Personal experience in treatment of recurrent narrowing of internal carotid artery and occlusion of the external carotid artery
Wstęp. W Katedrze i Klinice Chirurgii Naczyniowej, Ogólnej i Transplantacyjnej we Wrocławiu w latach
1996-2001 wykonano 1087 zabiegów udrożnienia tętnic szyjnych.
Materiał i metody. U 403 pacjentów tętnicę zamknięto szwem prostym, u 306 zastosowano metodę wynicowania,
a u kolejnych 316 - łatę w miejscu naciętej tętnicy. Technikę plastyki Y zastosowano w 62 przypadkach.
Wyniki. Z tej grupy w czasie ponad 3-letniej obserwacji w 42 przypadkach wykazano restenozę tętnicy
szyjnej wewnętrznej łączącą się w 12 przypadkach ze znacznymi zwężeniami lub zamknięciem tętnicy szyjnej zewnętrznej. Po przeprowadzeniu endarterektomii w 30 przypadkach wszyto łatę, a w 12 przypadkach
zastosowano plastykę Y podziału tętnicy szyjnej. We wszystkich przypadkach podstawą diagnostyki
były: obraz kliniczny, badanie przedmiotowe, USG tętnic dogłowowych (badanie doplerowskie kodowane
kolorem) i angiografia sposobem Seldingera.
Wnioski. Cięcie w kształcie litery "Y" zastosowane przez autorów pozwala na jednoczesne udrożnienie początkowych
odcinków zarówno tętnicy szyjnej zewnętrznej, jak i wewnętrznej. Dodatkowym efektem może
być zabezpieczenie przed ponowną restenozą poprzez przesunięcie podziału tętnicy szyjnej powyżej miejsca
endarterektomii, przed powstawaniem hiperplastycznych zmian przerostowych.Background. At the Department of Vascular, General and Transplant Surgery, 1087 procedures for restoring
the patency of a stenosis of an internal carotid artery were carried out in the years 1996 to 2001.
Material and methods. 403 patients out of this group were operated on by the traditional method
of endarterectomy secured by a plane suture and 306 by the eversion method. The ”Y”-arterioplasty technique
was applied in 62 patients.
Results. During the 3-year follow-up period, recurrent stenosis of an internal carotid artery was observed in
42 patients and involved the external carotid artery in 12 patients. An endarterectomy was closed with
a Dacron patch in 30 patients and an arterial partition reconstruction ”Y” surgery was performed on
12 patients.
In all cases, the diagnosis was made upon a physical examination, coloured Doppler examination and
angiography according to Seldinger’s method.
Conclusions. A ”Y”-incision allowed the precise endarterectomy, both of the partition and of the changed
initial sections of the external and internal carotid arteries. The described ”Y”-arterioplasty of the partition
of a common carotid artery complies with the requirements imposed on an endarterectomy of an
internal carotid artery partition. Additionally, the shifting of a partition superiorly to the site of a restored
patency may safeguard a patient against recurrent stenosis, according to our material, caused mainly
by hypertrophic fibrous pathologic changes developing within the site of endarterectomy
Sonoelastografia szyjki macicy jako nowa metoda diagnostyczna w ocenie stanu szyjki u kobiet ciężarnych – doniesienie wstępne
Objectives: The study aimed at determining whether there exists a correlation between the cervical cohesion parameters assessed in the elastography and the length of the cervix. Material and methods: Assessment of cervical cohesion parameters with the use of real-time sonoelastography was performed on 59 patients between 28 and 39 weeks of gestation. Results: The analysis showed that there exists a statistically significant (p=0.033) correlation between the cervical length and the elasticity of the front cervical labium (strain ratio A). Correlation coefficient (r) stood at (-) 0.28. Conclusions: 1. There exists a negative correlation between the condition on the front cervical labium in elastographic imaging and the length of the cervical canal in USG imaging. 2. Elastography of the uterine cervix may be helpful in assessing the risk of premature labour or cervical insufficiency. 3. There is a need to perform a study on a larger group of patients in order to determine whether elastography may find its place among routine obstetric diagnostic methods.Cel pracy: Celem pracy było zbadanie czy istnieje korelacja między spoistością szyjki macicy ocenianej metodą elastografii a długością szyjki macicy. Materiał i metody: Badaniu poddano 59 ciężarnych kobiet pomiędzy 28. i 39. tygodniem ciąży, u których oceniono parametry konsystencji szyjki macicy przy użyciu sonoelastografii w czasie rzeczywistym (real-time sonoelastography) . Wyniki: Analiza wykazała, iż istnieje istotna statystycznie (p=0.033) korelacja między długością szyjki macicy a konsystencją przedniej wargi szyjki macicy (strain ratio A). Współczynnik korelacji (r) wyniósł (-) 0.28. Wnioski: 1. Istnieje ujemna korelacja pomiędzy stanem przedniej wargi szyjki macicy ciężarnej ocenianej w badaniu elastograficznym z długością kanału szyjki macicy w obrazie USG. 2. Badanie elastograficzne szyjki macicy może być pomocne w prognozowaniu ryzyka porodu przedwczesnego oraz niewydolności szyjki macicy. 3. Potrzebne są badania na większej grupie pacjentek aby stwierdzić czy elastografia znajdzie miejsce wśród rutynowych położniczych metod diagnostycznych
Evaluation of the Effectiveness of Modified Pectoral Nerve Blocks Type II (PECS II) for Vascular Access Port Implantation Using Cephalic Vein Venesection
The vascular access port implantation procedure can be performed using the venesection method by inserting a catheter into the cephalic vein in the region of the deltopectoral groove. This method eliminates the need for catheter tunneling. An alternative method to infiltration anaesthesia for port implantation may be a modified pectoral nerve block type II (PECS II). This study aimed to evaluate the effectiveness of modified PECS II for vascular access port implantation using cephalic vein venesection. This retrospective observational study was conducted at the 4th Military Clinical Hospital in Wroclaw, Poland. A group of 114 patients underwent the modified PECS II block and additional cutaneous infiltration anesthesia at the incision line. Pain intensity was assessed on the NRS scale measured intraoperatively at four points. The QoR-15 questionnaire was used to assess patient satisfaction during the first 24 h after surgery. The operator’s condition assessment score was used to assess surgical conditions and operator comfort. The analysis showed that the median pain intensity during vascular port implantation was 0. A statistically significant difference in pain intensity was demonstrated between the specialist’s group and the resident’s group at the second and third measurement points (p p p < 0.029). In conclusion, the present study revealed that the modified PECS II block is an effective and safe method of anesthesia for Port-A-Cath implantation
