    The impact of infectious disease specialist consultation for Staphylococcus aureus bloodstream infections: A systematic review

    Staphylococcus aureus is a common cause of severe bloodstream infection.We performed a systematic review to assess whether consultation with infectious disease specialists decreased all-cause mortality or rate of complications of S aureus bloodstream infections. The review also assessed parameters associated with the quality of management of the infection. We searched for eligible studies in PubMed, Embase, Scopus, and clinical trials.gov as well as the references of included studies. We identified 22 observational studies and 1 study protocol for a randomized trial. A meta-analysis was not performed because of the high risk of bias in the included studies. The outcomes are reported in a narrative review. Most included studies reported survival benefit, in the adjusted analysis. Recommended management strategies were carried out significantly more often among patients seen by an infectious disease specialist. Trials, such as cluster-randomized controlled trials, can more validly assess the studies at low risk of bias.© The Author 2016.     Clean Positive Operator Valued Measures

    In quantum mechanics the statistics of the outcomes of a measuring apparatus is described by a positive operator valued measure (POVM). A quantum channel transforms POVM's into POVM's, generally irreversibly, thus loosing some of the information retrieved from the measurement. This poses the problem of which POVM's are "undisturbed", namely they are not irreversibly connected to another POVM. We will call such POVM clean. In a sense, the clean POVM's would be "perfect", since they would not have any additional "extrinsical" noise. Quite unexpectedly, it turns out that such cleanness property is largely unrelated to the convex structure of POVM's, and there are clean POVM's that are not extremal and vice-versa. In this paper we solve the cleannes classification problem for number n of outcomes n<=d (d dimension of the Hilbert space), and we provide a a set of either necessary or sufficient conditions for n>d, along with an iff condition for the case of informationally complete POVM's for n=d^2.Comment: Minor changes. amsart 21 pages. Such a law would surely burden the church’s religion, even if the government paid for the objectionable medications and compensated the church for the use of its resources.     Measuring velocity of sound with nuclear resonant inelastic x-ray scattering

    Nuclear resonant inelastic x-ray scattering is used to measure the projected partial phonon density of states of materials. A relationship is derived between the low-energy part of this frequency distribution function and the sound velocity of materials. Our derivation is valid for harmonic solids with Debye-like low-frequency dynamics. This method of sound velocity determination is applied to elemental, composite, and impurity samples which are representative of a wide variety of both crystalline and noncrystalline materials. Advantages and limitations of this method are elucidated

    Spin tunneling and topological selection rules for integer spins

    We present topological interference effects for the tunneling of a single large spin, which are caused by the symmetry of a general class of magnetic anisotropies. The interference originates from spin Berry phases associated with different tunneling paths exposed to the same dynamics. Introducing a generalized path integral for coherent spin states, we evaluate transition amplitudes between ground as well as low-lying excited states. We show that these interference effects lead to topological selection rules and spin-parity effects for integer spins that agree with quantum selection rules and which thus provide a generalization of the Kramers degeneracy to integer spins.     In Utero Exposure to Environmental Tobacco Smoke Potentiates Adult Responses to Allergen in BALB/c Mice

    BACKGROUND: Fetal stress has been linked to adult atherosclerosis, obesity, and diabetes. Epidemiology studies have associated fetal exposure to maternal smoking and postnatal exposure to environmental tobacco smoke (ETS) with increased asthma risk. OBJECTIVE: We tested the hypothesis, in a mouse model of asthma, that in utero ETS exposure alters airway function and respiratory immune responses in adults. METHODS: Pregnant Balb/c mice were exposed daily to ETS or HEPA-filtered air (AIR). Offspring inhaled aerosolized ovalbumin (OVA) or saline in weeks 7–8. Regardless of whether they inhaled OVA or saline, mice were sensitized by OVA injections in weeks 11 and 13 followed by OVA aerosol challenge in weeks 14–15. At three time points, we assessed OVA-specific serum immunoglobins, bronchoalveolar lavage cells and cytokines, lung and nasal histopathology, and airway hyperresponsiveness (AHR). RESULTS: At 6 weeks, we found no significant differences between in utero ETS and AIR mice. At 10 weeks, following OVA aerosol, ETS mice displayed greater AHR than AIR mice (α = 0.05), unaccompanied by changes in histopathology, cytokine profile, or antibody levels. At 15 weeks, mice that had inhaled saline in weeks 7–8 developed airway inflammation: eosinophilia (α = 0.05), interleukin-5 (α = 0.05), and AHR (α = 0.05) were greater in ETS mice than in AIR mice. Mice that had inhaled OVA in weeks 7–8 demonstrated no airway inflammation after sensitization and challenge. CONCLUSION: In utero ETS exposure exacerbates subsequent adult responses to initial allergen exposure

