hilbert space into orthogonal subspaces, a Fermi sea may be factored
into pairs of entangled modes, similar to a BCS state. We derive expressions
for the entropy and for the particle number fluctuations of a subspace of a
fermi sea, at zero and finite temperatures, and relate these by a lower bound
on the entropy. As an application we investigate analytically and numerically
these quantities for electrons in the lowest Landau level of a quantum Hall

An Adaptive Virtual Training System Based on Universal Design
The increasing complexity of manufacturing environments requires effective training systems to prepare the operation personnel for their tasks. Several training systems have been proposed. A common approach is the application of virtual environments to train interactions with an industrial machine in a safe, attractive, and efficient way. However, these training systems cannot adapt to the requirements of an increasingly diversified workforce. This paper introduces an approach for the design of an adaptive virtual training system based on the idea of universal design. The system is based on a two-step approach that consists of an initial adaptation to the user capabilities and real-time adaptations during training based on measurements of the user. The adaptations concern the use of different representations of lessons with different complexity and interaction modalities. The proposed approach provides a flexible training system that can adapt to the needs of a broad group of users
Red blood cell transfusion in patients with subarachnoid hemorrhage: a multidisciplinary North American survey
Abstract
Introduction
Anemia is associated with poor outcomes in patients with aneurysmal subarachnoid hemorrhage (SAH). It remains unclear whether this association can be modified with more aggressive use of red blood cell (RBC) transfusions. The degree to which restrictive thresholds have been adopted in neurocritical care patients remains unknown.
Methods
We performed a survey of North American academic neurointensivists, vascular neurosurgeons and multidisciplinary intensivists who regularly care for patients with SAH to determine hemoglobin (Hb) concentrations which commonly trigger a decision to initiate transfusion. We also assessed minimum and maximum acceptable Hb goals in the context of a clinical trial and how decision-making is influenced by advanced neurological monitoring, clinician characteristics and patient-specific factors.
Results
The survey was sent to 531 clinicians, of whom 282 (53%) responded. In a hypothetical patient with high-grade SAH (WFNS 4), the mean Hb concentration at which clinicians administered RBCs was 8.19 g/dL (95% CI, 8.07 to 8.30 g/dL). Transfusion practices were comparatively more restrictive in patients with low-grade SAH (mean Hb 7.85 g/dL (95% CI, 7.73 to 7.97 g/dL)) (P < 0.0001) and more liberal in patients with delayed cerebral ischemia (DCI) (mean Hb 8.58 g/dL (95% CI, 8.45 to 8.72 g/dL)) (P < 0.0001). In each setting, there was a broad range of opinions. The majority of respondents expressed a willingness to study a restrictive threshold of â€8 g/dL (92%) and a liberal goal of â„10 g/dl (75%); in both cases, the preferred transfusion thresholds were significantly higher for patients with DCI (P < 0.0001). Neurosurgeons expressed higher minimum Hb goals than intensivists, especially for patients with high-grade SAH (ÎČ = 0.46, P = 0.003), and were more likely to administer two rather than one unit of RBCs (56% vs. 19%; P < 0.0001). Institutional use of transfusion protocols was associated with more restrictive practices. More senior clinicians preferred higher Hb goals in the context of a clinical trial. Respondents were more likely to transfuse patients with brain tissue oxygen tension values <15 mmHg and lactate-to-pyruvate ratios >40.
Conclusions
There is widespread variation in the use of RBC transfusions in SAH patients. Practices are heavily influenced by the specific dynamic clinical characteristics of patients and may be further modified by clinician specialty and seniority, the use of protocols and advanced neurological monitoring
Virtual discussions to support climate risk decision making on farms
Climate variability represents a significant risk to farming enterprises. Effective extension of climate information may improve climate risk decision making and adaptive management responses to climate variability on farms. This paper briefly reviews current agricultural extension approaches and reports stakeholder responses to new web-based virtual world âdiscussion-supportâ tools developed for the Australian sugar cane farming industry. These tools incorporate current climate science and sugar industry better management practices, while leveraging the social-learning aspects of farming, to provide a stimulus for discussion and climate risk decision making. Responses suggest that such virtual world tools may provide effective support for climate risk decision making on Australian sugar cane farms. Increasing capacity to deliver such tools online also suggests potential to engage large numbers of farmers globally
Reframing e-assessment: building professional nursing and academic attributes in a first year nursing course
This paper documents the relationships between pedagogy and e-assessment in two nursing courses offered at the University of Southern Queensland, Australia. The courses are designed to build the academic, numeracy and technological attributes student nurses need if they are to succeed at university and in the nursing profession. The paper first outlines the management systems supporting the two courses and how they intersect with the e-learning and e-assessment components of course design. These pedagogical choices are then reviewed. While there are lessons to be learnt and improvements to be made, preliminary results suggest students and staff are extremely supportive of the courses. The e-assessment is very positively received with students reporting increased confidence and competency in numeracy, as well as IT, academic, research and communication skills
