    Quality of life after cesarean and vaginal delivery

    Objective: Cesarean rates in recent decades have been increasing and a number of studies have shown that cesarean increases maternal morbidities. The aim of this study is to compare the quality of life after cesarean and vaginal delivery. Methods: This prospective study was carried out on 356 pregnant women visiting urban health centers in Shahroud City, Northeast Iran, in 2011. The subjects completed the quality of life questionnaire in the third trimester of pregnancy and at 8 weeks postpartum. Results: In primiparas, the mean global QOL scores for the cesarean and vaginal delivery groups were 67.65±12.7 and 72.12±11.8, respectively. Also, the scores for the physical, psychological and social domains of QOL as well as the global score of QOL were higher in the vaginal delivery group than the cesarean group (p<0.05). In the case of primiparas, multiple regression analysis revealed that after adjusting for education, desirability of pregnancy and the General Health Questionnaire score, the delivery type remained as a predictor of the scores for the physical (R2=1.7%; B=-3.826; p=0.031; CI [-7.301, -.350]) and social (R2=2.5%; B=-5.708; p=0.017; CI [-10.392, -1.023]) domains of QOL and the global QOL score (R2=2.6%; B=-4.065; p=0.006; CI [-6.964, -1.164]). While multiparas, there was no relationship between QOL and type of delivery. Conclusion: In this sample of low-risk women, cesarean negatively affected the QOL of primiparas. More studies with larger sample sizes should be conducted to examine the effects of cesarean on QOL in both primiparas and multiparas within a shorter period after delivery. © OMSB, 2013

    Validation of the World Health Organization-5 well-being index; assessment of maternal well-being and its associated factors

    Objective: The aim of this study was to validate the World Health Organization-5 Well-Being Index (WHO-5) in a sample of pregnant Iranian women, to explore the changes in maternal well-being from pregnancy to postpartum, and to determine the factors associated with maternal well-being. Materials and Methods: This study included 341 pregnant women that presented to healthcare centers affiliated with Shahroud University of Medical Sciences, Iran, in 2011. The participants completed the General Health Questionnaire (GHQ-28) and Farsi version of WHO-5 during the third trimester of pregnancy and at 2 months postpartum. Results: Cronbach's alpha coefficient for WHO-5 items was 0.85. The correlation coefficient between WHO-5 and GHQ-28 was -0.64 (P < 0.001). Exploratory factor analysis yielded 1 factor with an eigen value equal to 3.15, which explained 63.1% of the total variance. Confirmatory factor analysis confirmed the 1-factor structure. The area under ROC curve was 0.82. A WHO-5 cut-off score of <50 exhibited optimal sensitivity (0.84) and specificity (0.59) for identifying psychological symptoms (GHQ-28 score ≥24). There was a significant difference between third trimester (58.4 ± 22) and postpartum (64.1 ± 22.3) mean WHO-5 scores (P < 0.001). Maternal level of education (OR = 1.130; 95% CI: 1.307, 1.232) was the only predictor of maternal well-being during third trimester. Breastfeeding difficulty (OR = 0.923; 95% CI: 0.882, 0.965) and maternal well-being scores during third trimester (OR = 1.038; 95% CI: 1.019, 1.058) were predictors of maternal well-being during the postpartum period. Conclusions: The Farsi version of WHO-5 was observed to be a reliable and valid instrument for screening psychological symptoms in pregnant Iranian women.     Today, the first requirement for increasing quality and strength in construction industry is utilization of appropriate and high quality materials so that usage of these materials, in addition to reducing maintenance costs and also upgrading the structure's longevity, in engineering respect through change on weight reduction and increasing strength in used materials, can cause general strength of the structure against earthquakes and probable runs due to natural disasters. This requirement is very significant in passive defense scope and this task will be considered in used material in main structure of a building and also in used materials in its frontage and in terminology elaborate work. Given that one of the effects of fire on the building part and parcel of passive defense, so, this look from the perspective of fire engineering is also of particular significance. Wide studies have been performed in this ground and various factories are trying to increase their produced materials quality in today competitive world with utilization of various technologies. One of technologies considered today is Nano technology which in fact by changing the molecular structure of a material causes creating special properties in it and utilization of these properties can answer needs of producers and users for increasing quality, longevity, strength and weight reduction. In this study, it is tried to introduce materials produced by Nano technology which have special properties like resistance against fire, settlement and weight reduction and increasing resistance in tolerance of strike and injected loads to the structure in defensive point of view in civil engineering scope. Keywords: Nano technology; passive defense; building material

