    Intrapatient comparison of Mycobacterium leprae by VNTR analysis in nasal secretions and skin biopsy in a Brazilian leprosy endemic region.

    Background: This study compares the strains of genotypes of M. leprae from nasal secretions (NS) and skin biopsy (SB) in the same patient, supplementing conventional epidemiology to gain insight into the infection of leprosy in Fortaleza, Brazil. Methods: The sample consisted of 38 newly diagnosed leprosy patients attending the National Reference Center of Dermatology Dona Libania (CDERM), in Fortaleza, who tested positive for M. leprae by PCR in DNA extracts of nasal secretions. DNA was also extracted from skin biopsy (SB) scrapings of each patient and used for multiplex PCR amplification of M. leprae VNTR loci. The number of repeats at 15 loci were determined by the fragment length analysis method. Results: Locus VNTR genotypes were achieved in 38 NS, and in 38 SB specimens. M. leprae strains differed in their genotypes in paired specimens in all but two of 38 patients. The genotype similarity in the remainder ranged from 53% to 87%. Conclusion: M. leprae 15 VNTR loci genotypes of paired nasal and biopsy skin samples from five patients were identical, while as many as seven loci differed in the 33 other patients. When the NS and biopsy genotypes were pooled and compared, it was found that there was a great variability among different VNTR markers.     Clinical presentation of leptospirosis: a retrospective study of 201 patients in a metropolitan city of Brazil

    INTRODUCTION: leptospirosis is a zoonosis of worldwide importance. The disease is endemic in Brazil. This study was conducted to describe the clinical and laboratory presentation of leptospirosis in a metropolitan city of Brazil. METHODS: this is a retrospective study including 201 consecutive patients with leptospirosis admitted to tertiary hospitals in Fortaleza, Brazil, between 1985 and 2006. All patients had clinical and epidemiological data suggestive of leptospirosis, and positive laboratorial test for leptospirosis (microscopic agglutination test, MAT, higher than 1:800). RESULTS: a total of 201 patients were included, with mean age of 38.9 ± 15.7 years; 79.1% were male. The mean length from onset of symptoms to admission was 7 ± 3 days. The main clinical signs and symptoms at admission were fever (96.5%), jaundice (94.5%), myalgia (92.5%), headache (74.6%), vomiting (71.6%) and dehydration (63.5%). Hemorrhagic manifestations were present in 35.8%. Acute kidney injury was found in 87% of the patients. Platelet count was less than 100,000/mm³ in 74.3%. Hematuria was found in 42.9%. Death occurred in 31 cases (15.4%). CONCLUSIONS: leptospirosis is a globally relevant disease with potential fatal outcome. Signs and symptoms suggestive of leptospirosis must be known by any physician in order to institute early adequate treatment to improve outcome. Early indication and daily hemodialysis seems to be beneficial in this group of patients

    Vídeo educativo como recurso para educação em saúde a pessoas com colostomia e familiares

    RESUMO Objetivo Conhecer as percepções de participantes de um grupo de apoio para pessoas com colostomia sobre a utilização de um vídeo educativo como recurso para atividade de educação em saúde. Método Pesquisa qualitativa com 16 participantes em um grupo de apoio a pessoas colostomizadas de Santa Maria, Rio Grande do Sul, Brasil. Os dados foram coletados em abril e maio de 2016, por meio de grupo focal, submetidos à análise temática proposta por Minayo. Resultados Emergiram três categorias: O cuidar e o cuidar-se aprendido sozinho: a realidade vivida; Educação em saúde e aprendizagem por meio do vídeo educativo: possibilidades percebidas; As singularidades do vídeo educativo na ótica de pessoas colostomizadas e seus familiares. Conclusão A exiguidade de orientações posterga a independência e dificulta a autonomia para o cuidado e o autocuidado. A aplicabilidade da tecnologia audiovisual desenvolvida complementa as orientações educativas, possibilitando transformar e repensar as práticas pedagógicas na enfermagem
