    The Productive Status of Laurentian French Liaison: Variation across Words and Grammar

    There are competing views in contemporary phonological theory about how to best represent processes that are pervasive, frequent, and phonologically motivated, yet still lexically sensitive. To what extent can – or should – a process that applies idiosyncratically to different morphemes, words, and even phrases, be represented in a way that allows it to generalize to novel forms? We examine this question by looking at prenominal liaison as it is used in contemporary Laurentian French, spoken in Canada. We present the results of an online production study that compares application of liaison in real vs. nonce nouns, and that considers the effect of nonce nouns’ phonological properties and morphosyntactic context on the process. We interpret our results as evidence that liaison behaviour is driven jointly by lexical representations and an abstract grammar, with properties of the real-word lexicon affecting liaison rates in nonce words. We further show that there is considerable variation in the population in the extent to which speakers produce liaison with real h-aspiré words, but that all speakers nonetheless share an understanding of what types of words are more vs. less likely to undergo liaison

    A feasibility study of indocyanine green fluorescence mapping for sentinel lymph node detection in cutaneous melanoma

    Objectives: Sentinel lymph node biopsy (SLNB) is standard of care for staging regional LN in AJCC stage IB-IIC melanoma; using dual localization with radiolabelled colloid and blue dye. Combining these gives optimal accuracy; drawbacks include cumulative radiation exposure for healthcare workers, coordination between disciplines and anaphylaxis. An alternative tracer agent is indocyanine green (ICG); an optical enhancer that fluoresces in the near infrared range. This prospective cohort study assesses the feasibility of using ICG as a tracer agent to detect SLN in cutaneous melanoma. Methods: Primary melanoma patients diagnosed with pT1b-pT4b tumours undergoing SLNB were recruited over a 6-month period at a tertiary referral centre. All underwent standard preoperative lymphoscintigraphy (LSG) using 20-40MBq of Tc99radiolabelled nanocolloid plus intraoperative Patent Blue dye (PBD). ICG was administered as a third tracer agent intraoperatively. Results: 62 patients (33M/29F) were recruited; median age was 61 years. Median melanoma Breslow thickness was 1.6mm. 144 specimens containing 135 SLN were excised. Concordance rate for all 3 tracer agents was 88.1%(119/135 LN); that for radioisotope/PBD was 88.2%(95%CI:82.2,93.7). There were no discordance pairs between radioisotope/PBD compared to radioisotope/PBD/ICG. Radioisotope/ICG significantly increased the sensitivity of detecting SLN to 98.5%(95%CI:94.8,99.8); p<0.00001 compared to radioisotope/PBD. Concordance rate of intraoperative ICG drainage pattern with LSG was 22.6%. Conclusion: ICG utilization showed comparable sensitivity with gold standard. Technical challenges e.g. ICG leakage into biopsy field, poor concordance with LSG limits its efficacy in melanoma SLNB. We therefore do not recommend replacing current practice with ICG alone or by using a combination with TC99

    Sequencing in management of in-transit melanoma metastasis: Diphencyprone versus isolate limb infusion

    Background: In-transit metastases (ITMs) in melanoma are associated with poor prognosis, however a significant proportion of these patients survive for extended periods without further disease progression. We routinely use locoregional treatment e.g. Diphencyprone (DPCP) and/or isolated limb infusion (ILI) as long-term palliation. This study aimed to identify correct sequencing of these therapies based on disease burden and progression. Method: Retrospective evaluation of all melanoma patients with ITMs treated with DPCP/ILI/both from 2010-2017 at our Cancer Centre was performed. Patients were initially assessed in a multidisciplinary setting and empirically prescribed DPCP for low-disease burden, ILI for high-disease burden. Patient demographics, tumour characteristics, response to therapy, ITM progression and patient outcomes were analysed. Results: 78 patients (M:F=30:48), aged 47-95years (median 74years) treated with DPCP/ILI/both (n=44/21/13) were identified. Progression-free survival (PFS) was significantly increased in patients responsive to DPCP or ILI as initial treatment. Patients who failed on DPCP and subsequently treated with ILI had a significantly increased PFS compared to DPCP alone (p=0.026,HR=0.048). This was not the case with patients who were treated with DPCP following failed ILI. All patients who failed to respond to the initial therapy progressed within 6 months. Conclusion: Our study shows that careful stratification ITM patients according to disease burden is fundamental to optimal outcomes. High-disease burden patients benefit from initial ILI; low-disease burden patients should commence on DPCP. ILI can be considered in DPCP patients who fail early. Systemic therapy should be considered when locoregional therapies fail after 12 months or after rapid relapse following ILI

    On February 16, 2018, Working Papers in Applied Linguistics and TESOL (represented by Kaylee Fernandez, Michelle Stabler-Havener, and Carol HoiYee Lo) had the great pleasure of interviewing Dr. Roy Lyster, the invited speaker for the 2018 Applied Linguistics & Language Education (APPLE) Lecture Series hosted annually by the Applied Linguistics and TESOL Program at Teachers College, Columbia University. Dr. Lyster shared his research and advice he has for current and future researchers and educators in Applied Linguistics and TESOL. Dr. Roy Lyster is Emeritus Professor of Second Language Education in the Department of Integrated Studies Education at McGill University in Montreal, Canada. His research examines content-based language teaching and the effects of instructional interventions designed to counterbalance form-focused and content-based approaches. His research interests also include professional development and collaboration among teachers for the purpose of integrated language learning and biliteracy development. He was co-recipient with colleague Leila Ranta of the 1998 Paul Pimsleur Award for Research in Foreign Language Education and was presented the Robert Roy Award by the Canadian Association of Second Language Teachers in 2017. He was co-president then president of the Canadian Association of Applied Linguistics from 2004 to 2008. He is author of a module called Content-Based Language Teaching published by Routledge in 2018, and two books: Learning and Teaching Languages Through Content published by Benjamins in 2007 and Vers une approche intégrée en immersion published by Les Éditions CEC in 2016