    Investigation of the Effect of Modeling of Control Tunnel in Retaining Structure of J2 Station of Mashhad Metro Using Plaxis 2D

    Nowadays using of underground concrete arch excavation method is very common for execution of retaining structures (soldier structure) of deep and semi deep metro stations all around the world. In this method executer can use retaining structure as a part of main structure of station in next construction steps with using of piles of retaining structure as a part of main structure\u27s wall. One of the first steps in building retaining structure of metro station is control tunnel (patrol tunnel) that usually is a prefabricated controlling tunnel in the middle of retaining structure. This tunnel is used for digging bores for concrete piles and arches of retaining structure and will be removed at the end of excavation process. Usually designers don\u27t consider this tunnel in modeling of whole retaining structures in numerical modeling in computer programs like PLAXIS. We have done 2 different modeling of retaining structure of J2 station in Mashhad metro (with and without control tunnel) for considering of the impact of modeling of control tunnel on stresses, deflections, axial forces, bending moments, etc of the model using PLAXIS 8.5 2D. Results and differences between these two kinds of modeling showed that neglecting the patrol tunnel in analyses can cause important reductions in forces and moments of piles and ribs of the retaining structure

    Statistical approach to identify the discharge source in MV cables and accessories

    Partial discharge (PD) analysis is a reliable tool to assess the integrity of electrical insulation. Representation and interpretation of the data, obtained from e.g. online PD monitoring, are key issues to reveal the discharge source, i.e. defect type, as well as the physical phenomena behind the occurrence. Analyses of various PD patterns such as discharge height distribution presented in this work provide useful statistical parameters to identify the discharge source. Research shows that the 2-parameter Weibull distribution is a reliable model to quantify the characteristics of the patterns of the defect. The model fits well to the charge-height distribution. In addition, trends in the discharge density pattern that occur over long times, can be used as complementary information to discover the discharge nature. It alerts for a possible failure and therefore assists in taking corrective measures to prevent failure.     The road to developing economically feasible plans for green, comfortable and energy efficient buildings

    Owing to the current challenges in energy and environmental crises, improving buildings, as one of the biggest concerns and contributors to these issues, is increasingly receiving attention from the world. Due to a variety of choices and situations for improving buildings, it is important to review the building performance optimization studies to find the proper solution. In this paper, these studies are reviewed by analyzing all the different key parameters involved in the optimization process, including the considered decision variables, objective functions, constraints, and case studies, along with the software programs and optimization algorithms employed. As the core literature, 44 investigations recently published are considered and compared. The current investigation provides sufficient information for all the experts in the building sector, such as architects and mechanical engineers. It is noticed that EnergyPlus and MATLAB have been employed more than other software for building simulation and optimization, respectively. In addition, among the nine different aspects that have been optimized in the literature, energy consumption, thermal comfort, and economic benefits are the first, second, and third most optimized, having shares of 38.6%, 22.7%, and 17%, respectively

