    Ni0.1Mn0.4Cu0.2Cd0.3Fe2O4 has been planned to prepare by the sol-gel auto combustion procedure. From the frequency 20 Hz to the frequency 15 MHz, dielectric properties, electric modulus, impedance spectroscopy, and ac conductivity has been documented with the help of an impedance analyzer. From the graph of the real dielectric part, it has been viewed that the dielectric constant value reduces with the boost of frequency that can be elucidated through Maxwell and Wagner's two-layer theory. A lucid peak has been found in the graph of the imaginary electric modulus part that is the clear distinction between two conduction regions in the frequency area. In the impedance spectroscopy, high impedance has been monitored in the low-frequency section. Enhance of ac conductivity in the high-frequency section reveals the activity of grains over grain boundary

    Client Satisfaction Towards Quality of Health Services: an Assessment at Primary Healthcare of District Gujranwala

    This survey designed to evaluate the satisfaction level and the factors that affect the patient satisfaction regarding health care delivery services with the aim to improve the services in the primary health care settings of Gujranwala. A Cross Sectional Study done on randomly selected patients attending the basic health units of Gujranwala, with more than18 years of age. Pretested structured "Liker scale questionnaire" was used for data collection. Out of total respondents, 62 (41.3%) clients were satisfied with the services provided by the basic health units of Gujranwala. The factors identified to determine patient satisfaction were accessibility of services, behavior of staff, health education, level of cleanliness, drug availability and miscellaneous services. Not a single ranked area of satisfaction noticed. Client\u27s occupation and income had significant relationship with the patient satisfaction level. Gender, age, and education of clients were not contributing factors; they not affect the client satisfaction level.Less than half clients were satisfied with the services provided by the basic health units. Management of health facilities needs to improve the services

    Improvement of third-order intermodulation product of RF and microwave amplifiers by injection

    This paper discusses the improvement in the third-order intermodulation product (IM3) performance obtainable from RF and microwave amplifiers by two alternative injection techniques. The first is the addition to the amplifier input of the second harmonics of the input spectrum and the second is the addition to the amplifier input of the difference frequencies between the spectral components of the input signal. Both techniques are considered in theory, by simulation and in practice. Both techniques give useful improvements in two-tone IM3 performance. The second harmonic technique reduced the IM3 level by 43 dB in an amplifier at 835 MHz. The difference-frequency technique gave a reduction of 48 dB in an amplifier at 880 MHz. The difference-frequency technique also gives a greater improvement for complex spectra signal

    Surgical ligation of scrotal varicocele for male factor infertility is a valid option of treatment

    Objective: To evaluate the role of surgical ligation of scrotal varicocele for treatment of male factor infertility. Methods: We studied 60 patients who presented with infertility and were also found to have scrotal varicocele. Patients with other probable causes contributing to infertility were excluded. Diagnosis was made by clinical examination and scrotal ultrasonography. All the patients underwent either laparoscopic varicocelectomy or open retroperitoneal high ligation of the testicular veins. Operative time and hospital stay was recorded. All patients were evaluated for postoperative improvement of semen parameters and development of any postoperative complication. The results were analyzed by commercially available software. Results: During the study period 5 patients were lost to follow up and were excluded. The rest of patients (n=55) were young, with age ranging from 20-35 years (Mean age 27.8 years, SD +/- 4.38). All patients had unilateral left sided varicocele; two patients (3.6%) had grade I varicocele, 21 patients (38.2%) had grade II varicocele and 32 patients (58.2%) had grade III varicocele. Statistically significant improvement in sperm density (p value \u3c 0.05), sperm activity (p value \u3c 0.05) and sperm morphology (p value \u3c 0.05) was observed after the surgical ligation. The mean operative time was 54.88 minutes (SD + 13). The mean hospital stay in laparoscopic procedure was 33.4 (SD + 15.3). Minor complications were noted in 13 patients and included superficial wound infection in 3 patients, 6 patients exhibited testicular pain and persistence of varicocele in 4 patients. Conclusion: Surgical ligation of scrotal varicocele is asafe and effective mode of treatment of male factor infertility in selected population

    Moyamoya disease: A clinical spectrum, literature review and case series from a tertiary care hospital in Pakistan

