    Breast MRI: State of the Art

    Contains fulltext : 208973.pdf (publisher's version ) (Open Access)MRI of the breast has the highest sensitivity for breast cancer detection among current clinical imaging modalities and is indispensable for breast imaging practice. While the basis of breast MRI consists of T1-weighted contrast-enhanced imaging, T2-weighted, ultrafast, and diffusion-weighted imaging may be used to improve lesion characterization. Such multiparametric assessment of breast lesions allows for excellent discrimination between benign and malignant breast lesions. Indications for breast MRI are expanding. In preoperative staging, multiple studies confirm the superiority of MRI to other imaging modalities for tumor size estimation and detection of additional tumor foci in the ipsilateral and contralateral breast. Ongoing studies show that in experienced hands this can be used to improve breast cancer surgery, although there is no evidence of improved long-term outcomes. Screening indications are likewise growing as evidence is accumulating that OncologicRI depicts cancers at an earlier stage than mammography in all women. To manage the associated costs for screening, the use of abbreviated protocols may be beneficial. In patients treated with neoadjuvant chemotherapy, MRI is used to document response. It is essential to realize that oncologic and surgical response are different, and evaluation should be adapted to the underlying question

    We present a fully quantum mechanical treatment of a single-mode atomic cavity with a pumping mechanism and an output coupling to a continuum of external modes. This system is a schematic description of an atom laser. In the dilute limit where atom-atom interactions are negligible, we have been able to solve this model without making the Born and Markov approximations. When coupling into free space, it is shown that for reasonable parameters there is a bound state which does not disperse, which means that there is no steady state. This bound state does not exist when gravity is included, and in that case the system reaches a steady state. We develop equations of motion for the two-time correlation in the presence of pumping and gravity in the output modes. We then calculate the steady-state output energy flux from the laser.Comment: 14 pages (twocloumn), 6 figure

    Interventions for improving clinical outcomes and health-related quality-of-life for people living with skeletal dysplasias: an evidence gap map

    Purpose: Skeletal dysplasias are rare genetic disorders that are characterized by abnormal development of bone and cartilage. There are multiple medical and non-medical treatments for specific symptoms of skeletal dysplasias e.g. pain, as well as corrective surgical procedures to improve physical functioning. The aim of this paper was to develop an evidence-gap map of treatment options for skeletal dysplasias, and their impact on patient outcomes. Methods: We conducted an evidence-gap map to identify the available evidence on the impact of treatment options on people with skeletal dysplasias on clinical outcomes (such as increase in height), and dimensions of health-related quality of life. A structured search strategy was applied to five databases. Two reviewers independently assessed articles for inclusion in two stages: titles and abstracts (stage 1), and full text of studies retained at stage 2. Results: 58 studies fulfilled our inclusion criteria. The included studies covered 12 types of skeletal dysplasia that are non-lethal with severe limb deformities that could result in significant pain and numerous orthopaedic interventions. Most studies reported on the effect of surgical interventions (n = 40, 69%), followed by the effect of treatments on dimensions of health quality-of-life (n = 4, 6.8%) and psychosocial functioning (n = 8, 13.8%). Conclusion: Most studies reported on clinical outcomes from surgery for people living with Achondroplasia. Consequently, there are gaps in the literature on the full range of treatment options (including no active treatment), outcomes and the lived experience of people living with other skeletal dysplasias. More research is warranted to examine the impact of treatments on health-related quality-of-life of people living with skeletal dysplasias, including their relatives to enable them to make preference- and valued based decisions about treatment

    Effects of interatomic collisions on atom laser outcoupling

    We present a computational approach to the outcoupling in a simple one-dimensional atom laser model, the objective being to circumvent mathematical difficulties arising from the breakdown of the Born and Markov approximations. The approach relies on the discretization of the continuum representing the reservoir of output modes, which allows the treatment of arbitrary forms of outcoupling as well as the incorporation of non-linear terms in the Hamiltonian, associated with interatomic collisions. By considering a single-mode trapped condensate, we study the influence of elastic collisions between trapped and free atoms on the quasi steady-state population of the trap, as well as the energy distribution and the coherence of the outcoupled atoms.Comment: 25 pages, 11 figures, to appear in J.     Exact quantum jump approach to open systems in Bosonic and spin baths