    Gum Tragacanth (GT): A Versatile Biocompatible Material beyond Borders

    The use of naturally occurring materials in biomedicine has been increasingly attracting the researchers' interest and, in this regard, gum tragacanth (GT) is recently showing great promise as a therapeutic substance in tissue engineering and regenerative medicine. As a polysaccharide, GT can be easily extracted from the stems and branches of various species of Astragalus. This anionic polymer is known to be a biodegradable, non-allergenic, non-toxic, and non-carcinogenic material. The stability against microbial, heat and acid degradation has made GT an attractive material not only in industrial settings (e.g., food packaging) but also in biomedical approaches (e.g., drug delivery). Over time, GT has been shown to be a useful reagent in the formation and stabilization of metal nanoparticles in the context of green chemistry. With the advent of tissue engineering, GT has also been utilized for the fabrication of three-dimensional (3D) scaffolds applied for both hard and soft tissue healing strategies. However, more research is needed for defining GT applicability in the future of biomedical engineering. On this object, the present review aims to provide a state-of-the-art overview of GT in biomedicine and tries to open new horizons in the field based on its inherent characteristics

    The relationship between workplace bullying and professional self-concept in Iranian nurses

    Aim: To determine the level of workplace bullying among Iranian nurses and its relationship with their professional self-concept. Design: This was a descriptive correlational study. Methods: This study was performed on 200 nurses working in the emergency departments and intensive care units of Imam Khomeini Hospital Complex affiliated to Tehran University of Medical Sciences, Iran, in 2020. The participants were chosen using the simple random sampling method and data collection tools were Negative Acts Questionnaire�Revised and Nurse Self-Concept Questionnaire. The Pearson correlation coefficient was used to determine the relationship between workplace bullying and professional self-concept. Results: Based on the experience of daily or weekly workplace bullying, the mean scores of workplace bullying in nurses in terms of work-related bullying, person-related dimension and physical intimidation were 10.11, 4.27 and 5.66, respectively, and the overall mean score was 6.68. The results of this study also showed that workplace bullying is inversely related to professional self-concept and almost all of its dimensions (p =.002, r = �.219). © 2020 The Authors.     Oral administration of the selective GPR120/FFA4 agonist compound A is not effective in alleviating tissue inflammation in mouse models of prototypical autoimmune diseases

    ω3-polyunsaturated free fatty acids (ω3-PUFAs), particularly docosahexaenoic (DHA) and eicosapentaenoic acid (EPA), are thought to exert health promoting effects in metabolic and in inflammatory diseases. The molecular mechanisms of these beneficial effects are only partially understood. DHA and EPA activate Free Fatty Acid receptor 4 (GPR120/FFA4). Recently, the first orally available, synthetic ligand of FFA4, 3-[2-chloro-5-(trifluoromethoxy)phenyl]-3-azaspiro[5.5]undecane-9-acetic acid ("compound A"; cpd A) has been developed. Cpd A exhibits distinctly higher potency, efficiency, and selectivity at FFA4 than ω3-PUFAs and ameliorates insulin resistance and adipose tissue inflammation in the mouse. With GPR120/FFA4 activation believed to also attenuate tissue inflammation in autoimmune diseases, cpd A may also have a beneficial effect in these diseases. We have therefore addressed the therapeutic potential of cpd A in mouse models of three prototypical autoimmune diseases, specifically psoriasis, rheumatoid arthritis, and bullous pemphigoid. The effect of cpd A on the course of Aldara™-induced psoriasis-like dermatitis, K/BxN serum transfer arthritis, and antibody transfer pemphigoid disease-like dermatitis was scrutinized. Cpd A did not alter the course of Aldara-induced psoriasis-like dermatitis, K/BxN serum transfer arthritis, or antibody transfer pemphigoid disease-like dermatitis. Our results suggest that therapeutic regimens solely relying on FFA4 activation do not bear the potential to treat inflammatory diseases. With cpd A distinctly more potent in activating GPR120/FFA4 than ω3-PUFAs, this also suggests that GPR120/FFA4 activation by ω3-PUFAs does not significantly contribute to the health-promoting effects of ω3-PUFAs in autoimmune diseases
