    Exploring accounts of joint working between Speech and Language Therapists and Stroke Association Communication Support Coordinators Introduction Both Speech and Language Therapists (SLTs) and Stroke Association Communication Support Coordinators (CSCs) are employed across the UK to provide services for people with communication difficulties following Stroke. The two roles are usually employed by different sectors and are subject to very different levels of qualification and professional regulation. Despite recommendations that the two roles work together, there is little existing literature examining current practices of joint working between SLTs and CSCs. Method Data were collected through a series of in depth individual interviews with five CSCs and seven SLTs working within the East of England. Data were analysed inductively using Thematic Analysis (Braun & Clark, 2006). Results Six themes were developed which highlighted the perceived benefits and challenges in this example of cross sector working. Both SLTs and CSCs identified strong incentives for joint working. The themes developed suggested a number of processes are engaged in negotiating the joint working relationship. These include ‘Developing and earning trust and respect’; ‘Pushing to establish place’; and ‘Struggling against external pressures and threats’. In addition, two further themes were developed to explore the process of finding agreement in the division of workload: ‘Sharing Out’ of aphasia; and ‘Local level Negotiation and Matching of expectations’. Conclusions: The findings suggest a number of unique features which characterise joint working relationships between SLTs and CSCs. Clinical applications based on the findings are discussed, along with suggested areas for further research

    The use and effectiveness of the eLib subject gateways: a preliminary investigation

    Internet subject gateways were set up under the Electronic Libraries Programme (eLib) in order to address some of the problems of searching the Internet which have been identified by information professionals, i.e. locating relevant, good quality information. This preliminary study examines the extent to which academics in two universities use three eLib subject gateways (EEVL, OMNI and SOSIG). The results are generally encouraging for the eLib programme, but it is necessary for the gateways to be more effectively promoted. The study also found that academics do not have the same misgivings about the general search engines as the information professionals and seem to use them more readily than the gateways

    Effect of the glucocorticoid receptor antagonist Org 34850 on fast and delayed feedback of corticosterone release

    We investigated the effect of the glucocorticoid receptor (GR) antagonist Org 34850 on fast and delayed inhibition of corticosterone secretion in response to the synthetic glucocorticoid methylprednisolone (MPL). Male rats were implanted with a catheter in the right jugular vein, for blood sampling and MPL administration, and with an s.c. cannula for Org 34850 administration. All experiments were conducted at the diurnal hormonal peak in the late afternoon. Rats were connected to an automated sampling system and blood samples were collected every 5 or 10 min. Org 34850 (10 mg/kg, s.c.) or vehicle (5% mulgofen in saline) was injected at 1630 h; 30 min later, rats received an injection of MPL (500 μg/rat, i.v.) or saline (0.1 ml/rat). We found that an acute administration of MPL rapidly decreased the basal corticosterone secretion and this effect was not prevented by acute pretreatment with Org 34850. However, blockade of GR with Org 34850 prevented delayed inhibition of MPL on corticosterone secretion measured between 4 and 12 h after MPL administration. Our data suggest an involvement of GR in modulating delayed, but not fast, inhibition induced by MPL on basal corticosterone secretion

    The effects of disease on optimal forest rotation : a generalisable analytical framework

    This work is from the project titled Modelling economic impact and strategies to increase resilience against tree disease outbreaks. This is one of seven projects in the Tree Health and Plant Biosecurity Initiative (phase 2) funded by BBSRC, Defra, ESRC, Forestry Commission, NERC and Scottish Government.The arrival of novel pathogens and pests can have a devastating effect on the market values of forests. Calibrating management strategies/decisions to consider the effect of disease may help to reduce disease impacts on forests. Here, we use a novel generalisable, bioeconomic model framework, which combines an epidemiological compartmental model with a Faustmann optimal rotation length model, to explore the management decision of when to harvest a single rotation, even-aged, plantation forest under varying disease conditions. Sensitivity analysis of the rate of spread of infection and the effect of disease on the timber value reveals a key trade-off between waiting for the timber to grow and the infection spreading further. We show that the optimal rotation length, which maximises the net present value of the forest, is reduced when timber from infected trees has no value; but when the infection spreads quickly, and the value of timber from infected trees is non-zero, it can be optimal to wait until the disease-free optimal rotation length to harvest. Our original approach provides an exemplar framework showing how a bioeconomic model can be used to examine the effect of tree diseases on management strategies/decisions.Publisher PDFPeer reviewe

    Constraint-Induced aphasia therapy: Three single case studies.

