State of Nebraska Digital Equity Plan: Understanding the Digital Equity Needs of Covered Populations in Nebraska
According to 2021 estimates from the U.S. Census Bureau’s American Community Survey, 7% of Nebraska households do not have access to a computer and 6% of Nebraska households have access to a computer but no internet. Although recent efforts suggest there is momentum and government action at both the federal and state levels to address lack of broadband (Hammel, 2023; Newman, 2023), this still leaves a considerable number of Nebraskans without access to reliable broadband in the meantime. Furthermore, broadband access is moot if Nebraskans do not have access or cannot afford technology and the cost of internet in the first place. The purpose of this report is to take a deeper look at specific covered populations in Nebraska who are more likely to be without affordable, reliable technology access and, therefore, are disproportionally impacted by the digital divide. The research team, representing the University of Nebraska at Omaha Center for Public Affairs Research and Department of Gerontology, was approached by the Nebraska Information Technology Commission to conduct focus groups with covered populations. Key findings from focus groups are shared. Findings indicate that access to technology and internet is not only vital for all Nebraskans but is also costprohibitive for already marginalized populations. Moving forward, these findings will inform the creation of a state digital equity plan for Nebraska
Working Memory in Collegiate Athletes and Non-Athletes: A Comparison of Team-Sports Athletes, Solo-Sports Athletes, Frequent Exercisers and Infrequent Exercisers
Past research suggests that exercise is associated with improved working memory (WM) abilities, and that athletes have better executive functioning and WM skills than non-athletes. However, no studies to date have separated the effects of exercise from the effects of athletic participation when examining WM. This study had three primary goals: (1) to determine whether amateur college athletes have better auditory and visual WM than their non-athlete peers who exercise frequently and whether non-athlete frequent exercisers have better WM than non-athlete infrequent exercisers (2) to determine whether amateur participation in particular types of sports (team-based versus solo sports) corresponds to differences in auditory or visual WM, and (3) to examine whether gender plays a role in the relationships between exercise, athletic participation and WM. This study was a cross-sectional, correlational design that examined WM in men versus women college students
divided into four groups (team-sports athletes, solo-sports athletes, frequent exercisers, and infrequent exercisers). 153 college students completed five tests of auditory (Digit Span subtest, PASAT) and visual (Spatial Span subtest, N-Back, WCST) WM. Collegiate men who participated in team-based sports outperformed collegiate men who participated in solo-sports in auditory WM. The two non-athlete groups (frequent and infrequent exercisers) earned scores between those of the two athlete groups. Male infrequent exercisers demonstrated better visual WM than the other three male athlete/exerciser groups. In contrast, neither athletic participation nor exercise affected collegiate women’s WM abilities. Our results did not support our hypothesis that all types of student athletes would exhibit better WM than students who exercise regularly but do not engage in competitive sports. Additionally, our data did not indicate that frequent exercise is associated with stronger WM skills than infrequent exercise in non-athlete college students. Participation in team-based sports was associated with better auditory WM skills than participation in solo sports, but this was only true for male, not female, athletes. Thus, gender was an important factor in delineating the relationships between exercise, athletic participation and WM
Smooth muscle archvillin: a novel regulator of signaling and contractility in vascular smooth muscle
The mechanisms by which protein kinase C (PKC) and extracellular-signal-regulated kinases (ERK1/2) govern smooth-muscle contractility remain unclear. Calponin (CaP), an actin-binding protein and PKC substrate, mediates signaling through ERK1/2. We report here that CaP sequences containing the CaP homology (CH) domain bind to the C-terminal 251 amino acids of smooth-muscle archvillin (SmAV), a new splice variant of supervillin, which is a known actin- and myosin-II-binding protein. The CaP-SmAV interaction is demonstrated by reciprocal yeast two-hybrid and blot-overlay assays and by colocalization in COS-7 cells. In differentiated smooth muscle, endogenous SmAV and CaP co-fractionate and co-translocate to the cell cortex after stimulation by agonist. Antisense knockdown of SmAV in tissue inhibits both the activation of ERK1/2 and contractions stimulated by either agonist or PKC activation. This ERK1/2 signaling and contractile defect is similar to that observed in CaP knockdown experiments. In A7r5 smooth-muscle cells, PKC activation by phorbol esters induces the reorganization of endogenous, membrane-localized SmAV and microfilament-associated CaP into podosome-like structures that also contain F-actin, nonmuscle myosin IIB and ERK1/2. These results indicate that SmAV contributes to the regulation of contractility through a CaP-mediated signaling pathway, involving PKC activation and phosphorylation of ERK1/2
Programs and processes for advancing pediatric acute kidney support therapy in hospitalized and critically ill children: A report from the 26th Acute Disease Quality Initiative (ADQI) consensus conference
Pediatric acute kidney support therapy (paKST) programs aim to reliably provide safe, effective, and timely extracorporeal supportive care for acutely and critically ill pediatric patients with acute kidney injury (AKI), fluid and electrolyte derangements, and/or toxin accumulation with a goal of improving both hospital-based and lifelong outcomes. Little is known about optimal ways to configure paKST teams and programs, pediatric-specific aspects of delivering high-quality paKST, strategies for transitioning from acute continuous modes of paKST to facilitate rehabilitation, or providing effective short- and long-term follow-up. As part of the 26th Acute Disease Quality Initiative Conference, the first to focus on a pediatric population, we summarize here the current state of knowledge in paKST programs and technology, identify key knowledge gaps in the field, and propose a framework for current best practices and future research in paKST

Individualized decision aid for diverse women with lupus nephritis (IDEA-WON): A randomized controlled trial.
