    Factors influencing variation in implementation outcomes of the redesigned community health fund in the Dodoma region of Tanzania: a mixed-methods study

    INTRODUCTION: Micro-health insurance (MHI) has been identified as a possible interim solution to foster progress towards Universal Health Coverage (UHC) in low- and middle- income countries (LMICs). Still, MHI schemes suffer from chronically low penetration rates, especially in sub-Saharan Africa. Initiatives to promote and sustain enrolment have yielded limited effect, yet little effort has been channelled towards understanding how such initiatives are implemented. We aimed to fill this gap in knowledge by examining heterogeneity in implementation outcomes and their moderating factors within the context of the Redesigned Community Health Fund in the Dodoma region in Tanzania. METHODS: We adopted a mixed-methods design to examine implementation outcomes, defined as adoption and fidelity of implementation (FOI) as well as their moderating factors. A survey questionnaire collected individual level data and a document review checklist and in-depth interview guide collected district level data. We relied on descriptive statistics, a chi square test and thematic analysis to analyse our data. RESULTS: A review of district level data revealed high adoption (78%) and FOI (77%) supported also by qualitative interviews. In contrast, survey participants reported relatively low adoption (55%) and FOI (58%). Heterogeneity in adoption and FOI was observed across the districts and was attributed to organisational weakness or strengths, communication and facilitation strategies, resource availability (fiscal capacity, human resources and materials), reward systems, the number of stakeholders, leadership engagement, and implementer's skills. At an individual level, heterogeneity in adoption and FOI of scheme components was explained by the survey participant's level of education, occupation, years of stay in the district and duration of working in the scheme. For example, the adoption of job description was statistically associated with occupation (p = 0.001) and wworking in the scheme for more than 20 months had marginal significant association with FOI (p = 0.04). CONCLUSION: The study demonstrates that assessing the implementation processes helps to detect implementation weaknesses and therefore address such weaknesses as the interventions are implemented or rolled out to other settings. Attention to contextual and individual implementer elements should be paid in advance to adjust implementation strategies and ensure greater adoption and fidelity of implementation

    The Role of the Courts in Mitigating Election Violence in Nigeria

    Violence occurs in a slight majority of elections held in sub-Saharan Africa. Several recent studies have examined the effectiveness of various interventions on the prevalence of violent elections; however, the role of the courts has been relatively less studied. Using data on electoral violence during the 2015 and 2019 Nigerian elections, we examine under what conditions courts can moderate election-related violence. We find that successfully challenging an election outcome in the courts is related to a reduction in the lethality of violence in the next election, but only if the courts are generally perceived as trustworthy. These findings indicate that in addition to building judicial capability, improving generalized trust in judicial institutions is key to reducing electoral violence.A violĂȘncia ocorre numa ligeira maioria das eleiçÔes realizadas na África subsariana.     Evolution and structural analysis of Glossina morsitans (Diptera; Glossinidae) Tetraspanins

    Tetraspanins are important conserved integral membrane proteins expressed in many organisms. Although there is limited knowledge about the full repertoire, evolution and structural characteristics of individual members in various organisms, data obtained so far show that tetraspanins play major roles in membrane biology, visual processing, memory, olfactory signal processing, and mechanosensory antennal inputs. Thus, these proteins are potential targets for control of insect pests. Here, we report that the genome of the tsetse fly, Glossina morsitans (Diptera: Glossinidae) encodes at least seventeen tetraspanins (GmTsps), all containing the signature features found in the tetraspanin superfamily members. Whereas six of the GmTsps have been previously reported, eleven could be classified as novel because their amino acid sequences do not map to characterized tetraspanins in the available protein data bases. We present a model of the GmTsps by using GmTsp42Ed, whose presence and expression has been recently detected by transcriptomics and proteomics analyses of G. morsitans. Phylogenetically, the identified GmTsps segregate into three major clusters. Structurally, the GmTsps are largely similar to vertebrate tetraspanins. In view of the exploitation of tetraspanins by organisms for survival, these proteins could be targeted using specific antibodies, recombinant large extracellular loop (LEL) domains, small-molecule mimetics and siRNAs as potential novel and efficacious putative targets to combat African trypanosomiasis by killing the tsetse fly vector

    atToc159 is a selective transit peptide receptor for the import of nucleus-encoded chloroplast proteins

    The members of the Toc159 family of GTPases act as the primary receptors for the import of nucleus-encoded preproteins into plastids. Toc159, the most abundant member of this family in chloroplasts, is required for chloroplast biogenesis (Bauer, J., K. Chen, A. Hiltbunner, E. Wehrli, M. Eugster, D. Schnell, and F. Kessler. 2000. Nature. 403:203–207) and has been shown to covalently cross-link to bound preproteins at the chloroplast surface (Ma, Y., A. Kouranov, S. LaSala, and D.J. Schnell. 1996. J. Cell Biol. 134:1–13; Perry, S.E., and K. Keegstra. 1994. Plant Cell. 6:93–105). These reports led to the hypothesis that Toc159 functions as a selective import receptor for preproteins that are required for chloroplast development. In this report, we provide evidence that Toc159 is required for the import of several highly expressed photosynthetic preproteins in vivo. Furthermore, we demonstrate that the cytoplasmic and recombinant forms of soluble Toc159 bind directly and selectively to the transit peptides of these representative photosynthetic preproteins, but not representative constitutively expressed plastid preproteins. These data support the function of Toc159 as a selective import receptor for the targeting of a set of preproteins required for chloroplast biogenesis

    Distortion product otoacoustic emissions as a health surveillance technique for hearing screening in workers in the steel manufacturing industry

