    Traditional and complementary medicine perspectives of family physicians in Istanbul

    Introduction: The World Health Organization, within the scope of the Traditional Medicine Strategy for 2014-2023, recommends the development of integrated health policies, particularly in the area of primary healthcare. However, family physicians often report feeling insufficiently trained to effectively integrate these practices into patient care. This study aims to assess the views, understanding, and experiences of family physicians regarding the utilization of Traditional amp; Complementary Medicine (T amp;CM).Methods: The family physicians employed by the Istanbul Health Directorate Public Health Services were solicited to participate in an 18-question online survey to assess their level of education in T amp;CM methods, their views on integration of T amp;CM into family medicine, and the impact of these practices on patient care.Results: A total of 324 family physicians took part in the survey; 12.0% were specialists, 64.5% had worked in primary care for over 10 years, and 59.6% were in the age range of 31-50 years. Among the physicians, the majority (70.1%) reported a preference for integrating T amp;CM practices with conventional medicine (ConvM). The majority of the participants (71.6%) had no formal T amp;CM training, and 66.4% reported inquiring about T amp;CM in their patient consultations. Over half of the physicians (56.79%) believed that T amp;CM methods should be used in preventive medicine. The most widely recognized T amp;CM technique was cupping therapy (18.5%), which was also the most frequently recommended method by physicians.Conclusion: Most of the family physicians who participated in the study expressed a desire for integrative medicine, despite acknowledging limited formal education in this field. Offering physicians training in evidence-based complementary medicine therapies could provide them with additional non-invasive treatment options. However, patients' diffidence to inform their family physicians about T amp;CM practices they undergo may pose significant risks. As a result, incorporating education on integrative medicine into formal medical education and residency training may be necessary.Keywords: Family physician, complementary medicine, integrative medicine, traditional medicin

    Smoking status of family physicians and their attitude on smoking cessation

    Objectives: Quitting smoking plays a vital role in preventing chronic diseases. Family physicians have an important role in smoking cessation processes. This study aimed to investigate the smoking status of family physicians who were working in the Family Health Centers in Istanbul and their knowledge and attitudes toward smoking cessation. Methods: In this study, an online survey was used to collect data from family physicians in Istanbul in 2019. The survey questions were sent to the family physicians using an e-mail and the physicians were asked to voluntarily answer the questions in the survey. Results: Total 237 physicians filled out the questionnaires and 133 (56.1%) of participants was male. The number of participants with response to smoking status as current smokers, former smokers, and never smokers were 46 (21.1%), 45 (20.5%), and 128 (58.4%), respectively. Among the participants, 37 (15.6%) were family physicians who were specialists, 43 (18.2) were aged between 41 and 45 years, and 122 (51.5%) were practicing family medicine for 7 to 9 years. The median Fagerström score of smokers was 3.0 [5.0] points. Nicotine replacement therapy was the most prominent application in the smoking cessation process according to 195 (84.3%) participants. Conclusion: Family physicians need to be encouraged to quit smoking and provide smoking cessation counseling

    Pediatrik Adenotonsillektomi Operasyonlarında Desfluran ile Sevofluran Etkilerinin Karşılaştırılması

    Amaç: Adenoidektomi ve adenotonsillektomi operasyonları çocukluk çağında sıklıkla yapılan günübirlik cerrahi işlemlerdendir. Kan,doku ve yağda düşük çözünürlüğe sahip inhalasyon anestezikleri, hızlı indüksiyon ve derlenme sağlaması ile günübirlik cerrahi için iyi bir seçenektir.Bu çalışmamızda desfluran ve sevofluran inhalasyon anestezik ilaçlarının çocuk olgularda hemodinamik etkileri ve derlenme kalitelerinin karşılaştırılması amaçlanmıştır. Hastalar ve Yöntemler: Adenoidektomi ve adenotonsillektomi operasyonu uygulanacak ASA I grubuna dahil, 3-10 yaşlarında, 52 olgu çalışmaya alındı. Tüm olgular rastgele iki gruba ayrıldı: Grup D ve Grup S. Standard anestezi indüksiyonu uygulandıktan sonra anestezi idamesi için %40 Oksijen (O2) - %60 Azot protoksit (N2O) ile birlikte Grup D’de %4 desfluran uygulanırken Grup S’de %2 sevofluran uygulandı. İndüksiyon zamanı, entübasyon zamanı, operasyonun 1., 5., 15. ve 30. dakikalarında, cerrahi bitiş ve ekstübasyon zamanında kalp atım hızı (KAH), ortalma arteryel basınç (OAB) ve periferik oksijen satürasyon (SpO2) değerleri kaydedildi. Olguların cerrahi, anestezi ve ekstübasyon süreleri kaydedildi. Olgular Derlenme Ünitesinde 20 dakika gözlendi. Modifiye Aldrete skalası ve Ağrı-konfor skalası ile anesteziden derlenmenin kalitesi değerlendirildi. Bulgular: Her iki grupta demografik özellikler ve hemodinamik etkiler benzer bulundu. Ekstübasyon zamanı Grup D’de daha kısa bulundu (p<0,05). Ayılma ünitesinde Modifiye Aldrete skala değerleri Grup D’de daha yüksek (p<0,05), Grup S’de ajitasyon insidansı daha yüksek olduğu tespit edildi (p<0,05). Sonuç: Pediatrik günübirlik olgularda, desfluran daha hızlı derlenme süresi ve daha az ajitasyon insidansı sağlanması nedeniyle, sevoflurana göre daha iyi bir seçenek olabilir

    Seven-Year Single-Center Experience of the Efficacy and Safety of Ferric Carboxymaltose in Cancer Patients with Iron-Deficiency Anemia

    Anemia remains an essential concern affecting the quality of life and the survival of cancer patients. Although there are different approaches to treating anemia in cancer patients, the number of studies reporting the efficacy of iron replacement in cancer patients is limited. In this study, the efficacy and safety of iron carboxymaltose, a parenteral iron treatment option, in the treatment of anemia, were examined retrospectively. A total of 1102 adult patients who received IV ferric carboxymaltose treatment at Hacettepe Oncology Hospital between 2014 and 2020 were included. The mean hemoglobin change observed at the end of the 12th week was 1.8 g/dL, and the rate of patients with an increase in hemoglobin of 1 g/dL or more was 72.1%. It was observed that the treatment demonstrated effectiveness in patients receiving active cancer treatment in all tumor types. The treatment was generally safe, and no grade 3–5 side effects were observed in the patients included in the study. According to one of the most extensive series published in the literature, iron carboxymaltose is an efficient and safe alternative for cancer patients with iron-deficiency anemia
