    Recent Advances in the Management Strategies for Buruli Ulcers

    Buruli ulcer (BU) is a bacterial skin infection that is caused by Mycobacterium ulcerans and mainly affects people who reside in the rural areas of Africa and in suburban and beach resort communities in Australia. The infection typically begins as a painless papule or nodule that gradually develops into a large ulcer that can cause substantial impairment, damaging soft tissues and even bones. Early detection and immediate treatment are crucial to preventing further tissue damage and any potential complications, although it is worth noting that access to proper therapeutic resources can be limited in certain areas. The most commonly used antibiotics for treating BU are rifampicin, streptomycin, and clarithromycin; efforts have recently been made to introduce new treatments that increase the effectiveness and adherence to therapy. This article presents the latest research and management strategies regarding BU, providing an updated and intriguing perspective on this topic

    The Expression of Forkhead Box P3 T Regulatory Lymphocytes as a Prognostic Factor in Malignant Melanomas

    Since transcription factor Forkhead Box P3 (FoxP3) was identified as a specific regulatory T cell (Treg) marker, researchers have scrutinized its value as a potential novel therapeutic target or a prognostic factor in various types of cancer with inconsistent results. The present analysis was performed to assess the influence of Treg FoxP3 expression on the prognosis of primary melanoma and to evaluate the correlations with various clinicopathological prognostic factors. We analyzed all eligible patients with stage pT3 primary malignant melanomas treated in a tertiary cancer center. Immunohistochemical staining for Treg FoxP3 expression was performed on retrospectively identified paraffin blocks and subsequently correlated with the outcomes of the patients. A total of 81% of the patients presented a positive Treg FoxP3 expression, being correlated with a higher risk of lymph node metastasis, tumor relapse, and death. Moreover, positive expression was statistically associated with a shorter OS. The tumor relapse rate was estimated at 36.7%. A positive expression of Treg FoxP3 and lymph node metastasis were associated with a higher risk of death based on multivariate analysis. Treg FoxP3 expression may be used as an independent prognostic factor in patients with malignant melanoma to evaluate tumor progression and survival

    Sustainable Development of Human Society in Terms of Natural Depleting Resources Preservation Using Natural Renewable Raw Materials in a Novel Ecological Material Production

    In the last few years, the building industry experienced a significant development as a response to the demographic growth of human society and to the increasing demand for housing. Their construction involves the traditional use of concrete as a material that provides added strength to the finished building. This is manufactured respecting standard recipes depending on the way of its use. Anyway, all concrete recipes involve the use of mineral aggregates extracted from the riverbed, as is happening in Romania, or rock blocks crushing, as reported in other countries. Under these conditions, the rationalization of the use of natural mineral resources and the identification of new possibilities to reduce their consumption through their replacement with vegetal waste has become an important research issue. In this study, two types of vegetal waste—namely, shredded corn cobs and sunflower stalks—were used to manufacture novel ecological concretes. The vegetal wastes, both in untreated and treated forms (with 20% and 40% of sodium silicate solution), were used to replace 50% of the river (mineral) aggregate volume. The obtained concretes were tested, and the values of some important parameters in the concrete characterization (such as bulk density, water adsorption capacity, compressive strength and splitting tensile strength) were compared with the concrete contains cement CEM II/A-LL 42.5R. The obtained results show that these vegetal wastes have the potential to be used in the manufacturing of new ecological concrete. In addition, this alternative material meets the requirements for the sustainable and healthy development of the environment, offering low-polluting solutions in the context of an increasing demand for constructions

    The Importance of Carbapenemase-Producing Enterobacterales in African Countries: Evolution and Current Burden