    A general method is developed which enables the exact treatment of the non-Markovian quantum dynamics of open systems through a Monte Carlo simulation technique. The method is based on a stochastic formulation of the von Neumann equation of the composite system and employs a pair of product states following a Markovian random jump process. The performance of the method is illustrated by means of stochastic simulations of the dynamics of open systems interacting with a Bosonic reservoir at zero temperature and with a spin bath in the strong coupling regime.Comment: 4 pages, 2 figure

    An Atom Laser Based on Raman Transitions

    In this paper we present an atom laser scheme using a Raman transition for the output coupling of atoms. A beam of thermal atoms (bosons) in a metastable atomic state 1>|1 > are pumped into a multimode atomic cavity. This cavity is coupled through spontaneous emission to a single mode of another cavity for the ground atomic state, 2>|2 >. Above a certain threshold pumping rate a large number of atoms, N2N_2, builds up in this single quantum state and transitions to the ground state of the cavity become enhanced by a factor (N2+1)(N_2 + 1). Atoms in this state are then coupled to the outside of the cavity with a Raman transition.     Kystovervåkingsprogrammet skal gi en oversikt over miljøtilstanden m.h.t. næringssalter og effektene av disse på alge- og dyresamfunn fra svenskegrensen til Fedje. Utvikling over tid overvåkes innen fagdisiplinene hydrografi, hydrokjemi, planteplankton, bløtbunnsfauna og hardbunnsflora og -fauna. Året 1996 ble innledet med en kald vinter og vår. Vanntemperaturen i Skagerrak ned til ca. 25 m var i januar - februar kaldere enn gjennomsnittet for 1980-1990, mens samme periode i 1995 var uvanlig varm. Til forskjell fra 1995 var det ingen flom på kontinentet eller på Østlandet i 1996, og det resulterte i små tilførsler av næringssalter fra land. Sammenlignet med langtidsobservasjoner fra Arendalstasjonene (1980-90) var 1996 et år med lavere næringssaltkonsentrasjoner i de øvre vannmasser vinter og vår og for de løste næringssaltene også tildels sommeren og høsten. C/N-forholdet i stortare fra Sørlandskysten var meget høyt og kan indikere næringsbegrenset algevekst på Sørlandskysten sommeren 1996. Utbredelse og forekomst av fastsittende alger var generelt sett noe redusert i 1996 i forhold til 1995 og mer lik hva som ble funnet i 1993/94. Variasjonene kan sees i sammenheng med vinter/vår temperaturene. Fortsatt tilvekst av tareskog i Skagerrak gjør stasjonene mer homogene. Forekomst av organismer på bløt- og hardbunn viste en normal øst-vest gradient, med økende artsantall og diversitet mot vest. Bløtbunnsfaunaen viste ingen større endringer fra 1995 til 1996. For enkelte stasjoner ble det funnet en nedgang i individtall, mens det ble funnet en økning for andre stasjoner, uten at det er noen klar årsakssammenheng eller tendens i variasjonene

    Developing Topology Discovery in Event-B

    We present a formal development in Event-B of a distributed topology discovery algorithm. Distributed topology discovery is at the core of several routing algorithms and is the problem of each node in a network discovering and maintaining information on the network topology. One of the key challenges is specifying the problem itself. Our specification includes both safety properties, formalizing invariants that should hold in all system states, and liveness properties that characterize when the system reaches stable states. We establish these by appropriately combining proofs of invariant preservation, event refinement, event convergence, and deadlock freedom. The combination of these features is novel and should be useful for formalizing and developing other kinds of semi-reactive systems, which are systems that react to, but do not modify, their environment
