    Test of an interprofessional collaborative practice model to improve obesity-related health outcomes in Michigan

    The purpose of the study was to test the effectiveness of an interprofessional collaborative practice (IPCP) education program on clinicians\u27 and students\u27 knowledge and attitudes toward IPCP and to determine the effectiveness of an IPCP weight loss program in two nurse-managed centers. The study team used the Midwest Interprofessional Practice, Education, and Research Center (MIPERC) collaborative practice education program that consists of online learning modules followed by daily huddles and collaborative care planning. The obesity intervention program was implemented by faculty and staff practitioners and students in two clinics with very different patient populations (community residents and college students). Staff/faculty practitioners and students demonstrated statistically significant knowledge gains as a result of online learning modules (Introduction to IPE p \u3c .05; Motivational Interviewing p \u3c .001; Safety Behaviors p \u3c .001; Team Dynamics p \u3c .001). Small, but not statistically significant changes in attitudes toward IPCP were seen with both groups. At program completion, enrolled patients showed statistical significant (p \u3c .001) weight losses and decreases in body mass indices. Other health outcomes showed no significant changes (blood pressure, prevalence of smoking, exercise frequency or duration p \u3e .05). The study demonstrated the potential of an IPCP program to affect weight loss in two populations

    In order to evaluate the effectiveness of the Georgia College & State University (GC&SU) Nursing Program, a non-experimental, survey research study was conducted by senior nursing students in the Nursing Research class. The survey evaluated the teaching and learning strategies used in the program, the students\u27 perceptions of preparedness for nursing after graduation, and the students\u27 perceptions of the strengths and weaknesses of the GC&SU nursing curriculum. Each participant signed an informed consent form. Forty-five out of a possible 142 nursing students responded to the survey. A statistical analysis of the responses was conducted and comparisons of answers were analyzed across the different nursing cohorts. Implications and recommendations for changes for the GC&SU Nursing Program were stated. As demand for registered nurses continues to increase, nursing programs in the United States are rising to meet this need by educating clinically proficient nurses. GC&SU nursing program graduates approximately 80 nursing students per year. In May 2003, 94% of the nursing graduates passed the licensure exam, helping fill the need in healthcare settings. Nursing faculty realize that they must prepare nurses who are adept at performing essential nursing skills including assessment, safe medication administration, nursing care for ill clients and health teaching

    Clomiphene, Metformin, or Both for Infertility in the Polycystic Ovary Syndrome

    Background The polycystic ovary syndrome is a common cause of infertility. Clomiphene and insulin sensitizers are used alone and in combination to induce ovulation, but it is unknown whether one approach is superior. Methods We randomly assigned 626 infertile women with the polycystic ovary syndrome to receive clomiphene citrate plus placebo, extended-release metformin plus placebo, or a combination of metformin and clomiphene for up to 6 months. Medication was discontinued when pregnancy was confirmed, and subjects were followed until delivery. Results The live-birth rate was 22.5% (47 of 209 subjects) in the clomiphene group, 7.2% (15 of 208) in the metformin group, and 26.8% (56 of 209) in the combinationtherapy group (P\u3c0.001 for metformin vs. both clomiphene and combination therapy; P=0.31 for clomiphene vs. combination therapy). Among pregnancies, the rate of multiple pregnancy was 6.0% in the clomiphene group, 0% in the metformin group, and 3.1% in the combination-therapy group. The rates of first-trimester pregnancy loss did not differ significantly among the groups. However, the conception rate among subjects who ovulated was significantly lower in the metformin group (21.7%) than in either the clomiphene group (39.5%, P=0.002) or the combinationtherapy group (46.0%, P\u3c0.001). With the exception of pregnancy complications, adverse-event rates were similar in all groups, though gastrointestinal side effects were more frequent, and vasomotor and ovulatory symptoms less frequent, in the metformin group than in the clomiphene group. Conclusions Clomiphene is superior to metformin in achieving live birth in infertile women with the polycystic ovary syndrome, although multiple birth is a complication.     THE FORMATION OF ORGANIZATIONS: A CASE STUDY OF THE NORTH AMERICAN SOCCER LEAGUE

    The formation of organizations is an important process in our society. Yet, little research exists on the factors that influence this process. Thus, this study was devised to contribute to the literature on organizational formation and to formulate a framework for the study of organizational formation. A case study was done of the formation of the North American Soccer League. Historical data was collected from various secondary sources. Influential factors were found to exist at the environmental, organizational, and individual levels. At the environmental level, the cultural environment was an influential factor in organizational formation: the NASL was formed when the cultural environment was guided by a leisure ethic. The legal environment was also influential in organizational formation: the legal system provided incentives for the formation of NASL. In addition, the demographic environment was influential in organizational formation: a high degree of urbanization helped to promote the formation of the NASL. Within the organizational level, it was found that the NASL was formed when the sports environment was non-competitive. An adequate supply of players and coaches was also influential in the NASL\u27s formation. At the individual level, those individuals who were influential in the formation of the NASL were wealthy and had a love of sports. Additional factors were somewhat influential in organizational formation, but these factors were not uniformly supported. At the environmental level, the economic environment was somewhat influential in organizational formation: high Gross National Products and stock market levels positively influenced the NASL\u27s formation. At the organizational level, the availability of facilities was somewhat influential in the NASL\u27s formation, as was the existence of subsidization. In addition, a positive attitude toward soccer was somewhat influential in the formation of the NASL. These findings have important implications for the organizational literature and for the formers of organizations
