    Patency of endoscopic ultrasound-guided gastroenterostomy in the treatment of malignant gastric outlet obstruction

    Background and study aims Endoscopic ultrasoundguided gastroenterostomy (EUS-GE) with a lumen-apposing metal stent (LAMS) is a novel, minimally invasive technique in the palliative treatment of malignant gastric outlet obstruction (GOO). Several studies have demonstrated feasibility and safety of EUS-GE, but evidence on long-term durability is limited. The aim of this study was to evaluate patency of EUS-GE in treatment of malignant GOO. Patients and Methods An international multicenter study was performed in seven centers in four European countries. Patients who underwent EUS-GE with a LAMS between March 2015 and March 2019 for palliative treatment of symptomatic malignant GOO were included retrospectively. Our main outcome was recurrent obstruction due to LAMS dysfunction; other outcomes of interest were technical success, clinical success, adverse events (AEs), and survival. Results A total of 45 patients (mean age 69.9 ± 12.3 years and 48.9% male) were included. Median duration of followup was 59 days (interquartile range [IQR] 41–128). Recurrent obstruction occurred in two patients (6.1 %), after 33 and 283 days of follow-up. Technical success was achieved in 39 patients (86.7 %). Clinical success was achieved in 33 patients (73.3 %). AEs occurred in 12 patients (26.7 %), of which five were fatal. Median overall survival was 57 days (IQR 32–114). Conclusions EUS-GE showed a low rate of recurrent obstruction. The relatively high number of fatal AEs underscores the importance of careful implementation of EUSGE in clinical practice

    The discovery of human metapneumovirus and its implications for respiratory tract disease have emphasized the need for a sensitive, specific, and rapid assay to detect this virus in a clinical setting. It recently became clear that human metapneumovirus can be grouped into at least four genetic lineages. Previously described assays for the detection of human metapneumovirus were developed by using limited sequence information and failed to detect viruses from all four genetic lineages with comparable sensitivities. Here we describe the development and evaluation of a real-time reverse transcriptase PCR assay that detects human metapneumovirus from the four known genetic lineages with equal specificities and sensitivities

    Producción CientíficaThe switching dynamics of TiN/Ti/HfO2/W-based resistive memories is investigated. The analysis consisted in the systematic application of voltage sweeps with different ramp rates and temperatures. The obtained results give clear insight into the role played by transient and thermal effects on the device operation. Both kinetic Monte Carlo simulations and a compact modeling approach based on the Dynamic Memdiode Model are considered in this work with the aim of assessing, in terms of their respective scopes, the nature of the physical processes that characterize the formation and rupture of the filamentary conducting channel spanning the oxide film. As a result of this study, a better understanding of the different facets of the resistive switching dynamics is achieved. It is shown that the temperature and, mainly, the applied electric field, control the switching mechanism of our devices.     An evidence‐base for developing ambitious yet realistic national biodiversity targets

    Biodiversity targets are a key tool, used at a global and national policy level, to align biodiversity goals, promote conservation action, and recover nature. Yet most biodiversity targets are not met. In England, the government has committed to legally-binding targets to halt and recover the decline in species abundance by 2030 and 2042. We present evidence from recent population trends of 670 terrestrial animal species (for which abundance time series are available) as a species abundance indicator, together with a synthesis of case studies on species recovery, to assess the degree to which these targets are achievable. The case studies demonstrate that recovery is possible through a range of approaches. The indicator demonstrates that theoretically targets can be achieved by addressing severe declines in a relatively small number of species, as well as creating smaller benefits for many species through landscape-scale interventions. The fact that multiple pathways exist to achieve the species abundance targets in England presents choices but also raises the possibility that targets might be reached with perverse consequences. We demonstrate that evidence on achievability is a necessary but not sufficient condition for determining what is required to deliver conservation outcomes and restore biodiversity

    Adubos verdes e seus efeitos no rendimento da cana-de-açúcar em sistema de plantio direto.

    O objetivo deste trabalho foi avaliar adubos verdes e seus efeitos no rendimento da cana-de-açúcar em sistema de plantio direto (SPD). O trabalho foi realizado em Campos dos Goytacazes (RJ), no período de dezembro de 2003 a julho de 2005. O delineamento experimental foi em blocos casualizados, com quatro repetições. Os tratamentos foram: feijão-de-porco (Canavalia ensiformis), mucuna-preta(Mucuna aterrimum), crotalária (Crotalária juncea) em plantio direto e vegetação espontânea em preparo convencional (testemunha). Com crotalária aos 35 dias após emergência (DAE) houve maior taxa de cobertura do solo – 87% – e, aos 92 DAE produziu 17.852 kg ha-1 de matéria seca, respectivamente, 41%, 78% e 407% superior ao feijão-de-porco, mucuna e vegetação espontânea, além de superá-las em acúmulos de K, Mg, S, Zn e Fe. O feijão-de-porco e a mucuna proporcionaram o maior teor de N na parte aérea. Com feijão-de-porco, os teores de P e Ca foram maiores que a crotalária e a mucuna. Com vegetação espontânea, o maior teor de K foi na parte aérea. As leguminosas acumularam maiores quantidades de N e Cu do que a vegetação espontânea. A crotalária e o feijão-de-porco acumularam 66% a mais de P na parte área que a mucuna. O SPD utilizando a adubação verde contribuiu significativamente para a maior produtividade de cana-de-açúcar, 135.863 kg ha-1, sendo 37% superior ao PC com a vegetação espontânea

    Bacteriophage therapy reduces Staphylococcus aureus in a porcine and human ex vivo burn wound infection model

    Burn wounds are a major burden, with high mortality rates due to infections. Staphylococcus aureus is a major causative agent of burn wound infections, which can be difficult to treat because of antibiotic resistance and biofilm formation. An alternative to antibiotics is the use of bacteriophages, viruses that infect and kill bacteria. We investigated the efficacy of bacteriophage therapy for burn wound infections, in both a porcine and a newly developed human ex vivo skin model. In both models, the efficacy of a reference antibiotic treatment (fusidic acid) and bacteriophage treatment was determined for a single treatment, successive treatment, and prophylaxis. Both models showed a reduction in bacterial load after a single bacteriophage treatment. Increasing the frequency of bacteriophage treatments increased bacteriophage efficacy in the human ex vivo skin model, but not in the porcine model. In both models, prophylaxis with bacteriophages increased treatment efficacy. In all cases, bacteriophage treatment outperformed fusidic acid treatment. Both models allowed investigation of bacteriophage-bacteria dynamics in burn wounds. Overall, bacteriophage treatment outperformed antibiotic control underlining the potential of bacteriophage therapy for the treatment of burn wound infections, especially when used prophylactically
