    Correlation of Mandibular Second Molar Impaction with Third Molar Size, Angulation, Developmental Stage, and Bud Position

    Objective: The present study assessed the association of size, axial angulation, developmental stage and bud position of the mandibular third molar with the second molar impaction in patients with mandibular second molar impaction.Methods: In this descriptive-analytical study (Case-Control Design), dental records of 5,420 patients in the age range of 12-15 years who underwent orthodontic treatment were assessed out of which 20 patients (14 females and 6 males) with lower second molar impaction were studied. Twenty control patients with erupted second molars were selected from the same centers and matched with the case group. Total sample size was 40 (Case and Control) and had normal distribution for the determined measurements. Third molar axial angulation towards the second molar, first molar and mandibular base, mesiodistal width of 3rd molar /2nd molar ratio, Nolla developmental stage and third molar bud position were measured and calculated in patients with impacted and erupted second molars and analyzed using student t and chi-square tests.Results: The mean degree of third molar angulation towards the second molar, first molar and mandibular base was 30.20, 53.6, and 51.3 degrees in cases and 21.4, 34.65 and 45.15 degrees in controls, respectively. Significant differences were found between the two groups in terms of third molar angulation towards the second molar (30.2 Vs 21.4 degrees p<0.047), and first molar (53.6 Vs34.65 degrees p<0.0001); while no differences were found between the two groups in terms of third molar angulation towards the mandibular base (51.3 Vs 45.15 degrees). The mean mesiodistal width of third molar/ second molar ratio was 0.99 in cases and 0.95 in controls with no significant difference. Nolla developmental stage and position of the marginal ridge of the third molar bud towards the second molar showed no definite relation with second molar impaction.Conclusion: It seems that angulation of third molar to the second and first molars is a major contributing factor to increase the risk of second molar impaction. Third molars size, developmental stages, and bud positions, do not show a significant relation to the second molar impaction

    Polycyclic aromatic hydrocarbons (PAHs) in coastal sediments from urban and industrial areas of Asaluyeh Harbor, Iran: distribution, potential source and ecological risk assessment

    The distribution and toxicity levels of 16 EPA priority pollutant polycyclic aromatic hydrocarbons (PAHs) in the sediments of Asaluyeh shore, Iran were investigated. The total concentrations of the PAHs in surface sediments ranged from 1,054 to 17,448 ng/g dry weights with a mean concentration of 8,067 ng/g. The spatial distribution of PAHs showed that PAH levels are much higher in the industrial areas in comparison with urban areas. Based on diagnostic ratios, pyrogenic activities were dominant sources of PAHs pollution in sediments comparing petroleum sources. The toxic equivalent concentrations (TEQ Carc) of PAHs ranged from 172 to 2,235 ng TEQ/g with mean value of 997.9. Toxicity levels were evaluated using sediment quality guidelines (SQGs) and toxic equivalent factors. Samples were collected from industrial and urban stations in Asaluyeh shores. According to SQGs, ΣPAHs concentrations in sediments of urban areas were below the ERL (effects range low), but the industrial samples had ΣPAHs concentrations between ERL and ERM (effects range median). Furthermore, ΣHPAHs (heavy PAHs) and some individual PAHs in some industrial stations exceeded ERM, indicating adverse ecological risk effects frequently occur. Findings demonstrate that the surface sediment from Asaluyeh shore is highly to very highly contaminated with PAHs

    Comparison of Peripapillary Retinal Nerve Fiber Layer Thickness in Patients with MS and Normal Population

    Purpose: To compare peripapillary retinal nerve fiber layer thickness (RNFLT) between patients with multiple sclerosis (MS) and healthy controls using optical coherence tomography (OCT).  Patients and Methods: In this prospective case control study, peripapillary RNFLT of 120 eyes from 60 patients with multiple sclerosis (MS)  was compared to 120 eyes from 60 age and sex matched healthy controls using OCT.  The RNFLT in 4 peripapillary quadrants and the mean RNFLT of all four quadrants were compared between the case and control groups. The relation between MS variables such as age of onset, type and duration of disease, history of optic neuritis (ON) and other non-ocular episodes with RNFLT was evaluated in the case group. Results: The mean RNFLT of all four quarters was significantly lower in patients with MS compared to the controls (P < 0.001). Also RNFLT was significantly lower in each of 4 quadrants (superior, temporal, inferior; P < 0.001, nasal P = 0.003). There was no significant relation between RNFLT, the age of onset of MS disease, and history of non-ocular episodes. RNFLT had a significant relation with duration of the disease (P < 0.001), the type of MS (P < 0.001), history of ON (P = 0.002), and the number of ON episodes (P = 0.021). Conclusion: We found that RNFLT decreases in MS patients and its reduction is related to the duration and type of disease as well as history and number of ON episodes. Therefore measuring RNFLT may help in estimating the progress of MS and can potentially be included as a part of patients’ follow up protocol.Keywords: Multiple sclerosis;  Tomography; Optical Coherence;;Optic Neuritis; Retinal; Nerve Fibers

    Predictors of Death in the Liver Transplantation Adult Candidates: An Artificial Neural Networks and Support Vector Machine Hybrid-Based Cohort Study

    Background: Model for end-stage liver disease (MELD) is currently used for liver transplantation (LT) allocation, however, it is not a sufficient criterion. Objective: This current study aims to perform a hybrid neural network analysis of different data, make a decision tree and finally design a decision support system for improving LT prioritization.Material and Methods: In this cohort follow-up-based study, baseline characteristics of 1947 adult patients, who were candidates for LT in Shiraz Organ Transplant Center, Iran, were assessed and followed for two years and those who died before LT due to the end-stage liver disease were considered as dead cases, while others considered as alive cases. A well-organized checklist was filled for each patient. Analysis of the data was performed using artificial neural networks (ANN) and support vector machines (SVM). Finally, a decision tree was illustrated and a user friendly decision support system was designed to assist physicians in LT prioritization. Results: Between all MELD types, MELD-Na was a stronger determinant of LT candidates’ survival. Both ANN and SVM showed that besides MELD-Na, age and ALP (alkaline phosphatase) are the most important factors, resulting in death in LT candidates. It was cleared that MELD-Na <23, age <53 and ALP <257 IU/L were the best predictors of survival in LT candidates. An applicable decision support system was designed in this study using the above three factors.  Conclusion: Therefore, Meld-Na, age and ALP should be used for LT allocation. The presented decision support system in this study will be helpful in LT prioritization by LT allocators
