Clinicopathological features as prognostic predictors of poor outcome in papillary thyroid carcinoma
Papillary thyroid cancer (PTC) has an indolent nature and usually excellent prognosis. Some PTC clinicopathological features may contribute to the development of aggressive metastatic disease. In this work, we want to evaluate PTC clinicopathological features that are presurgical prognostic predictors of patientsâ outcomes and find which indicators are more adequate for tailoring surgical procedures and follow-up. We studied a series of 241 PTC patients submitted to surgery. All patientsâ files and histological tumor samples were reviewed. The 8th edition AJCC/UICC (American Joint Committee on Cancer/Union for International Cancer) Controlstaging system and the 2015 American Thyroid Association risk stratification system were used. Total thyroidectomy was performed in 228 patients, lymphadenectomy in 28 patients. Gross extrathyroidal extension (ETE) was present in 10 patients and 31 tumor resection margins were incomplete. Cervical lymph node metastases (LNMs) were present in 34 patients and distant metastases at diagnosis in four patients. In multivariate analysis, male gender (OR = 15.4, p = 0.015), venous invasion (OR = 16.7, p = 0.022), and lateral compartment LNM (OR = 26.7, p = 0.004) were predictors of mortality; psammoma bodies (PBs) (OR = 4.5, p = 0.008), lymph vessel invasion (OR = 6.9, p < 0.001), and gross ETE (OR = 16.1, p = 0.001) were predictors of structural disease status; male gender (OR = 2.9, p = 0.011), lymph vessel invasion (OR = 2.8, p = 0.006), and incomplete resection margins (OR = 4.6, p < 0.001) were predictors of recurrent/persistent disease. Our study supports that the factors helping to tailor patientâs surgery are male gender, presence of PBs, gross ETE, and lateral compartment LNM. Together with pathological factors, lymph vessel invasion, venous invasion, necrosis, and incomplete surgical resection, should be taken into consideration regarding treatment and follow-up of patients.This study was supported by FCT, the Portuguese Foundation for Science and Technology through a PhD grant to E.T. Management of imatinib-resistant CML patients
Imatinib has had marked impact on outcomes in chronic myelogenous leukemia (CML) patients for all stages of the disease and is endorsed by international treatment guidelines as the first line option. Although imatinib is highly effective and well tolerated, the development of resistance represents a clinical challenge. Since the most frequently identified mechanism of acquired imatinib resistance is bcr-abl kinase domain point mutations, periodic hematologic, cytogenetic, and molecular monitoring is critical throughout imatinib therapy. Once cytogenetic remission is achieved, residual disease can be monitored by bcr-abl transcript levels as assayed by reverse transcription polymerase chain reaction (RT-PCR). Detection of bcr-abl mutants prior to and during imatinib therapy can aid in risk stratification as well as in determining therapeutic strategies. Thus, mutation screening is indicated in patients lacking or losing hematologic response. Moreover, search for mutations should also be performed when a 3-log reduction of bcr-abl transcripts is not achieved or there is a reproducible increase of transcript levels. In patients harboring mutations which confer imatinib resistance, novel second line tyrosine kinase inhibitors have demonstrated encouraging efficacy with low toxicity. Only the T315I bcr-abl mutant has proved totally resistant to all clinically available bcr-abl inhibitors. Strategies to further increase the rates of complete molecular remissions represent the next frontier in the targeted therapy of CML patients
Adaptive introgression underlies polymorphic seasonal camouflage in snowshoe hares
Snowshoe hares (Lepus americanus) maintain seasonal camouflage by molting to a white winter coat, but some hares remain brown during the winter in regions with low snow cover. We show that cis-regulatory variation controlling seasonal expression of the Agouti gene underlies this adaptive winter camouflage polymorphism. Genetic variation at Agouti clustered by winter coat color across multiple hare and jackrabbit species, revealing a history of recurrent interspecific gene flow. Brown winter coats in snowshoe hares likely originated from an introgressed black-tailed jackrabbit allele that has swept to high frequency in mild winter environments. These discoveries show that introgression of genetic variants that underlie key ecological traits can seed past and ongoing adaptation to rapidly changing environments. A data envelopment approach to support the bid/no-bid decision of smallholder farmers on public calls participation
Institutional markets are one of the main sources of income for smallholder farmers in Brazil. Among these markets, the National School Meal Program (PNAE) offers to the farmers the opportunity to supply food for public schools. There may exist distinct PNAE public calls for each school. The participation of the smallholder farmers in these public calls may be limited by their scarce sources. Thus, it became necessary to create a tool to support their decision whether they should or not take part in the completion of attending a public call. The objective of this paper is to propose a tool for priority setting decision. Data Envelopment Analysis (DEA) is applied to rank the public calls (where the public calls are incorporated into the model as Decision-Making Units - DMUs) using the relative efficiency as a ranking criterion, also the methodology proposed to evaluate the bid/no-bid decisions using DEA, applying the Composite Index (CI) tie-breaking method to all DMUs in the context of institutional markets for smallholder farmers, considers all efficient and inefficient DMU as a choice if profitable, which makes it also different from what was done in the literature. The final result shows a priority attending setting to the smallholder farmers, according to the efficiency rank. An empirical application for a group of smallholder farmers in the Brazilian State of GoiĂĄs is presented. The main contribution is helping smallholder farmers to make more grounded decisions and the application of DEA model in conjunction with tie-breaking technique of the composite index (Leta et al., 2005) for a bid/no-bid supporting-decision tool in a new context (institutional markets for smallholder farmers) and considering the inefficient DMUs if profitable
Cultivares de batata para sistemas orgùnicos de produção.
