    Evaluation of the initial level of development and physical training of students from the 7th grade

    Physical education in general and sports games in particular offer multiple means by which the physical and mental development of students is achieved. All the means, carefully selected and optimally dosed, put into practice through participative-active methods, have an increased efficiency regarding technical training, educating the motor qualities required by the game, but also important for behavioral changes, the formation of moral qualities – will necessary for any young person to his integration into society. The method of studying the specialized literature allowed us to study specialized works in the field of physical education, the observation method made it possible to observe the attitude of the students within the experiment, the statistical method helped us process the results obtained from a mathematical-statistical point of view, and the method graphical and tabular in presenting data using tables and graphs. The objective of the test method was to ascertain and highlight the evolution of previously established groups of students. The purpose of the research is to evaluate the initial level of development and physical training of students in the 7th grade

    Background and purpose. The aim of our study was to assess the value of imaging methods (echocardiography, thorax radiography, cardiac catheterization) in diagnosing and selecting the surgical approach to patients with FT. The goal of the study was to determine the sensitive and specific diagnostic value of echocardiography (ECOCG) for a correct decision of the therapeutic or surgical approach in patients with congenital diseases (CHD). The main objectives objectives of this research were: conceiving a normalized protocol of echocardiographic examination of a patient with CHD, defining the most important echocardiographic parameters for a correct decision of surgical treatment and for a postoperative supervision of the patients, determining the sensitivity and specificity of ECOCG in the diagnosis of CHD, carrying out a comparative analysis of imagistic methods in the diagnosis of CHD, creating an algorithm for ECOCG diagnosis and decision of surgical treatment in CHD. Materials and methods. The study included 81 patients with Fallot tetralogy and ventricular septal defect with severe stenosis of pulmonary artery, operated in the Cardiosurgical Department of the Cardiac Surgery Centre between 2003 and 2007. Conclusion. Echocardiography represents a modern non-invasive method in diagnosing the congenital heart malformations, which allow timely determination of diagnosis, defining therapeutic approach and favorable conditions for surgical intervention and postoperative monitoring. The main ECOCG parameters for the decision of therapy and estimation of the efficiency of surgical care were determined for these 2 groups of patients. A normalized protocol for ECOCG examination of a patient with CHD was conceived. The algorithms for ECOCG diagnosis and decision of surgical treatment in the case of ventricular septal defect, tetralogy of Fallot and pulmonary stenosis were created, allowing the estimation of haemodynamic disturbances and choice of cardiac surgery.Цель исследования: оценка значимости методов лучевой визуализации (ЭХОкардиография, радиография грудной клетки, катетеризация сердца) в диагностике и выборе хирургической тактики при ТФ. Целью исследования было определение чувствительности и специфичности эхокардиографии для диагностики и выбора тактики лечения пациентов с врожденными пороками сердца (ВПС). Задачи: разработка унифицированного протокола эхокардиографического исследования больных с ВПС, определение эхокардиографических параметров, наиболее важных для выбора тактики лечения и оценки результатов хирургического вмешательства, определение чувствительности и специфичности ЭХОКГ в диагностике ВПС, сравнительная оценка изобразительных методов исследования в диагностике ВПС, разработка диагностического алгоритма, способствующего определению показаний к хирургическому лечению этой категории больных. Материалы и методы. В исследовании были включены пациенты с тетрадой Фалло или дефектом межжелудочковой перегородки со значительным стенозом легочного ствола, прооперированные в отделении кардиохирургии Центра Хирургии Сердца за период 2003-2007, общим числом 81 пациент. Выводы. Эхокардиография представляет собой современный неинвазивный метод диагностики врожденных мальформаций сердца, который позволяет произвести своевременное установление диагноза, определяя терапевтический подход и оптимальное время для хирургического вмешательства, и постоперационное мониторизирование пациента. Определены наиболее важные эхокардиографически определяемые параметры для правильного выбора тактики лечения и оценки результатов хирургической коррекции больных с ВПС. Разработан унифицированный протокол эхокардиографического исследования больных с ВПС и диагностические алгоритмы ЭХОКГ для больных с ДМЖП, тетрадой Фалло и стенозом легочной артерии

    Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training. All the elements of an industry can be identified in these complex “places”: provision of goods and services, human resources issues, technology, marketing, regulatory and legal aspects, management...all of which pointing towards an approach that needs to be looked at as such.     Synapsis of Recombination Signal Sequences Located in cis and DNA Underwinding in V(D)J Recombination

    V(D)J recombination requires binding and synapsis of a complementary (12/23) pair of recombination signal sequences (RSSs) by the RAG1 and RAG2 proteins, aided by a high-mobility group protein, HMG1 or HMG2. Double-strand DNA cleavage within this synaptic, or paired, complex is thought to involve DNA distortion or melting near the site of cleavage. Although V(D)J recombination normally occurs between RSSs located on the same DNA molecule (in cis), all previous studies that directly assessed RSS synapsis were performed with the two DNA substrates in trans. To overcome this limitation, we have developed a facilitated circularization assay using DNA substrates of reduced length to assess synapsis of RSSs in cis. We show that a 12/23 pair of RSSs is the preferred substrate for synapsis of cis RSSs and that the efficiency of pairing is dependent upon RAG1-RAG2 stoichiometry. Synapsis in cis occurs rapidly and is kinetically favored over synapsis of RSSs located in trans. This experimental system also allowed the generation of underwound DNA substrates containing pairs of RSSs in cis. Importantly, we found that the RAG proteins cleave such substrates substantially more efficiently than relaxed substrates and that underwinding may enhance RSS synapsis as well as RAG1/2-mediated catalysis. The energy stored in such underwound substrates may be used in the generation of DNA distortion and/or protein conformational changes needed for synapsis and cleavage. We propose that this unwinding is uniquely sensed during synapsis of an appropriate 12/23 pair of RSSs

    Fluorescence Resonance Energy Transfer Analysis of Recombination Signal Sequence Configuration in the RAG1/2 Synaptic Complex▿ †

    A critical step in V(D)J recombination is the synapsis of complementary (12/23) recombination signal sequences (RSSs) by the RAG1 and RAG2 proteins to generate the paired complex (PC). Using a facilitated ligation assay and substrates that vary the helical phasing of the RSSs, we provide evidence that one particular geometric configuration of the RSSs is favored in the PC. To investigate this configuration further, we used fluorescent resonance energy transfer (FRET) to detect the synapsis of fluorescently labeled RSS oligonucleotides. FRET requires an appropriate 12/23 RSS pair, a divalent metal ion, and high-mobility-group protein HMGB1 or HMGB2. Energy transfer between the RSSs was detected with all 12/23 RSS end positions of the fluorescent probes but was not detected when probes were placed on the two ends of the same RSS. Energy transfer was confirmed to originate from the PC by using an in-gel FRET assay. The results argue against a unique planar configuration of the RSSs in the PC and are most easily accommodated by models in which synapsed 12- and 23-RSSs are bent and cross one another, with implications for the organization of the RAG proteins and the DNA substrates at the time of cleavage