    Antimicrobial resistance (AMR) is a worldwide healthcare problem. Multidrug-resistant organisms (MDROs) can spread quickly owing to their resistance mechanisms. Although colonized individuals are crucial for MDRO dissemination, colonizing microbes can lead to symptomatic infections in carriers. Carbapenemase-producing Enterobacterales (CPE) are among the most important MDROs involved in colonizations and infections with severe outcomes. This review aimed to track down the first reports of CPE in Africa, describe their dissemination throughout African countries and summarize the current status of CRE and CPE data, highlighting current knowledge and limitations of reported data. Two database queries were undertaken using Medical Subject Headings (MeSH), employing relevant keywords to identify articles that had as their topics beta-lactamases, carbapenemases and carbapenem resistance pertaining to Africa or African regions and countries. The first information on CPE could be traced back to the mid-2000s, but data for many African countries were established after 2015–2018. Information is presented chronologically for each country. Although no clear conclusions could be drawn for some countries, it was observed that CPE infections and colonizations are present in most African countries and that carbapenem-resistance levels are rising. The most common CPE involved are Klebsiella pneumoniae and Escherichia coli, and the most prevalent carbapenemases are NDM-type and OXA-48-type enzymes. Prophylactic measures, such as screening, are required to combat this phenomenon

    Cystic Echinococcosis in the Early 2020s: A Review

    Cystic echinococcosis (CE) is a zoonosis caused by metacestodes, the larval stage of Echinococcus granulosus. Although the World Health Organization (WHO) has defined CE as a neglected disease, it is the second most important foodborne parasitic disease, and it remains an important public health issue, considering its zonal endemicity and potential morbidity. The control and prevention of CE is a relevant WHO target, especially from a One Health perspective, as the disease affects not only animals and humans but also the food chain. Since not all countries have a CE surveillance strategy or reporting system and specific management guidelines, recent epidemiological data are relatively scarce, and research concerning the specific geographical distribution of the disease is ongoing. To add new information to the subject, we have analyzed and collected data from national guidelines and several medical databases. Out of the 751 research articles that were originally identified, only 52 were included in the investigation after applying specific inclusion and exclusion criteria. Notable international projects that have provided significant contributions and had a positive impact are presented. The available data were correlated with WHO recommendations on the subject, thus showcasing the measures taken and those that are still needed to properly control the disease’s spread

    Biological and Chemical Insights of Beech (Fagus sylvatica L.) Bark: A Source of Bioactive Compounds with Functional Properties

    The present study aimed, on the one hand, to improve the yield of microwave assisted extraction (MAE) of polyphenols from beech bark by using a design of experiments (DoE) approach. On the other hand, beech bark extracts (BBE) were characterized in terms of their phytochemical profile and evaluated for biological potential (antioxidant, antibacterial, antifungal, antimutagen, anti-α-glucosidase, and anti-tyrosinase). The extraction time varies with the amount of extracted total phenolic content (TPC). The microwave power favors TPC extraction but in different proportions. The optimum conditions which gave the highest TPC (76.57 mg GAE/g dry plant material) were reached when the microwave power was 300 W, extraction time was 4 min, and the solvent was an ethanol–water (50:50) mixture. The practical value of TPC after a controlled experiment was 76.49 mg GAE/g plant material. The identified compounds were vanillic acid, gallic acid, epicatechin, catechin, protocatechuic acid, chlorogenic acid, ferulic acid, and isoquercitrin. The antioxidant potential of BBEs was demonstrated by in vitro experiments. The BBEs were active against Staphylococcus aureus, Pseudomonas aeruginosa, Salmonella typhimurium, Escherichia coli, and Candida species. All extracts were antimutagenic and expressed an inhibition on α-glucosidase and tyrosinase activity. Regarding antimutagen activity, the assayed extracts may be considered to have low or no antimutagen effects

    Aim of the study The main objective of this paper is to perform an updated literature review of future endodontics trends based on the available up-to-date scientific literature to identify and describe the new techniques, new materials and their benefits or their limitations. Material and methods Electronic databases (PubMed, Wiley Online Library, Google Scholar and Web of Science) were used to perform a literature search from 2018 to 2023 using MESH terms: Endodontics, Artificial Intelligence, Lasers. After discarding duplicates, out of 1283 results, a total of 18 articles were eligible and point out how endodontic protocol may change with the onset of new technologies and procedures. Results Nanoparticle, laser, regenerative treatment, guided endodontics and artificial intelligence are the main areas that shown promising results in endodontic treatment. New materials with biological capacities will improve the long-term results. Overall, endodontic treatment tends to became more precise and the complications can be reduced. Conclusions Endodontics will see many exciting developments over the coming years, some incremental, others paradigm shifting