    TITLE OF POSTER: CONSTRAINT INDUCED APHASIA THERAPY - THREE SINGLE CASE STUDIES INTRODUCTION This research project was inspired by the Specific Interest Group in Aphasia study day given by Professor Pulvermuller in January 2011. Constraint Induced Aphasia Therapy (CIAT) is sometimes referred to as Intensive Language Action Therapy (ILAT). This technique has an impressive research base to support its use for people with aphasia (PWA) and the evidence base includes both single case and randomised control trial evidence (Pulvermuller, Neininger, Elbert, Mohr, Rockstroh, Koebbl & Taub, 2001). Pulvermuller et al (2001) describe CIAT as a game of pairs. The game is normally played by four players: a therapist and three PWA. A 32 pack of cards consisting of 16 pairs is shuffled and divided equally between the four players. No one can see anyone else’s cards. The aim of the game is to win the most pairs of cards. Clients can win a pair by asking each of the other players in turn, for a matching card. All requests and responses should be verbal but participants may describe the target word or use a gesture in order to achieve saying the word. An essential part of the game is that each player should see and hear the name of the card in play. RESEARCH METHOD Three PWA were prioritised from a typical general hospital SLT caseload. All three clients were one to two years post onset of their aphasia and had already been provided with impairment focussed, functional and psychosocial SLT input. They had moderate to severe aphasia, had difficulties at several levels of single word processing and frequently failed to convey their message despite having a degree of linguistic competence and an ability to use alternative communication strategies. Accessible formal assessment measures (Kaplan, Goodglass & Weintraub 1983, Goodglass, Kaplan & Barresi 2000 & Swinburn, Porter & Howard 2004) were used to evaluate the success of therapy in a four step repeated measures research design: Baseline 1, 30 hours CIAT therapy, Baseline 2, Baseline 3. SUMMARY OF RESULTS For 2/3 clients, assessment results suggested that participation in the CIAT programme resulted in a positive measurable change in language behaviour. This positive change was not apparent in assessments of understanding. Selective improvement of language ability suggests that the therapy directed at language output had resulted in improved word and sentence level skills for 2/3 of our clients. There were also positive changes that were not captured by the assessment data: reduced use of written and therapist cues, improved repetition skills, improved self-monitoring and a reduced tendency to produce jargon type utterances. Relatives corroborated therapist perceptions. CONCLUSIONS In 2006 Beeson & Robey (2006, p162) proposed that rehabilitation outcome research should be conducted in five phases. Our study is one of the first to provide evidence to support the use of intensive CIAT therapy in the community (phase 4 – an effectiveness study). Other studies have assessed the usefulness of the therapeutic effect of ILAT (phase 1), optimised the ILAT procedure (phase 2) and tested its usefulness under ideal conditions (phase 3). The final phase outlined by Beeson & Robey (2006) is the cost-benefit analysis (phase 5) and we would welcome debate within the profession on improving this type of input and the role of Speech and Language Therapists in intensive aphasia therapy provision.     A terrestrial brine-seepage analog for Martian slope streaks near Salar de Pedernales in the Atacama Desert, Chile

    Martian slope streaks are elongated down-slope, low-albedo surface features that currently form within sub-annual time scales in the high-albedo (“dusty”) regions of equatorial Mars. These km-scale streaks, which can persist up to several decades on the Martian surface, present one of the most enigmatic and dynamic suites of active surface features on present-day Mars. Two categories of explanations remain in debate regarding their nature: 1) “Dry” formation models, in which surface darkening is associated with mass wasting processes, such as dust avalanches; and 2) “Wet” formation models, in which surface darkening is associated with transient wetting of the surface by either seepage of sub-surface brines or deliquescence of atmospheric moisture. Here, we report recently discovered dark slope streaks that occur in the high Atacama Desert in Chile and display a close geomorphic resemblance to the Martian streaks. Field examination of the Atacama slope streaks revealed that they formed through down-slope seepage of groundwater brines sourced from the Salar de Pedernales located 500 m away. Chemical and mineralogical analyses demonstrate that salts deposited from the Pedernales brines combined with detrital input from soils/dust are responsible for surface darkening in the Atacama case. Field-based spectral measurements in the 0.4-2.5 micron wavelength range compare to those obtained from orbit for the Martian slope streaks. In addition, high-resolution topography derived from drone imagery revealed that the Atacama streaks are rougher than their surroundings at the decimeter-scale roughness of the entire hillslope they occur on. A similar distinction was previously established between Martian slope streaks and their surrounding slopes, although on Mars these roughness variations appear to occur at lower-range sub-centimeter scales. Our study of the unique Atacama slope streaks may support “wet” formation as a viable hypothesis for some of the Martian slope streaks.Fil: Mushkin, Amit.