    <p>Abstract</p> <p>Background</p> <p>Moyamoya is a rare cerebrovascular disease of unknown etiology. The data on moyamoya disease from Pakistan is sparse. We report a case series of 13 patients who presented with moyamoya disease to a tertiary care hospital in Pakistan with a national referral base.</p> <p>Methods</p> <p>We conducted a retrospective review of thirteen patients who presented to The Aga Khan University and diagnosed with "Moyamoya Disease" during the period 1988 – 2006. These patients were identified from existing hospital database via ICD-9 codes. A predesigned questionnaire containing information about clinical presentation, management and neuroimaging was administered to all identified patients.</p> <p>Results</p> <p>There were seven males and six females. Mean age at presentation was 16.5 years and a female predominance was found in the pediatric age group (n = 10, 71.4%). Stroke (n = 11, 84.2%) was the most common presentation with motor deficit being the universal cortical symptom. Fever was a common symptom in the lower age groups (n = 4, 51.7%). Cerebral Angiography and Magnetic Resonance Angiography showed bilateral involvement of the vessels in eleven patients while unilateral in two. Subarachnoid and interventricular haemorrhage appeared in 2(15.4%) adults. Twelve (92.3%) patients were discharged as independent with minor deficits regardless of therapeutic modality. Only three (23.0%) patients underwent surgery whereas the remaining were managed conservatively.</p> <p>Conclusion</p> <p>Physicians when dealing with childhood strokes and characteristic deficits in adult population should consider Moyamoya disease.</p

    Leiomyosarcoma presenting as a spontaneously ruptured renal tumor-case report

    BACKGROUND: Ruptured renal neoplasms can be a catastrophic clinical presentation. Angiomyolipoma is the commonest renal tumor which presents in this fashion. Renal sarcomas are rare renal neoplasms. Renal leiomyosarcomas are the most common histological subtype of renal sarcomas, accounting for approximately 50–60% of the reported cases. These tumors are usually peripherally located and appear to arise from either the renal capsule or smooth muscle tissue in the renal pelvic wall. CASE PRESENTATION: A 70 years old male, with hypertension and ischemic disease, developed acute left flank pain. The general physician evaluated this using ultrasound, which showed a solid left renal mass. Two weeks later, he presented in the emergency room in a state of shock with a palpable flank mass. CT scan of the abdomen showed a large heterogeneous mass lesion in the left perinephric space with minimal post contrast enhancement. Per-operatively, large retroperitoneal hematoma was found within Gerota's fascia along with spleen plastered to the upper limit of hematoma. Nephrectomy and splenectomy were performed. Postoperative course was uneventful and patient was discharged on the 10(th )post-operative day. Histopathological evaluation of the specimen showed high-grade leiomyosarcoma CONCLUSIONS: Spontaneous rupture of renal neoplasm is a rare clinical presentation. Angiomyolipoma is the commonest cause of spontaneous rupture of the kidney. Presentation of a leimyosarcoma as a ruptured renal neoplasm has not been previously reported in the English literature

    Optimal energy management of a campus microgrid considering financial and economic analysis with demand response strategies

    An energy management system (EMS) was proposed for a campus microgrid (&micro;G) with the incorporation of renewable energy resources to reduce the operational expenses and costs. Many uncertainties have created problems for microgrids that limit the generation of photovoltaics, causing an upsurge in the energy market prices, where regulating the voltage or frequency is a challenging task among several microgrid systems, and in the present era, it is an extremely important research area. This type of difficulty may be mitigated in the distribution system by utilizing the optimal demand response (DR) planning strategy and a distributed generator (DG). The goal of this article was to present a strategy proposal for the EMS structure for a campus microgrid to reduce the operational costs while increasing the self-consumption from green DGs. For this reason, a real-time-based institutional campus was investigated here, which aimed to get all of its power from the utility grid. In the proposed scenario, solar panels and wind turbines were considered as non-dispatchable DGs, whereas a diesel generator was considered as a dispatchable DG, with the inclusion of an energy storage system (ESS) to deal with solar radiation disruptions and high utility grid running expenses. The resulting linear mathematical problem was validated and plotted in MATLAB with mixed-integer linear programming (MILP). The simulation findings demonstrated that the proposed model of the EMS reduced the grid electricity costs by 38% for the campus microgrid. The environmental effects, economic effects, and the financial comparison of installed capacity of the PV system were also investigated here, and it was discovered that installing 1000 kW and 2000 kW rooftop solar reduced the GHG generation by up to 365.34 kg CO2/day and 700.68 kg CO2/day, respectively. The significant economic and environmental advantages based on the current scenario encourage campus owners to invest in DGs and to implement the installation of energy storage systems with advanced concepts
