Pathophysiological mechanism of post-lobectomy air leaks
Background: Air leak post-lobectomy continues to remain a significant clinical problem, with upper lobectomy associated with higher air leak rates. This paper investigated the pathophysiological role of pleural stress in the development of post-lobectomy air leak. Methods: Preoperative characteristics and postoperative data from 367 consecutive video assisted thoracic surgery (VATS) lobectomy resections from one centre were collected prospectively between January 2014 and March 2017. Computer modelling of a lung model using finite element analysis (FEA) was used to calculate pleural stress in differing areas of the lung. Results: Air leak following upper lobectomy was significantly higher than after middle or lower lobectomy (6.3% versus 2.5%, P=0.044), resulting in a significant six-day increase in mean hospital stay, P=0.004. The computer simulation model of the lung showed that an apical bullet shape was subject to eightyfold higher stress than the base of the lung model. Conclusions: After upper lobectomy, the bullet shape of the apex of the exposed lower lobe was associated with high pleural stress, and a reduction in mechanical support by the chest wall to the visceral pleura due to initial post-op lack of chest wall confluence. It is suggested that such higher stress in the lower lobe apex explains the higher parenchymal air leak post-upper lobectomy. The pleural stress model also accounts for the higher incidence of right-sided prolonged air leak post-resection.peer-reviewe
Structure formation in the presence of dark energy perturbations
We study non-linear structure formation in the presence of dark energy. The
influence of dark energy on the growth of large-scale cosmological structures
is exerted both through its background effect on the expansion rate, and
through its perturbations as well. In order to compute the rate of formation of
massive objects we employ the Spherical Collapse formalism, which we generalize
to include fluids with pressure. We show that the resulting non-linear
evolution equations are identical to the ones obtained in the Pseudo-Newtonian
approach to cosmological perturbations, in the regime where an equation of
state serves to describe both the background pressure relative to density, and
the pressure perturbations relative to the density perturbations as well. We
then consider a wide range of constant and time-dependent equations of state
(including phantom models) parametrized in a standard way, and study their
impact on the non-linear growth of structure. The main effect is the formation
of dark energy structure associated with the dark matter halo: non-phantom
equations of state induce the formation of a dark energy halo, damping the
growth of structures; phantom models, on the other hand, generate dark energy
voids, enhancing structure growth. Finally, we employ the Press-Schechter
formalism to compute how dark energy affects the number of massive objects as a
function of redshift.Comment: 21 pages, 8 figures. Matches published version, with caption of Fig.

A novel method of personnel cooling in an operating theatre environment
Funding obtained from University of Malta. An optimized theatre environment, including personal temperature regulation, can help maintain concentration, extend work times and may improve surgical outcomes. However, devices, such as cooling vests, are bulky and may impair the surgeon's mobility. We describe the use of a low-cost, low-energy 'bladeless fan' as a personal cooling device. The safety profile of this device was investigated by testing air quality using 0.5- and 5-µm particle counts as well as airborne bacterial counts on an operating table simulating a wound in a thoracic operation in a busy theatre environment. Particle and bacterial counts were obtained with both an empty and full theatre, with and without the 'bladeless fan'. The use of the 'bladeless fan' within the operating theatre during the simulated operation led to a minor, not statistically significant, lowering of both the particle and bacterial counts. In conclusion, the 'bladeless fan' is a safe, effective, low-cost and low-energy consumption solution for personnel cooling in a theatre environment that maintains the clean room conditions of the operating theatre.peer-reviewe
Composition, Size, and Surface Functionalization dependent Optical Properties of Lead Bromide Perovskite Nanocrystals
The photoluminescence (PL), color purity, and stability of lead halide
perovskite nanocrystals depend critically on the surface passivation. We
present a study on the temperature dependent PL and PL decay dynamics of lead
bromide perovskite nanocrystals characterized by different types of A cations,
surface ligands, and nanocrystal sizes. Throughout, we observe a single
emission peak from cryogenic to ambient temperature. The PL decay dynamics are
dominated by the surface passivation, and a post-synthesis ligand exchange with
a quaternary ammonium bromide (QAB) results in a more stable passivation over a
larger temperature range. The PL intensity is highest from 50K-250K, which
indicates that the ligand binding competes with the thermal energy at ambient
temperature. Despite the favorable PL dynamics of nanocrystals passivated with
QAB ligands (monoexponential PL decay over a large temperature range, increased
PL intensity and stability), the surface passivation still needs improvement
toward increased emission intensity in nanocrystal films.Comment: 19 pages, 5 figure
Justification of the symmetric damping model of the dynamical Casimir effect in a cavity with a semiconductor mirror
A "microscopic" justification of the "symmetric damping" model of a quantum
oscillator with time-dependent frequency and time-dependent damping is given.
This model is used to predict results of experiments on simulating the
dynamical Casimir effect in a cavity with a photo-excited semiconductor mirror.
It is shown that the most general bilinear time-dependent coupling of a
selected oscillator (field mode) to a bath of harmonic oscillators results in
two equal friction coefficients for the both quadratures, provided all the
coupling coefficients are proportional to a single arbitrary function of time
whose duration is much shorter than the periods of all oscillators. The choice
of coupling in the rotating wave approximation form leads to the "mimimum
noise" model of the quantum damped oscillator, introduced earlier in a pure
phenomenological way.Comment: 9 pages, typos corrected, corresponds to the published version,

Trends and extremes of drought episodes in Vietnam sub-regions during 1980–2017 at different timescales
This study investigated the temporal occurrence of dry conditions in the seven climatic sub-regions of Vietnam during the 1980–2017 period. This assessment was performed using the Standardized Precipitation Evapotranspiration Index (SPEI) and the Standardized Precipitation Index (SPI) at 1 to 24 months timescales. Results show that the main periods of extreme drought occurred simultaneously throughout the country in 1992–1993 and 2003–2004, except for 2015–2016, when it was not identified in the southern region. In addition, a slight temporal lag was identified latitudinally (north–south) at the beginning of dry conditions, revealing the largest difference between the northern and southern regions. A positive trend in the time series of both indices (SPEI and SPI) prevailed in all sub-regions, with the SPEI minus SPI difference always being negative, suggesting the importance of temperature and evapotranspiration for this trend. Further detailed analyses were then performed using SPEI at 1-month and 12-months timescales for all climate sub-regions, as well as the main indicators to characterize duration and severity. Results show that the number of drought episodes did not vary much between regions, but they did vary in duration and severity at the annual scale. Moreover, changes in the soil root zone are largely associated with dry and wet conditions not only from season to season, but also in longer accumulation periods and more strongly in the northern regions of Vietnam. Indeed, a study of the most severe drought episodes also revealed the occurrence of negative anomalies of the root-soil moisture in the subsequent four or more months. Dynamic atmospheric conditions associated with the peak of most severe drought episodes show the crucial role of subsidence of dry air in the middle and high atmosphere, which prevents convection in the lower troposphere. Finally, the linkages between drought conditions in Vietnam and large-scale atmospheric and oceanic teleconnection patterns were revealed to be quite different among northern and southern sub-regions. During the positive phase of El Niño–Southern Oscillation (ENSO), drought episodes at different timescales were identified in the southern climate sub-regions, while the negative phase was associated with drought conditions in the northern regions.Xunta de Galicia | Ref.