    Pauli Diagonal Channels Constant on Axes

    We define and study the properties of channels which are analogous to unital qubit channels in several ways. A full treatment can be given only when the dimension d is a prime power, in which case each of the (d+1) mutually unbiased bases (MUB) defines an axis. Along each axis the channel looks like a depolarizing channel, but the degree of depolarization depends on the axis. When d is not a prime power, some of our results still hold, particularly in the case of channels with one symmetry axis. We describe the convex structure of this class of channels and the subclass of entanglement breaking channels. We find new bound entangled states for d = 3. For these channels, we show that the multiplicativity conjecture for maximal output p-norm holds for p=2. We also find channels with behavior not exhibited by unital qubit channels, including two pairs of orthogonal bases with equal output entropy in the absence of symmetry. This provides new numerical evidence for the additivity of minimal output entropy

    Spin Tunneling and Phonon-assisted Relaxation in Mn12-acetate

    We present a comprehensive theory of the magnetization relaxation in a Mn12-acetate crystal in the high-temperature regime (T>1 K), which is based on phonon-assisted spin tunneling induced by quartic magnetic anisotropy and weak transverse magnetic fields. The overall relaxation rate as function of the longitudinal magnetic field is calculated and shown to agree well with experimental data including all resonance peaks measured so far. The Lorentzian shape of the resonances, which we obtain via a generalized master equation that includes spin tunneling, is also in good agreement with recent data. We derive a general formula for the tunnel splitting energy of these resonances. We show that fourth-order diagonal terms in the Hamiltonian lead to satellite peaks. A derivation of the effective linewidth of a resonance peak is given and shown to agree well with experimental data. In addition, previously unknown spin-phonon coupling constants are calculated explicitly. The values obtained for these constants and for the sound velocity are also in good agreement with recent data. We show that the spin relaxation in Mn12-acetate takes place via several transition paths of comparable weight. These transition paths are expressed in terms of intermediate relaxation times, which are calculated and which can be tested experimentally.Comment: 18 pages, 22 EPS figures, REVTe

    Human oxygen sensing may have origins in prokaryotic elongation factor Tu prolyl-hydroxylation

    Significance The Fe(II)- and 2-oxoglutarate (2OG)-dependent hypoxia-inducible transcription factor prolyl-hydroxylases play a central role in human oxygen sensing and are related to other prolyl-hydroxylases involved in eukaryotic collagen biosynthesis and ribosomal modification. The finding that a PHD-related prolyl-hydroxylase in Pseudomonas spp. regulates pyocyanin biosynthesis supports prokaryotic origins for the eukaryotic prolyl-hydroxylases. The identification of the switch I loop of elongation factor Tu (EF-Tu) as a Pseudomonas prolyl-hydroxylase domain containing protein (PPHD) substrate provides evidence of roles for 2OG oxygenases in both translational and transcriptional regulation. A structure of the PPHD:EF-Tu complex, the first to the authors' knowledge of a 2OG oxygenase with its intact protein substrate, reveals that major conformational changes occur in both PPHD and EF-Tu and will be useful in the design of new prolyl-hydroxylase inhibitors. </jats:p
