Incidence and predictors of left ventricular thrombus formation following acute ST-segment elevation myocardial infarction : a serial cardiac MRI study
Aims: Left ventricular (LV) thrombus is a complication of acute ST-segment elevation myocardial infarction (STEMI). We determined the incidence and predictors of LV thrombus formation using serial cardiac magnetic resonance (CMR) and two-dimensional echocardiography studies. Methods and results: Two hundred and ten patients underwent CMR (median 4 days [IQR 3-7]) and transthoracic echocardiography (median 4 days [IQR 3-7]) early after STEMI presentation with serial follow-up CMR (median 55 days [IQR 46-64]) and echocardiography studies (median 54 days [IQR 45-64]) performed subsequently. The incidence of LV thrombus was 12.3% (26/210) by CMR and 6.2% (13/210) by two-dimensional echocardiography. Echocardiography had 50% sensitivity and 100% specificity for LV thrombus detection compared to CMR. LV thrombus was found in 23.6% of patients with anterior STEMI (22/93). Ischaemic stroke occurred in 1.4% of patients (3/210). Patients with LV thrombus had lower baseline LV ejection fraction (LVEF) (34.9% vs 47.4%, p < 0.001). Microvascular obstruction was more common in patients with LV thrombus (77% vs 39%, p < 0.001). Patients with LV thrombus had increased LV dimensions with larger LV end-diastolic (19 ml [IQR 9-44] vs 6 ml [IQR -4-18], p < 0.001) and end-systolic volumes (10 ml [IQR 0–22] vs -4 ml [IQR -12-4], p < 0.001). Conclusion: CMR increases the detection of LV thrombi which standard echocardiography may underestimate. Serial studies post-STEMI may improve detection of LV thrombus, which is more prevalent in patients with anterior infarction, moderate LV dysfunction and adverse LV remodelling. This subgroup of patients may represent a high-risk group for targeted serial screening with CMR
The epidemiology of lymphatic filariasis in Ghana, explained by the possible existence of two strains of Wuchereria bancrofti
Introduction
Lymphatic filariasis is a debilitating disease caused by the filarial worm Wuchereria bancrofti. It is earmarked for elimination by the year 2020 through the Global Program for the Elimination of LF (GPELF). In Ghana, mass treatment has been on-going since the year 2000. Earlier studies have revealed differing epidemiology of LF in the North and South of Ghana. This study was therefore aimed at understanding the possible impacts of W. bancrofti diversity on the epidemiology and control of LF in Ghana.
Methods
The Mitochondrial, Cytochrome C Oxidase I gene of W. bancrofti samples was sequenced and analyzed. The test sequences were grouped into infrapopulations, and pairwise differences (Π) and mutation rates (θ) were computed. The amount of variance within and among populations was also computed using the AMOVA. The evolutionary history was inferred using the Maximum Parsimony method.
Results
Seven samples from the South and 15 samples from the North were sequenced, and submitted to GenBank with accession numbers GQ479497- GQ479518. The results revealed higher mutation frequencies in the southern population, compared to the northern population. Haplotype analyses revealed a total of 11 haplotypes (Hap) in all the 22 DNA sequences, with high genetic variation and polymorphisms within the southern samples.
Conclusion
This study showed that there is considerable genetic variability within W. bancrofti populations in Ghana, differences that might explain the observed epidemiology of LF. Further studies are however required for an in-depth understanding of LF epidemiology and control
Large-Scale Turnover of Functional Transcription Factor Binding Sites in Drosophila
The gain and loss of functional transcription factor binding sites has been proposed as a major source of evolutionary change in cis-regulatory DNA and gene expression. We have developed an evolutionary model to study binding-site turnover that uses multiple sequence alignments to assess the evolutionary constraint on individual binding sites, and to map gain and loss events along a phylogenetic tree. We apply this model to study the evolutionary dynamics of binding sites of the Drosophila melanogaster transcription factor Zeste, using genome-wide in vivo (ChIP–chip) binding data to identify functional Zeste binding sites, and the genome sequences of D. melanogaster, D. simulans, D. erecta, and D. yakuba to study their evolution. We estimate that more than 5% of functional Zeste binding sites in D. melanogaster were gained along the D. melanogaster lineage or lost along one of the other lineages. We find that Zeste-bound regions have a reduced rate of binding-site loss and an increased rate of binding-site gain relative to flanking sequences. Finally, we show that binding-site gains and losses are asymmetrically distributed with respect to D. melanogaster, consistent with lineage-specific acquisition and loss of Zeste-responsive regulatory elements

There is a growing realisation that to better understand and intervene in the interconnected political, cultural and psycho-social dynamics that constitute the contexts for these current global challenges requires transdisciplinary and intersectoral approaches; approaches that include and value diverse perspectives and pay particular attention to the perspectives and experiences of those who are the most vulnerable and those who are currently excluded from the knowledge creation processes. This article discusses the learning gained from an inter-disciplinary and inter-institutional project entitled Transformative Engagement Network (TEN).
 
(31) P and (1) H MRS of DB-1 melanoma xenografts: lonidamine selectively decreases tumor intracellular pH and energy status and sensitizes tumors to melphalan.