    Optimization of the rapid carbapenem inactivation method for use with AmpC hyperproducers

    International audienceOBJECTIVES: Detection of carbapenemase-producing Enterobacterales (CPEs) is sometimes difficult with AmpC-hyperproducing Enterobacterales (AHEs), as they may falsely be classified as CPEs. Here, we present a rapid Carbapenem Inactivation Method (rCIM) optimized for AmpC producers (rCIM-A) that allows rapid and easy discrimination between AHEs and CPEs.METHODS: Enterobacterales (n = 249), including natural AmpC producers, AHEs, CPEs and non-carbapenemase-producing carbapenem-resistant control strains were evaluated, using Carba NP, rCIM and rCIM-A. The rCIM-A differs from the rCIM by the addition of cloxacillin (400 Όg/mL) to the initial antibiotic incubation step.RESULTS: The rCIM-A yielded a sensitivity and specificity of 84.26% (95% CI: 76.00%-90.55%) and 99.29% (95% CI: 96.11%-99.98%), respectively, while those of the rCIM were 86.11% (95% CI: 78.13%-92.01%) and 80.85% (95% CI: 73.38%-86.99%), respectively; those of Carba NP were lower at 84.04% (95% CI: 75.05%-90.78%) and 91.37% (95% CI: 85.41%-95.46%), respectively, due to indeterminate results. The rCIM-A was capable of discriminating between AHEs and true CPEs, but still failed to identify OXA-23-producing Proteus mirabilis isolates and remained only partially reliable for identifying IMI-like producers and a few MBL (2 NDM-1, 1 LMB-1, 1 TMB-1 and 1 IMP-13) producers. One chromosomally encoded AmpC variant, MIR-10, gave repeatedly positive results using all three tests and was thus considered a false positive.CONCLUSIONS: Specificity for AHEs greatly improved with the rCIM-A without altering the test performance for the other resistance mechanisms. It may replace the rCIM as a cheap, easy, rapid and accurate CPE detection test

    IN VIVO ASSESSMENT OF CHARCOAL TOOTHPASTE EFFECT ON ENAMEL APPEARANCE – A NATURAL ALTERNATIVE FOR DENTAL AESTHETICS

    Teeth colour and dental aesthetic are sensitive subjects in the contemporary society with impact in everyday life. This clinical study aims to evaluate the effect of activated charcoal-based toothpaste on tooth enamel appearance, after use for tooth brushing, 2 times a day, for 4 consecutive weeks. Material and methods. Patient assessment (n=81) was carried out in the morning session at the same clinic, using good lighting conditions by 2 calibrated operators. Colour evaluation was realised using the VITA Classical Shade Guide for selected teeth in the upper and lower arch (central incisors, canines, 1st premolars), considering the teeth mainly visible by patients in smile and speech. Patients were asked to answer a short questionnaire about preferences regarding toothpaste selection, information’s about whitening toothpaste and tooth brushing technique. The results reveal statistically significant differences in shade changes for maxillary teeth (centrals incisors; p=0.021, canines; p<0.001, 1st premolars; p=0.008). In the mandible, the differences were statistically significant for canines (p<0.001) and 1st premolars (p=0.001) and non-significant for central incisors (p=0.126). According to patients’ responses, 18.5% of this used whitening toothpaste daily. Only 11.1% of patients asked for dentist advice regarding whitening toothpaste. Conclusions. Toothpaste with activated charcoal produce significant changes in enamel appearance, for evaluated teeth, except central mandibular incisors, element that allow us to consider this type of toothpaste as an alternative for preserving and improving dental enamel appearance.