    BACKGROUND : Distortion product otoacoustic emissions (DPOAEs) are a promising screening technique for the early detection of subtle noise-induced cochlear function changes. OBJECTIVES : To determine the applicability of DPOAEs as a health surveillance technique for the early detection of noise-induced hearing loss (NIHL) in workers at a steel manufacturing industry. METHODS : DPOAE measurements were recorded in 20 participants with no history of occupational noise exposure and 20 participants exposed to noise in the steel manufacturing industry. Participants were not exposed to noise for at least 48 hours prior to testing. All participants were male, with normal audiometric thresholds of ≀15 dB HL. The DPOAE presence and response levels for different frequencies were compared between the two groups. The study further evaluated the shortterm test-retest repeatability of DPOAE measurements. RESULTS : The noise-exposed group had significantly lower DPOAE response amplitudes than the control group for all the tested frequencies: p<0.001 at 2002 to 4004 Hz; p=0.01 and p=0.001 at 6348 and 7996 Hz, respectively, suggesting early outer hair cell damage in the noise-exposed group. DPOAEs showed good reproducibility. CONCLUSION : DPOAEs appear to be a sensitive technique for detecting noise-induced subtle cochlear function changes.     Chemical profiling of the street cocktail drug ‘Nyaope’ in South Africa using GC-MS II: Stability studies of the cannabinoid, opiate and antiretroviral components during sample storage

    Nyaope is a mixture of low grade heroin, cannabis products, antiretroviral drugs and other materials added as bulking agents. It is a highly physically additive mixture which is smoked by users. As part of the development of a method for the analysis and profiling of nyaope this study evaluates the stability of the cannabinoid, opiate and antiretroviral components of nyaope during storage following seizure. Conditions used were those typically used for storage of drug seizures: in a desiccator in a refrigerator, in a desiccator in the dark at room temperature, in a desiccator in daylight at room temperature and ambient room temperature in the dark in a cabinet used for storage of drug seizures. Street samples of cannabis (Δ9-tetrahydrocannabinol) and heroin were mixed with efavirenz and nevirapine tablets to mimic a nyaope sample. The samples were homogenized and transferred into glass bottles and extracted with tertiary butyl alcohol (tBuOH) and analysed by gas chromatography – mass spectrometry (GC-MS) after the powdered drugs had been stored for intervals of 0 and 24 hours under each storage condition. The data obtained indicates that the target drug components in nyaope samples decompose and that for comparison purposes the drug extracts should be prepared in tBuOH immediately after seizure because of the decomposition of the drug components during storage prior to extraction and analysis. The implications of this work are that law enforcement agencies dealing with nyaope and wanting to compare drug samples may need to change their practice around how the drug is handled after seizure but prior to analysis

    A validated method for the analysis and profiling of ‘nyaope’ using gas chromatography – mass spectrometry

    Nyaope, a Tswana word for a mixture or ‘mish-mash’, describes a drug cocktail consisting of heroin, cannabis, and on occasion other controlled substances and warfarin. It is highly addictive with extremely unpleasant side effects caused by withdrawal from the drug. It is a problem drug especially in townships in South Africa. However, its prevalence in neighbouring southern African states and further afield is not yet known. There is currently no validated method for the analysis and comparison of nyaope. We describe a validated method for the gas chromatography – mass spectrometry analysis of nyaope so that within-batch and between-batch comparisons of nyaope can successfully be made for the first time. The validated method managed an accuracy within the range 80–120%, the precision was less than 20% for all analytes and managed linearity with R2≄0.99. The detection limits for diamorphine, efavirenz, nevirapine and Δ9-tetrahydrocannabinol were 14.2, 18.6, 18.7 and 9.94 pg on column, respectively, and the limits of quantitation were 43.1, 56.3, 56.6 and 30.1 pg on column, respectively. The simulated and casework samples were successfully discriminated into original batches using the identified nyaope components, the unsupervised chemometric methods principal component analysis and hierarchical clustering, as well as chromatographic profiles

    Chemical profiling of the street cocktail drug ‘Nyaope’ in South Africa using GC-MS I : Stability studies of components of ‘Nyaope’ in organic solvents

    Nyaope, a street drug commonly found in South Africa, is a mixture of low grade heroin, cannabis products, antiretroviral drugs and other materials added as cutting agents. It is a highly physiologically addictive substance which is smoked by users. Little work has been published on the chemical analysis and profiling of nyaope. Sample preparation prior to chromatographic or spectrometric analysis normally involves dissolution of the sample in an organic solvent. This study determined the most suitable organic solvent in which the common components of nyaope, namely Δ9-tetrahydrocannabinol, diamorphine, caffeine, dextromethorphan, phenacetin and the antiretrovirals efavirenz and nevirapine, which have different chemical characteristics, are stable during extraction and prior to analysis of nyaope samples i.e. autosampler stability. Street samples of cannabis (Δ9-tetrahydrocannabinol), heroin (diamorphine) and antiretrovirals were mixed to mimic a nyaope sample and dissolved in the organic solvents dichloromethane, ethanol, ethyl acetate, hexane, isopropanol and tertiary butyl alcohol. Analysis was performed after intervals of 0, 1, 6, 8, 24, 48 and 72 hours, prior to analysis by gas chromatography - mass spectrometry. Tertiary butyl alcohol resulted in the most stable extracts of the main nyaope components after 72 hours of storage. The analysis was also repeated on actual street samples of nyaope. These results show that tertiary butyl alcohol is a suitable solvent for sample preparation for the identification, comparison and profiling of nyaope samples
