    Patterns and drivers of carbohydrate budgets in ice algal assemblages from first year <scp>A</scp>rctic sea ice

    AbstractOngoing changes in sea ice distribution will have major implications for the ecology of the Arctic Ocean. First year ice (FYI) supports abundant ice‐algae communities that produce dissolved and particulate carbohydrates, including extracellular polymeric substances (EPS), which are significant carbon sources, influence ice formation and microbial survival within sea ice, and water column carbon cycling following ice melt. Key drivers of the distribution and composition of these carbohydrates are poorly characterised. Carbohydrates and chlorophyll a concentrations were linearly related in springtime bottom FYI at 36 sites in the Canadian Archipelago region, with high levels of spatial heterogeneity. Nanoeukaryote cell density and phosphate concentration were strong drivers of total and dissolved carbohydrate and uronic acid concentrations. Particulate carbohydrates were strongly related to total bacterial abundance. Dissolved carbohydrates contributed 77% of total carbohydrate: the most abundant (51%) size fraction being dissolved carbohydrates &lt; 8 kDa in size, with dissolved EPS contributing 7% to total carbohydrate. Carbohydrate fractions differed in monosaccharide profiles; dissolved components being glucose rich; particulate EPS containing more mannose, xylose, fucose and arabinose. These profiles corresponded to those of cultured sea‐ice diatoms. Microbial abundance, silicate, nitrate and phosphate concentration and ice thickness were important environmental drivers, with thicker ice containing relatively more particulate EPS, with thinner ice containing high amounts of glucose‐rich smaller‐sized carbohydrate moieties. Changes in ice characteristics will alter the relative balance of labile and refractory carbohydrates generated within bottom ice layers, with implications for food webs and carbon turnover in the warming Arctic Ocean.</jats:p

    Quantum jumps of light recording the birth and death of a photon in a cavity

    A microscopic system under continuous observation exhibits at random times sudden jumps between its states. The detection of this essential quantum feature requires a quantum non-demolition (QND) measurement repeated many times during the system evolution. Quantum jumps of trapped massive particles (electrons, ions or molecules) have been observed, which is not the case of the jumps of light quanta. Usual photodetectors absorb light and are thus unable to detect the same photon twice. They must be replaced by a transparent counter 'seeing' photons without destroying them3. Moreover, the light has to be stored over a duration much longer than the QND detection time. We have fulfilled these challenging conditions and observed photon number quantum jumps. Microwave photons are stored in a superconducting cavity for times in the second range. They are repeatedly probed by a stream of non-absorbing atoms. An atom interferometer measures the atomic dipole phase shift induced by the non-resonant cavity field, so that the final atom state reveals directly the presence of a single photon in the cavity. Sequences of hundreds of atoms highly correlated in the same state, are interrupted by sudden state-switchings. These telegraphic signals record, for the first time, the birth, life and death of individual photons. Applying a similar QND procedure to mesoscopic fields with tens of photons opens new perspectives for the exploration of the quantum to classical boundary

    "Brace Technology" Thematic Series - The Lyon approach to the conservative treatment of scoliosis

    <p>Abstract</p> <p/> <p>The Lyon Brace, or adjustable multi-shell brace, has been used for more than 60 years.</p> <p>The use and function of the Lyon Brace includes:</p> <p>- The utilization of one or two corrective plaster casts, which enables a true lengthening of the concave ligaments.</p> <p>- An oriented CAD-CAM moulding in 3D auto correction after the removal of the plaster cast.</p> <p>- A blueprint adapted to Lenke's classification.</p> <p>- A specific physiotherapy program.</p> <p>Background</p> <p>Pierre Stagnara created the Lyon Brace in 1947. The brace has the following characteristics:</p> <p>- It adjusts to allow for a child's growth of up to seven centimetres and for an increase in weight of up to seven kilograms.</p> <p>- It is 'active' in that the rigidity of the PMM (polymetacrylate of methyl) structure stimulates the user to auto-correct. The active axial auto-correction decreases the pressures of the brace on the trunk.</p> <p>- It is decompressive in that the effect of extension between the two pelvic and scapular girdles decreases the pressure on the intervertebral disc allowing for more effective pushes in the other planes.</p> <p>- It is symmetrical making it both more aesthetically pleasing and easier to build.</p> <p>- It is stable at both shoulders and pelvic girdle, facilitating the intermediate 3D corrections.</p> <p>- It is transparent. The pressure of the shells on the skin can be directly controlled so "pads" are usually not necessary.</p> <p>Brace description</p> <p>Two metal bars are fixed vertically, one anterior the other posterior and all shells are attached from the bottom to the top in this order:</p> <p>- Two pelvic shells ensure an optimal stability of the brace.</p> <p>- One lumbar shell T12-L4, which can be either independent or extending, at the abdominal chondrocostal level.</p> <p>- One thoracic shell at the level of the thoracic convexity.</p> <p>- One opposite thoracic shell used as a counter push.</p> <p>- One shoulder balance shell on the side of the thoracic convexity.</p> <p>Long term follow up results</p> <p>This is a retrospective study of 1,338 completed treatments checked a minimum of two years after weaning from the brace.</p> <p>Only 5% of the curves progressed more than 5° from the initial magnitudes. This translates to an effectiveness index of 0.95.</p> <p>A subset of 174 subjects who started treatment at Risser 0 was isolated. The global progressive angular mean curve was superimposed on the statistic general curve and the effectiveness index was calculated at 0.80.</p> <p>The Surgery rate was just 2% of the patients presenting with an initial curve below 45°.</p> <p>Conclusion</p> <p>The Lyon Brace is the historical reference of bracing AIS. To be fully effective, it requires the patient to wear a plaster cast for at least one month and receive specific physiotherapy training. Although this is a retrospective study, the results are very positive, and clearly indicate a need for a prospective study.</p