In vivo (31) P MRS demonstrates that human melanoma xenografts in immunosuppressed mice treated with lonidamine (LND, 100 mg/kg intraperitoneally) exhibit a decrease in intracellular pH (pH(i) ) from 6.90 ± 0.05 to 6.33 ± 0.10 (p \u3c 0.001), a slight decrease in extracellular pH (pH(e) ) from 7.00 ± 0.04 to 6.80 ± 0.07 (p \u3e 0.05) and a monotonic decline in bioenergetics (nucleoside triphosphate/inorganic phosphate) of 66.8 ± 5.7% (p \u3c 0.001) relative to the baseline level. Both bioenergetics and pH(i) decreases were sustained for at least 3 h following LND treatment. Liver exhibited a transient intracellular acidification by 0.2 ± 0.1 pH units (p \u3e 0.05) at 20 min post-LND, with no significant change in pH(e) and a small transient decrease in bioenergetics (32.9 ± 10.6%, p \u3e 0.05) at 40 min post-LND. No changes in pH(i) or adenosine triphosphate/inorganic phosphate were detected in the brain (pH(i) , bioenergetics; p \u3e 0.1) or skeletal muscle (pH(i) , pH(e) , bioenergetics; p \u3e 0.1) for at least 120 min post-LND. Steady-state tumor lactate monitored by (1) H MRS with a selective multiquantum pulse sequence with Hadamard localization increased approximately three-fold (p = 0.009). Treatment with LND increased the systemic melanoma response to melphalan (LPAM; 7.5 mg/kg intravenously), producing a growth delay of 19.9 ± 2.0 days (tumor doubling time, 6.15 ± 0.31 days; log(10) cell kill, 0.975 ± 0.110; cell kill, 89.4 ± 2.2%) compared with LND alone of 1.1 ± 0.1 days and LPAM alone of 4.0 ± 0.0 days. The study demonstrates that the effects of LND on tumor pH(i) and bioenergetics may sensitize melanoma to pH-dependent therapeutics, such as chemotherapy with alkylating agents or hyperthermia
High-resolution, H band Spectroscopy of Be Stars with SDSS-III/APOGEE: I. New Be Stars, Line Identifications, and Line Profiles
APOGEE has amassed the largest ever collection of multi-epoch,
high-resolution (R~22,500), H-band spectra for B-type emission line (Be) stars.
The 128/238 APOGEE Be stars for which emission had never previously been
reported serve to increase the total number of known Be stars by ~6%. We focus
on identification of the H-band lines and analysis of the emission peak
velocity separations (v_p) and emission peak intensity ratios (V/R) of the
usually double-peaked H I and non-hydrogen emission lines. H I Br11 emission is
found to preferentially form in the circumstellar disks at an average distance
of ~2.2 stellar radii. Increasing v_p toward the weaker Br12--Br20 lines
suggests these lines are formed interior to Br11. By contrast, the observed IR
Fe II emission lines present evidence of having significantly larger formation
radii; distinctive phase lags between IR Fe II and H I Brackett emission lines
further supports that these species arise from different radii in Be disks.
Several emission lines have been identified for the first time including
~16895, a prominent feature in the spectra for almost a fifth of the sample
and, as inferred from relatively large v_p compared to the Br11-Br20, a tracer
of the inner regions of Be disks. Unlike the typical metallic lines observed
for Be stars in the optical, the H-band metallic lines, such as Fe II 16878,
never exhibit any evidence of shell absorption, even when the H I lines are
clearly shell-dominated. The first known example of a quasi-triple-peaked Br11
line profile is reported for HD 253659, one of several stars exhibiting intra-
and/or extra-species V/R and radial velocity variation within individual
spectra. Br11 profiles are presented for all discussed stars, as are full
APOGEE spectra for a portion of the sample.Comment: accepted in A
The Open Cluster Chemical Analysis and Mapping Survey: Local Galactic Metallicity Gradient with APOGEE using SDSS DR10
The Open Cluster Chemical Analysis and Mapping (OCCAM) Survey aims to produce
a comprehensive, uniform, infrared-based dataset for hundreds of open clusters,
and constrain key Galactic dynamical and chemical parameters from this sample.
This first contribution from the OCCAM survey presents analysis of 141 members
stars in 28 open clusters with high-resolution metallicities derived from a
large uniform sample collected as part of the SDSS-III/Apache Point Observatory
Galactic Evolution Experiment (APOGEE). This sample includes the first
high-resolution metallicity measurements for 22 open clusters. With this
largest ever uniformly observed sample of open cluster stars we investigate the
Galactic disk gradients of both [M/H] and [alpha/M]. We find basically no
gradient across this range in [alpha/M], but [M/H] does show a gradient for
R_{GC} < 10 kpc and a significant flattening beyond R_{GC} = 10 kpc. In
particular, whereas fitting a single linear trend yields an [M/H] gradient of
-0.09 +/- 0.03$ dex/kpc --- similar to previously measure gradients inside 13
kpc --- by independently fitting inside and outside 10 kpc separately we find a
significantly steeper gradient near the Sun (7.9 <= R_{GC} <= 10) than
previously found (-0.20 +/- 0.08 dex/kpc) and a nearly flat trend beyond 10 kpc
