    Irinotecan plus folinic acid/continuous 5-fluorouracil as simplified bimonthly FOLFIRI regimen for first-line therapy of metastatic colorectal cancer

    BACKGROUND: Combination therapy of irinotecan, folinic acid (FA) and 5-fluorouracil (5-FU) has been proven to be highly effective for the treatment of metastatic colorectal cancer. However, in light of safety and efficacy concerns, the best combination regimen for first-line therapy still needs to be defined. The current study reports on the bimonthly FOLFIRI protocol consisting of irinotecan with continuous FA/5-FU in five German outpatient clinics, with emphasis on the safety and efficiency, quality of life, management of delayed diarrhea, and secondary resection of regressive liver metastases. METHODS: A total of 35 patients were treated for metastatic colorectal cancer. All patients received first-line treatment according to the FOLFIRI regimen, consisting of irinotecan (180 mg/m(2)), L-FA (200 mg/m(2)) and 5-FU bolus (400 mg/m(2)) on day 1, followed by a 46-h continuous infusion 5-FU (2400 mg/m(2)). One cycle contained three fortnightly administrations. Staging was performed after 2 cycles. Dosage was reduced at any time if toxicity NCI CTC grade III/IV was observed. Chemotherapy was administered only to diarrhea-free patients. RESULTS: The FOLFIRI regimen was generally well tolerated. It was postponed for one-week in 51 of 415 applications (12.3%). Dose reduction was necessary in ten patients. Grade III/IV toxicity was rare, with diarrhea (14%), nausea/vomiting (12%), leucopenia (3%), neutropenia (9%) and mucositis (3%). The overall response rate was 31% (4 CR and 7 PR), with disease control in 74%. After primary chemotherapy, resection of liver metastases was achieved in three patients. In one patient, the CR was confirmed pathologically. Median progression-free and overall survival were seven and 17 months, respectively. CONCLUSIONS: The FOLFIRI regimen proved to be safe and efficient. Outpatient treatment was well tolerated. Since downstaging was possible, combinations of irinotecan and continuous FA/5-FU should further be investigated in neoadjuvant protocols

Der Sport überwindet die Länder-, Kultur- und Rassengrenzen und vereint die Öffentlichkeit mithilfe der Medien. Diese inszenieren die Sportarten gemäß ihrer Wirkungslogik, und verändern somit den Sport selbst. Trotz Skandalen nimmt die Bedeutung der Spiele stetig zu, sie erreichen mit der Eröffnungsfeier das weltweit größte (Fernseh-)Publikum. Diese Feier ist ein globaler sakraler Ritus. Besonders das Fernsehen lässt alle um Ritus teilhaben. Der Sport als soziales Handeln vermittelt einen Glauben an Fortschritt durch Leistung, und die Zuschauer können mithilfe der Medien auch teilnehmen. In Zeiten des Wandels wird der Ritus wichtig für die Gemeinschaft.The Olympic Games appear to be a rite of integration in a world of increasing globalization. Sport overcomes national, cultural und racial boundaries, und unites the public with the help of the media. The media (re)produce the different sports according to the functional logic of their medial presentation und therefore change the sports itself: Economy has replaced morality as the highest precept. Despite certain scandals, the Games become more und more significant. The opening ceremony reaches the largest (TV-)audience worldwide. This ceremony is a global religious rite which could have never been accomplished without the media. It is especially television that enables everyone to be a part of the rite. Sport as social action educates a belief in Progress through achievement, and the audience can participate with the help of the media. In times of constant changes, this rite becomes highly important to the community

    Blood Vessel-Targeted Therapy in Colorectal Cancer: Current Strategies and Future Perspectives

    The vasculature is a key player and regulatory component in the multicellular microenvironment of solid tumors and, consequently, a therapeutic target. In colorectal carcinoma (CRC), antiangiogenic treatment was approved almost 20 years ago, but there are still no valid predictors of response. In addition, treatment resistance has become a problem. Vascular heterogeneity and plasticity due to species-, organ-, and milieu-dependent phenotypic and functional differences of blood vascular cells reduced the hope of being able to apply a standard approach of antiangiogenic therapy to all patients. In addition, the pathological vasculature in CRC is characterized by heterogeneous perfusion, impaired barrier function, immunosuppressive endothelial cell anergy, and metabolic competition-induced microenvironmental stress. Only recently, angiocrine proteins have been identified that are specifically released from vascular cells and can regulate tumor initiation and progression in an autocrine and paracrine manner. In this review, we summarize the history and current strategies for applying antiangiogenic treatment and discuss the associated challenges and opportunities, including normalizing the tumor vasculature, modulating milieu-dependent vascular heterogeneity, and targeting functions of angiocrine proteins. These new strategies could open perspectives for future vascular-targeted and patient-tailored therapy selection in CRC

    Empirical Fading Model and Bayesian Calibration for Multipath-Enhanced Device-Free Localization

    Multipath-enhanced device-free localization (MDFL) systems determine presence and location of objects and users not necessarily equipped with localization devices. For localization, MDFL systems exploit user-induced changes in the power of all received signal components, including both line-of-sight and multipath components (MPCs). In this work, we therefore provide a statistical fading model that describes user-induced changes in received power specifically for MPCs. The model is derived and validated empirically using an extensive set of wideband and ultra-wideband measurement data. Since the localization performance of MDFL systems strongly depends on the information about the propagation paths within the wireless network, we further propose a Bayesian calibration approach that estimates the location of the reflection points of MPCs caused by single-bounce reflections. For MPCs caused by single-bounce reflections, the solution space of possible locations of reflection points is constrained to the delay ellipse, which allows the formulation of a computationally efficient one-dimensional estimation problem. Eventually, the problem is solved by sequential Bayesian estimation. The applicability of the proposed approach is demonstrated and evaluated using measurement data. Independent of the underlying measurement system, the Bayesian calibration approach is shown to robustly estimate the locations of the reflection points in different environments. Finally, the localization results of MDFL for an indoor scenario confirm the applicability of the Bayesian calibration approach

    The performance of multipath-enhanced device-free localization severely depends on the information about the propagation paths within the network. While known for the line-of-sight, the propagation paths have yet to be determined for multipath components. This work provides a novel Bayesian calibration approach for determining the propagation paths by estimating reflection points. Therefore, first a statistical fading model is presented, that describes user-induced changes in the received signal of multipath components. The model is derived and validated empirically using an extensive set of wideband and ultra-wideband measurement data. Second, the Bayesian approach is presented, which, based on the derived empirical fading model, relates measured changes in the power of a multipath component to the location of the reflection point. Exploiting the geometric properties of multipath components caused by single-bounce reflections, the solution space of possible locations of reflection points is constrained to the delay ellipse. Thus, a one-dimensional elliptic estimation problem can be formulated, which is solved using a point mass filter. The applicability of the proposed approach is demonstrated and evaluated based on measurement data. Independent of the underlying measurement system, the Bayesian calibration approach is shown to robustly estimate the locations of the reflection points in different environments.Comment: 14 pages, 9 figure