BackgroundTreatment decision-making regarding immunosuppressive therapy is challenging for individuals with lupus. We assessed the effectiveness of a decision aid for immunosuppressive therapy in lupus nephritis.Methods and findingsIn a United States multicenter, open-label, randomized controlled trial (RCT), adult women with lupus nephritis, mostly from racial/ethnic minority backgrounds with low socioeconomic status (SES), seen in in- or outpatient settings, were randomized to an individualized, culturally tailored, computerized decision aid versus American College of Rheumatology (ACR) lupus pamphlet (1:1 ratio), using computer-generated randomization. We hypothesized that the co-primary outcomes of decisional conflict and informed choice regarding immunosuppressive medications would improve more in the decision aid group. Of 301 randomized women, 298 were analyzed; 47% were African-American, 26% Hispanic, and 15% white. Mean age (standard deviation [SD]) was 37 (12) years, 57% had annual income of <$40,000, and 36% had a high school education or less. Compared with the provision of the ACR lupus pamphlet (n = 147), participants randomized to the decision aid (n = 151) had (1) a clinically meaningful and statistically significant reduction in decisional conflict, 21.8 (standard error [SE], 2.5) versus 12.7 (SE, 2.0; p = 0.005) and (2) no difference in informed choice in the main analysis, 41% versus 31% (p = 0.08), but clinically meaningful and statistically significant difference in sensitivity analysis (net values for immunosuppressives positive [in favor] versus negative [against]), 50% versus 35% (p = 0.006). Unresolved decisional conflict was lower in the decision aid versus pamphlet groups, 22% versus 44% (p < 0.001). Significantly more patients in the decision aid versus pamphlet group rated information to be excellent for understanding lupus nephritis (49% versus 33%), risk factors (43% versus 27%), medication options (50% versus 33%; p ≤ 0.003 for all); and the ease of use of materials was higher in the decision aid versus pamphlet groups (51% versus 38%; p = 0.006). Key study limitations were the exclusion of men, short follow-up, and the lack of clinical outcomes, including medication adherence.ConclusionsAn individualized decision aid was more effective than usual care in reducing decisional conflict for choice of immunosuppressive medications in women with lupus nephritis.Trial registrationClinicaltrials.gov, NCT02319525
Rationale and study protocol for the \u27Active Teen Leaders Avoiding Screen-time\u27 (ATLAS) group randomized controlled trial: An obesity prevention intervention for adolescent boys from schools in low-income communities
Introduction The negative consequences of unhealthy weight gain and the high likelihood of pediatric obesity tracking into adulthood highlight the importance of targeting youth who are \u27at risk\u27 of obesity. The aim of this paper is to report the rationale and study protocol for the \u27Active Teen Leaders Avoiding Screen-time\u27 (ATLAS) obesity prevention intervention for adolescent boys living in low-income communities. Methods/design The ATLAS intervention will be evaluated using a cluster randomized controlled trial in 14 secondary schools in the state of New South Wales (NSW), Australia (2012 to 2014). ATLAS is an 8-month multi-component, school-based program informed by self-determination theory and social cognitive theory. The intervention consists of teacher professional development, enhanced school-sport sessions, researcher-led seminars, lunch-time physical activity mentoring sessions, pedometers for self-monitoring, provision of equipment to schools, parental newsletters, and a smartphone application and website. Assessments were conducted at baseline and will be completed again at 9- and 18-months from baseline. Primary outcomes are body mass index (BMI) and waist circumference. Secondary outcomes include BMI z-scores, body fat (bioelectrical impedance analysis), physical activity (accelerometers), muscular fitness (grip strength and push-ups), screen-time, sugar-sweetened beverage consumption, resistance training skill competency, daytime sleepiness, subjective well-being, physical self-perception, pathological video gaming, and aggression. Hypothesized mediators of behavior change will also be explored. Discussion ATLAS is an innovative school-based intervention designed to improve the health behaviors and related outcomes of adolescent males in low-income communities
The Murchison Widefield Array Transients Survey (MWATS). A search for low frequency variability in a bright Southern hemisphere sample
We report on a search for low-frequency radio variability in 944 bright (>
4Jy at 154 MHz) unresolved, extragalactic radio sources monitored monthly for
several years with the Murchison Widefield Array. In the majority of sources we
find very low levels of variability with typical modulation indices < 5%. We
detect 15 candidate low frequency variables that show significant long term
variability (>2.8 years) with time-averaged modulation indices M = 3.1 - 7.1%.