    Carbon isotope offsets between benthic foraminifer species of the genus Cibicides (Cibicidoides) in the glacial sub-Antarctic Atlantic

Epibenthic foraminifer δ 13 C measurements are valuable for reconstructing past bottom water dissolved inorganic carbon δ 13 C (δ 13 C DIC ), which are used to infer global ocean circulation patterns. Epibenthic δ 13 C, however, may also reflect the influence of 13 C-depleted phytodetritus, microhabitat changes, and/or variations in carbonate ion concentrations. Here we compare the δ 13 C of two benthic foraminifer species, Cibicides kullenbergi and Cibicides wuellerstorfi, and their morphotypes, in three sub-Antarctic Atlantic sediment cores over several glacial-interglacial transitions. These species are commonly assumed to be epibenthic, living above or directly below the sediment-water interface. While this might be consistent with the small δ 13 C offset that we observe between these species during late Pleistocene interglacial periods (Δδ 13 C = −0.19 ± 0.31‰, N = 63), it is more difficult to reconcile with the significant δ 13 C offset that is found between these species during glacial periods (Δδ 13 C = −0.76 ± 0.44‰, N = 44). We test possible scenarios by analyzing Uvigerina spp. δ 13 C and benthic foraminifer abundances: (1) C. kullenbergi δ 13 C is biased to light values either due to microhabitat shifts or phytodetritus effects and (2) C. wuellerstorfi δ 13 C is biased to heavy values, relative to long-term average conditions, for instance by recording the sporadic occurrence of less depleted deepwater δ 13 C DIC . Neither of these scenarios can be ruled out unequivocally. However, our findings emphasize that supposedly epibenthic foraminifer δ 13 C in the sub-Antarctic Atlantic may reflect several factors rather than being solely a function of bottom water δ 13 C DIC . This could have a direct bearing on the interpretation of extremely light South Atlantic δ 13 C values at the Last Glacial Maximum

    Which activities threaten independent living of elderly when becoming problematic : inspiration for meaningful service robot functionality

    Purpose: In light of the increasing elderly population and the growing demand for home care, the potential of robot support is given increasing attention. In this paper, an inventory of activities was made that threaten independent living of elderly when becoming problematic. Results will guide the further development of an existing service robot, the Care-O-bot®. Method: A systematic literature search of PubMed was performed, focused on the risk factors for institutionalization. Additionally, focus group sessions were conducted in the Netherlands, United Kingdom and France. In these focus group sessions, problematic activities threatening the independence of elderly people were discussed. Three separate target groups were included in the focus group sessions: (1) elderly persons (n = 41), (2) formal caregivers (n = 40) and (3) informal caregivers (n = 32). Results: Activities within the International Classification of Functioning domains mobility, self-care, and interpersonal interaction and relationships were found to be the most problematic. Conclusions: A distinct set of daily activities was identified that may threaten independent living, but no single activity could be selected as the main activity causing a loss of independence as it is often a combination of problematic activities that is person-specific.     Tamsulosin-induced severe hypotension during general anesthesia: a case report.

