RECOVERY AFTER LONGTERM PULSATILE MECHANICAL CIRCULATORY SUPPORT IN PATIENT WITH CHRONIC HEART FAILURE: A CASE REPORT
Mehanička potpora cirkulaciji danas je dio standardne terapije u liječenju završne faze zatajivanja sistoličke funkcije srca. U radu opisujemo karakteristike uređaja Thoratec pVAD za mehaničku potporu cirkulaciji, tehniku ugradnje, kao i najvažnije prednosti i komplikacije primjene uređaja. Prikazujemo slučaj 41-godišnjeg bolesnika s dilatacijskom kardiomiopatijom, kojemu je prvi put u Republici Hrvatskoj zbog zatajivanja srca ugrađen uređaj za parakorporalnu mehaničku potporu lijevom srcu (LVAD), koji je nakon 130 dana potpore odstranjen zbog poboljšanja srčane funkcije.Use of mechanical circulatory support (MCS) is a part of today’s standard therapy in the treatment of end-stage heart failure. In this paper we describe characteristics of Thoratec pVAD device for MCS, implantation techniques, as well as the most important advantages and complications of application of the device. We present a 41-year-old patient with dilatated cardiomyopathy, who was the first recipient of paracorporeal left ventricular assist device (LVAD) in the Republic of Croatia due to end-stage heart failure. Kopački Rit Traditional Fishery: Its Anthroposocial and Ethnolinguistic Characteristics
Kopačevo je selo u močvarnom području Hrvatskog Podunavlja, u kojem se razvio jedinstven način ribarstva. Zbog nepotrebne zabrane ribolova i povezanih tradicijskih aktivnosti krajem 20. stoljeća, danas nestaje zadnja generacija ribara, nositelja tih umijeća. Istovremeno to stvara ekološku prijetnju, jer bez održavanja meandri nestaju zbog biljne sedimentacije. Društvo za tradicijski ribolov nastoji zaštititi tradicijska znanja kao kulturno dobro, turističku atrakciju i mjeru za nadzor i zaštitu okoliša. Cilj istraživanja bio je procjena ekološke, kulturne i ekonomske održivosti moguće revitalizacije tradicijskog ribolova u Kopačkom ritu.Kopačevo is a Croatian Danube Region wetland village where a unique type of fishery was developed. Tradicijski ribolov u Kopačkom ritu kao kulturno turistički potencijal i mjera zaštite okoliša (Sažetak)
Kopačevo is a Croatian Danube Region wetland village where a unique type of fishery was developed. Due to the late 20th century ban on fishing and related traditional activities, today the last generation of fishermen, who have these skills, has all but disappeared. At the same time, this represents a threat to the ecosystem, because without maintenance the meanders are disappearing due to the plant deposits. Povod istraživanja je dvojba o tome, sugeriraju li „novi“ turistički slogan Hrvatske turističke zajednice, Croatia Full of Life, objavljen u veljači 2015. i pripadajući turističko promotivni TV spotovi, kulturni, odgovorni i održivi turizam? Jer brendiranje Hrvatske isključivo kao prostora netaknute prirode, Mediterana kakav je nekada bio, punog života, vraćanje je u prošlost, a u pogledu istočne Hrvatske, koja je u fokusu rada, na polaznu točku, terra nove s konca 17. st., kao postkolonijalnog sindroma. Koincidencija objave predmetnog slogana i 200-te godišnjice rođenja J.J. Strossmayera, ponudila je prikladan kulturno povijesni kontekst, bez kojeg nije moguće razumjeti nalaze analize turističke promidžbe i drugih razvojnih aktivnosti, s obzirom na to da upravo J.J. Strossmayer ima presudan utjecaj na promjenu rečene kolonijalne paradigme. Osvrt na njegovo ulaganje u kulturu otkriva danas vrijedan turistički potencijal i u jasnom je kontrastu s razvojnim i promotivnim aktivnostima hrvatskih institucija u domeni turizma. Shodno, analizirana je promidžba hrvatskog turizma u strategiji i kampanji Hrvatske turističke zajednice, na temelju spotova pod sloganom Croatia Full of Life, u odnosu na usporediva odredišta. Komparativna analiza više turističko razvojnih projekata, odgovara na pitanje postoji li zadovoljavajuća suradnja na planu održivog razvoja turizma između državnih tijela i drugih dionika u turizmu. U radu se ispituju i stavovi javnosti o ulozi baštine u turizmu, te postojeća regulativa i strategija turizma u odnosu na mogući održivi smjer razvitka, što također potvrđuje da ne postoje društveni, organizacijski i institucionalni uvjeti za razvitak kulturnog i odgovornog turizma, kao održivog i prihvatljivog modela.The Research starts from the doubt, whether the "new" tourist slogan of the Croatian Tourist Board, Croatia Full of Life, published on February 2015 and the associated tourism promotional TV spots, are suggesting cultural, responsible and sustainable tourism? And in the case of eastern Croatia, which is the focus of the research, at the start point of the terra nova from the end of the 17th century, as a postcolonial syndrome. The coincidence of the publication of the slogan in question and the 200th birthday of J.J. Strossmayer, offered a suitable cultural and historical context, without which it is impossible to understand the findings of the analysis of tourism promotion and other developmental activities, given that J.J. Strossmayer has a decisive influence on changing the said colonial paradigm. The review of his investment in culture reveals valuable tourism potential today and is in clear contrast to the development and promotion activities of Croatian institutions in the field of tourism. Accordingly, the promotion of Croatian tourism in the strategy and the campaign of the Croatian National Tourist Board has been analyzed, based on the videos under the slogan Croatia Full of Life, in relation to comparable destinations. Comparative analysis of several tourism development projects, answers the question, whether there is satisfactory cooperation in the field of sustainable tourism development between state bodies and other stakeholders in tourism. The paper also examines public attitudes about the role of heritage in tourism, and existing tourism regulations and strategies concerning a possible sustainable development direction, which also confirms that there are no social, organizational and institutional conditions for the development of cultural and responsible tourism, as a sustainable and acceptable model