With 7/15 of these variable sources having peaked spectral energy
distributions, and only 5.7% of the overall sample having peaked spectra, we
find an increase in the prevalence of variability in this spectral class. We
conclude that the variability seen in this survey is most probably a
consequence of refractive interstellar scintillation and that these objects
must have the majority of their flux density contained within angular diameters
less than 50 milli-arcsec (which we support with multi-wavelength data). At 154
MHz we demonstrate that interstellar scintillation time-scales become long
(~decades) and have low modulation indices, whilst synchrotron driven
variability can only produce dynamic changes on time-scales of hundreds of
years, with flux density changes less than one milli-jansky (without
relativistic boosting). From this work we infer that the low frequency
extra-galactic southern sky, as seen by SKA-Low, will be non-variable on
time-scales shorter than one year.Comment: 19 pages, 11 figure
Isolation Precautions for Visitors
Transmission of organisms within the hospital setting has become a topic of major concern not only for patients and healthcare facilities but also for government agencies and the general public. This increased awareness has occurred in part due to the spread of organisms that have limited treatment options, such as carbapenem-resistant Enterobacteriaceae (CRE), as well as the heightened recognition that many hospital-associated infections (HAIs) are preventable. A large body of literature shows that horizontal transmission of multidrug-resistant organisms involves the hands, and potentially the attire, of healthcare workers (HCWs). This evidence provides the rationale for the use of standard and contact isolation precautions among HCWs. However, the health risks to visitors and the role of visitors in the horizontal transmission of pathogens within acute care hospitals is not as clearly defined. Consequently, uncertainty remains regarding which precautions visitors should take when interacting with patients placed on isolation precautions. Frequent arguments against the use of isolation precautions among visitors include lack of visitor movement between patient rooms, the difficulty of educating visitors, and the difficulty of enforcing compliance with isolation practices
Projected Evolution of California's San Francisco Bay-Delta-River System in a Century of Climate Change
Background: Accumulating evidence shows that the planet is warming as a response to human emissions of greenhouse gases. Strategies of adaptation to climate change will require quantitative projections of how altered regional patterns of temperature, precipitation and sea level could cascade to provoke local impacts such as modified water supplies, increasing risks of coastal flooding, and growing challenges to sustainability of native species. Methodology/Principal Findings: We linked a series of models to investigate responses of California’s San Francisco Estuary-Watershed (SFEW) system to two contrasting scenarios of climate change. Model outputs for scenarios of fast and moderate warming are presented as 2010–2099 projections of nine indicators of changing climate, hydrology and habitat quality. Trends of these indicators measure rates of: increasing air and water temperatures, salinity and sea level; decreasing precipitation, runoff, snowmelt contribution to runoff, and suspended sediment concentrations; and increasing frequency of extreme environmental conditions such as water temperatures and sea level beyond the ranges of historical observations. Conclusions/Significance: Most of these environmental indicators change substantially over the 21 st century, and many would present challenges to natural and managed systems. A Multi-City Comparison of Front and Backyard Differences in Plant Species Diversity and Nitrogen Cycling in Residential landscapes
We hypothesize that lower public visibility of residential backyards reduces households’ desire for social conformity, which alters residential land management and produces differences in ecological composition and function between front and backyards. Using lawn vegetation plots (7 cities) and soil cores (6 cities), we examine plant species richness and evenness and nitrogen cycling of lawns in Boston, Baltimore, Miami, Minneapolis-St. Paul, Phoenix, Los Angeles (LA), and Salt Lake City (SLC). Seven soil nitrogen measures were compared because different irrigation and fertilization practices may vary between front and backyards, which may alter nitrogen cycling in soils. In addition to lawn-only measurements, we collected and analyzed plant species richness for entire yards—cultivated (intentionally planted) and spontaneous (self-regenerating)—for front and backyards in just two cities: LA and SLC. Lawn plant species and soils were not different between front and backyards in our multi-city comparisons. However, entire-yard plant analyses in LA and SLC revealed that frontyards had significantly fewer species than backyards for both cultivated and spontaneous species.