    Introduction: Tamsulosin, a selective alpha1-adrenergic receptor (alpha1-AR) antagonist, is a widely prescribed first-line agent for benign prostatic hypertrophy (BPH). Its interaction with anesthetic agents has not been described. Case Presentation: We report the case of 54-year-old Asian man undergoing elective left thyroid lobectomy. The only medication the Patient was taking was tamsulosin 0.4 mg for the past year for BPH. He developed persistent hypotension during the maintenance phase of anesthesia while receiving oxygen, nitrous oxide and 1% isoflurane. The hypotension could have been attributable to a possible interaction between inhalational anesthetic and tamsulosin. Conclusion: Vigilance for unexpected hypotension is important in surgical Patients who are treated with selective alpha1-AR blockers. If hypotension occurs, vasopressors that act directly on adrenergic receptors could be more effective

    Designing an automated clinical decision support system to match clinical practice guidelines for opioid therapy for chronic pain

    Abstract Background Opioid prescribing for chronic pain is common and controversial, but recommended clinical practices are followed inconsistently in many clinical settings. Strategies for increasing adherence to clinical practice guideline recommendations are needed to increase effectiveness and reduce negative consequences of opioid prescribing in chronic pain patients. Methods Here we describe the process and outcomes of a project to operationalize the 2003 VA/DOD Clinical Practice Guideline for Opioid Therapy for Chronic Non-Cancer Pain into a computerized decision support system (DSS) to encourage good opioid prescribing practices during primary care visits. We based the DSS on the existing ATHENA-DSS. We used an iterative process of design, testing, and revision of the DSS by a diverse team including guideline authors, medical informatics experts, clinical content experts, and end-users to convert the written clinical practice guideline into a computable algorithm to generate patient-specific recommendations for care based upon existing information in the electronic medical record (EMR), and a set of clinical tools. Results The iterative revision process identified numerous and varied problems with the initially designed system despite diverse expert participation in the design process. The process of operationalizing the guideline identified areas in which the guideline was vague, left decisions to clinical judgment, or required clarification of detail to insure safe clinical implementation. The revisions led to workable solutions to problems, defined the limits of the DSS and its utility in clinical practice, improved integration into clinical workflow, and improved the clarity and accuracy of system recommendations and tools. Conclusions Use of this iterative process led to development of a multifunctional DSS that met the approval of the clinical practice guideline authors, content experts, and clinicians involved in testing.     Continuity Culture: A Key Factor for Building Resilience and Sound Recovery Capabilities

    This article investigates the extent to which Jordanian service organizations seek to establish continuity culture through testing, training, and updating of their business continuity plans. A survey strategy was adopted in this research. Primary and secondary data were used. Semistructured interviews were conducted with five senior managers from five large Jordanian service organizations registered with the Amman Stock Exchange. The selection of organizations was made on the basis of simple random sampling. Interviews targeted the headquarters only in order to obtain a homogenous sample. Three out of five organizations could be regarded as crisis prepared and have better chances for recovery. The other two organizations exhibited characteristics of standard practice that only emphasizes the recovery aspect of business continuity management (BCM), while paying less attention to establishing resilient cultures and embedding BCM. The findings reveal that the ability to recover following major incidents can be improved by embedding BCM in the culture of the organization and by making BCM an enterprise-wide process. This is one of few meticulous studies that have been undertaken in the Middle East and the first in Jordan to investigate the extent to which service organizations focus on embedding BCM in the organizational culture

    Accuracy and repeatability of wrist joint angles in boxing using an electromagnetic tracking system

    © 2019, The Author(s). The hand-wrist region is reported as the most common injury site in boxing. Boxers are at risk due to the amount of wrist motions when impacting training equipment or their opponents, yet we know relatively little about these motions. This paper describes a new method for quantifying wrist motion in boxing using an electromagnetic tracking system. Surrogate testing procedure utilising a polyamide hand and forearm shape, and in vivo testing procedure utilising 29 elite boxers, were used to assess the accuracy and repeatability of the system. 2D kinematic analysis was used to calculate wrist angles using photogrammetry, whilst the data from the electromagnetic tracking system was processed with visual 3D software. The electromagnetic tracking system agreed with the video-based system (paired t tests) in both the surrogate ( 0.9). In the punch testing, for both repeated jab and hook shots, the electromagnetic tracking system showed good reliability (ICCs > 0.8) and substantial reliability (ICCs > 0.6) for flexion–extension and radial-ulnar deviation angles, respectively. The results indicate that wrist kinematics during punching activities can be measured using an electromagnetic tracking system
