    A Fuzzy-Based Multimedia Content Retrieval Method Using Mood Tags and Their Synonyms in Social Networks

    The preferences of Web information purchasers are rapidly evolving. Cost-effectiveness is now becoming less regarded than cost-satisfaction, which emphasizes the purchaser’s psychological satisfaction. One method to improve a user’s cost-satisfaction in multimedia content retrieval is to utilize the mood inherent in multimedia items. An example of applications using this method is SNS (Social Network Services), which is based on folksonomy, but its applications encounter problems due to synonyms. In order to solve the problem of synonyms in our previous study, the mood of multimedia content is represented with arousal and valence (AV) in Thayer’s two-dimensional model as its internal tag. Although some problems of synonyms could now be solved, the retrieval performance of the previous study was less than that of a keyword-based method. In this paper, a new method that can solve the synonym problem is proposed, while simultaneously maintaining the same performance as the keyword-based approach. In the proposed method, a mood of multimedia content is represented with a fuzzy set of 12 moods of the Thayer model. For the analysis, the proposed method is compared with two methods, one based on AV value and the other based on keyword. The analysis results demonstrate that the proposed method is superior to the two methods

    Multidirectional Instability Accompanying an Inferior Labral Cyst

    Paralabral cyst of the shoulder joint can be observed in 2% to 4% of the general population, particularly in men during the third and fourth decade. On average, these cysts measure 10 mm to 20 mm in diameter and are located preferentially on the postero-superior aspect of the glenoid. The MRI has increased the frequency of the diagnosis of paralabral cysts of the shoulder joint. Paralabral cysts of the shoulder joint usually develop in the proximity of the labrum. The relationship between shoulder instability and labral tears is well known, however, the association of shoulder instability with a paralabral cyst is rare. Shoulder instability may cause labral injury or labral injury may cause shoulder instability, and then injured tear develops paralabral cyst.     We report observation of lasing in the scarred modes in an asymmetrically deformed microcavity made of liquid jet. The observed scarred modes correspond to morphology-dependent resonance of radial mode order 3 with their Q values in the range of 10^6.     The development of a web-based app employing machine learning for delirium prevention in long-term care facilities in South Korea

    Background Long-term care facilities (LCFs) in South Korea have limited knowledge of and capability to care for patients with delirium. They also often lack an electronic medical record system. These barriers hinder systematic approaches to delirium monitoring and intervention. Therefore, this study aims to develop a web-based app for delirium prevention in LCFs and analyse its feasibility and usability. Methods The app was developed based on the validity of the AI prediction model algorithm. A total of 173 participants were selected from LCFs to participate in a study to determine the predictive risk factors for delerium. The app was developed in five phases: (1) the identification of risk factors and preventive intervention strategies from a review of evidence-based literature, (2) the iterative design of the app and components of delirium prevention, (3) the development of a delirium prediction algorithm and cloud platform, (4) a pilot test and validation conducted with 33 patients living in a LCF, and (5) an evaluation of the usability and feasibility of the app, completed by nurses (Main users). Results A web-based app was developed to predict high risk of delirium and apply preventive interventions accordingly. Moreover, its validity, usability, and feasibility were confirmed after app development. By employing machine learning, the app can predict the degree of delirium risk and issue a warning alarm. Therefore, it can be used to support clinical decision-making, help initiate the assessment of delirium, and assist in applying preventive interventions. Conclusions This web-based app is evidence-based and can be easily mobilised to support care for patients with delirium in LCFs. This app can improve the recognition of delirium and predict the degree of delirium risk, thereby helping develop initiatives for delirium prevention and providing interventions. Moreover, this app can be extended to predict various risk factors of LCF and apply preventive interventions. Its use can ultimately improve patient safety and quality of care. © 2022, The Author(s).1

    Lung cancer with superior vena cava syndrome diagnosed by intravascular biopsy using EBUS-TBNA

    AbstractSince superior vena cava syndrome (SVCS) is a critical condition, immediate diagnostic approach and therapy are imperative to avoid potentially life-threatening complications. Here, we report a case of lung cancer with SVCS, which was diagnosed through intravascular tumor biopsy using endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). EBUS-TBNA enabled us to obtain tissue sufficient for diagnosis, without significant complications. Prompt diagnosis was followed by appropriate anticancer treatment and improvement in the symptoms. For patients suspected of SVCS and requiring prompt pathologic diagnosis, we can consider EBUS-TBNA to diagnose intravascular or mediastinal tumors and provide an accurate diagnosis

    Long-Term Glycaemic Durability of Early Combination Therapy Strategy versus Metformin Monotherapy in Korean Patients with Newly Diagnosed Type 2 Diabetes Mellitus

    We assessed the glycaemic durability with early combination (EC; vildagliptin+metformin [MET], n=22) versus MET monotherapy (n=17), among newly-diagnosed type 2 diabetes mellitus (T2DM) enrolled (between 2012 and 2014) in the VERIFY study from Korea (n=39). Primary endpoint was time to initial treatment failure (TF) (glycosylated hemoglobin [HbA1c]>= 7.0% at two consecutive scheduled visits after randomization [end of period 1]). Time to second TF was assessed when both groups were receiving and failing on the combination (end of period 2). With EC the risk of initial TF significantly reduced by 78% compared to MET (n=3 [15%] vs. n=10 [58.7%], P=0.0228). No secondary TF occurred in EC group versus five patients (29.4%) in MET. Patients receiving EC treatment achieved consistently lower HbA1c levels. Both treatment approaches were well tolerated with no hypoglycaemic events. In Korean patients with newly diagnosed T2DM, EC treatment significantly and consistently improved the long-term glycaemic durability as compared with MET.Peer reviewe

    As part of the Japan/East Sea (JES) initiative supported by the U.S. Office of Naval Research, we conducted an observational experiment to understand the physics of the mesoscale circulation in the Ulleung Basin, located in the southwestern corner of the JES. The current passing through the Korea Strait divides upon entering the JES, with portions of the current flowing along the Korean and Japanese coasts. The variability of these currents is especially energetic in our study region. Our objectives were to measure the time-varying currents in the upper and deep levels of the JES.
