Subsidized pharmacological treatment for smoking cessation by the Spanish public health system
INTRODUCTION Research has shown that financing drug therapy
increases smoking abstinence rates, although most of these
studies have been carried out in the private healthcare setting.
The aim of this work is to assess the effect of subsidized
pharmacological treatment on smoking cessation rates by the
Spanish public healthcare system.
METHODS A pragmatic, randomized, clinical trial was performed
by clusters. Randomization unit was the primary healthcare
center and the analysis unit was the patient. Smokers
consuming ≥10 cigarettes/day were randomly assigned to an
intervention group that received financed pharmacological
treatment or to a control group that followed usual care. The
main outcome was self-reported or CO-confirmed continuous
abstinence at 12 months. The main outcome, continuous
abstinence rates (%), were compared between groups at 12
months post-intervention. A model was adjusted using mixedeffect
logistic regression.
RESULTS A total of 1154 patients were included from 23
healthcare centers. In the intention-to-treat analysis, selfreported
abstinence after 12 months in the control and
intervention groups, respectively, was 9.6% (37/387) and
15.4% (118/767) (gender-adjusted OR=1.75; 95% CI: 1.1–
2.8); for CO-confirmed abstinence the corresponding values
were 3.1% (12/387) and 6.4% (49/767) (gender-adjusted
OR=1.72; 95% CI: 0.7–4.0). Pharmacological treatment use was
35.1% (136/387) in the control group, and 58.3% (447/767)
in the intervention group (adjusted OR=4.25; 95% CI: 1.8–9.9)

Existen intervenciones muy eficaces y eficientes para ayudar a nuestros pacientes a dejar de fumar. La estrategia que goza de mayor evidencia y consenso para atención primaria es la de las 5 aes, es decir, preguntar, aconsejar, valorar la disposición a intentar dejar de fumar, ayudar a aquellos que quieren intentarlo y hacer visitas de seguimiento.
Sin embargo, intervenimos menos de lo que deberíamos en nuestros pacientes fumadores.
Los protocolos disponibles nos proponen a veces intervenciones sobredimensionadas, sin evidencia científica ni repercusión terapéutica. Creemos que es necesario realizar intervenciones más sencillas, útiles y basadas en la evidencia que nos ayuden en el desempeño de nuestro trabajo, dejando de hacer aquellas que no aporten.
En este artículo usaremos como ejemplo una revisión crítica del servicio de Atención al Fumador de la Cartera de Servicios de la Comunidad de Madrid y propondremos una serie de alternativas que permitan una intervención más sencilla, efectiva y basada en pruebas

Communication is a human ability that allows us to relate; build our identity (individual,
groupal and collective); share norms, habits and behaviors and evolve personally and
socially from the creation, development and acquisition of new knowledge and skills.
Health communication is special because of the asymmetry between the professional
and the patient. Its objective is to cure, prevent diseases or strengthen healthy attitudes
and aptitudes. Its base (anamnesis) is very concrete. The patient sets out his reason for
consultation and the healthcare provider obtains data that guides the exploration to
obtain a diagnosis and treatment. The emergence of digital tools to communicate is
changing this relationship for their comfort and immediacy, aspects highly appreciated
by the population, and the healthcare providers cannot remain outside. Technological
innovations develop faster than adjustments in ethics and legislation. What will happen
when one of the subjects of the communication act is not human? A Chatbot is defined
as a conversational robot that integrates an expert system or artificial intelligence that
allows a conversation with a human. The development of health chatbots is still scarce,
but it will be a more and more usual tool in the coming years due to its great efficiency,
posing technological, social, ethical, communicative and health challenges. The
DejaloBot project is the first independent clinical trial conducted in Primary Health Care
to help patients quit smoking assisted by a chatbot. This has generated human-machine
communication problems displayed in this article.La comunicación es una habilidad humana que permite relacionarnos; construir nuestra
identidad (individual, grupal y colectiva); compartir normas, hábitos y conductas e ir
evolucionando personal y socialmente a partir de la creación, desarrollo y adquisición
de nuevos conocimientos y habilidades. La comunicación en salud es especial por la
asimetría entre el sanitario y el paciente. Su objetivo es curar, prevenir enfermedades o
potenciar aptitudes y actitudes saludables. Su base (anamnesis) es muy concreta. El
paciente expone su motivo de consulta y el sanitario obtiene datos que orientan la
exploración para obtener un diagnóstico y tratamiento. La aparición de herramientas
digitales para comunicarnos está modificando esta relación debido a su comodidad e
inmediatez, ambos aspectos muy apreciados por la población, y los sanitarios no
podemos permanecer ajenos. Las innovaciones tecnológicas se desarrollan a mayor
velocidad que los ajustes en materia de ética y legislación ¿Qué ocurre cuando uno de
los sujetos del acto de la comunicación no es humano? Se define chatbot como un
robot conversacional que integra un sistema experto o inteligencia artificial que permite
una conversación con un humano. El desarrollo de chatbots en salud todavía es
escaso, pero va a ser una herramienta cada vez más habitual en los próximos años por
su gran eficiencia, planteando retos tecnológicos, sociales, éticos, comunicativos y sanitarios. El proyecto Dejal@Bot es el primer ensayo clínico independiente y realizado
en Atención Primaria de Salud para ayudar a los pacientes a dejar de fumar asistidos
por un chatbot. Ello ha generado problemas de comunicación humano-máquina

Dejal@Bot: Un chatbot aplicable en el tratamiento de la deshabituación tabáquica
En el contexto del estudio “Efectividad de un bot conversacional para dejar de fumar en población adulta: ensayo clínico pragmático en Atención Primaria (Déj@lo)” (PI17/01942)que evalúa la efectividad de una intervención para ayudar a los fumadores a abandonar el tabaco a través de un bot conversacional para teléfonos inteligentes en comparación con el manejo habitual en Atención Primaria de Salud debíamos construir un chatbot sobre el que no existen aún unos estándares de calidad y nos basamos en los existentes para aplicaciones.Un bot (o chatbot) es una aplicación informática dotada de inteligencia artificial (IA) capaz de generar una conversación bidireccional entre un ser humano y una máquina (robot) a través de un interfaz conversacional o chat. En este trabajo presentamos la herramienta utilizada y los criterios que hemos seguido para su construcción y elaboración de contenidos.In the context of the study "Effectiveness of a conversational bot to quit smoking in the adult population: pragmatic clinical trial in Primary Care (Déj@lo)" (PI17/01942) that evaluates the effectiveness of an intervention to help smokers to quit tobacco through a conversational bot for smartphones compared to the usual management in primary health care we had to build a chatbot on which there are no quality standards yet and we rely on existing standards for applications. A bot (or chatbot) is a computer application with artificial intelligence (AI) capable of generating a two-way conversation between a human being and a machine (robot) through a conversational interface or chat. In this work we present the tool used and the criteria we have followed for its construction