InformaçÔes a respeito de cultivares adaptadas ao sistema de cultivo orgĂąnico sĂŁo escassas. O objetivo deste estudo foi avaliar, sob sistema de cultivo orgĂąnico, genĂłtipos nacionais e estrangeiros desenvolvidos para o cultivo convencional, quanto ao potencial produtivo, em condiçÔes de campo. O experimento foi conduzido em 2008, no PĂłlo APTA Leste Paulista, em Monte Alegre do Sul-SP. O delineamento experimental foi em blocos ao acaso, com 18 tratamentos e quatro repetiçÔes. Cada parcela foi constituĂda por 80 batatas-semente, dispostas em quatro linhas de 5 m de comprimento, espaçadas de 80 cm, com 25 cm entre tubĂ©rculos. Os genĂłtipos avaliados foram Agata, Asterix, Caesar, Cupido, Ăden, Melody, Novella e Vivaldi, de origem estrangeira; e ApuĂŁ, Aracy, Catucha, IAC Aracy Ruiva, ItararĂ©, Monte Alegre 172, IAC 6090, APTA 16.5, APTA 15.20 e APTA 21.54, nacionais. Foram avaliadas as caracterĂsticas de produtividade total e comercial de tubĂ©rculos, massa mĂ©dia total e comercial de tubĂ©rculos, teor de matĂ©ria seca e severidade da pinta-preta (Alternaria solani). Os clones APTA 16.5, APTA 21.54 e IAC 6090, e as cultivares Cupido, ApuĂŁ, ItararĂ© e Monte Alegre 172 foram os mais produtivos. âAPTA 21.54â superou os demais em relação a produtividade comercial (18,07 t ha-1), sendo que âAPTA 16.5â, âCupidoâ, âIAC 6090â e âItararĂ©â formaram o segundo grupo. As maiores massas mĂ©dias de tubĂ©rculos foram apresentadas pelas cultivares ItararĂ© e Cupido. O clone IAC 6090 e as cultivares Aracy e Aracy Ruiva foram as que apresentaram maiores teores de matĂ©ria seca, com valor mĂ©dio de 22,91%. âAPTA 16.5â, âApuĂŁâ, âAracyâ, âAracy Ruivaâ, âĂdenâ, âIbituaçĂșâ e âMonte Alegre 172â apresentaram alto nĂvel de resistĂȘncia Ă pinta-preta. As cultivares ItararĂ©, ApuĂŁ e Cupido sĂŁo adaptadas ao cultivo orgĂąnico, e os clones avançados APTA 16.5, APTA 21.54 e IAC 6090 apresentam potencial de cultivo no sistema orgĂąnico
Body composition and body fat distribution are related to cardiac autonomic control in non-alcoholic fatty liver disease patients
BACKGROUND/OBJECTIVES: Heart rate recovery (HRR), a cardiac autonomic control marker, was shown to be related to body composition (BC), yet this was not tested in non-alcoholic fatty liver disease (NAFLD) patients. The aim of this study was to determine if, and to what extent, markers of BC and body fat (BF) distribution are related to cardiac autonomic control in NAFLD patients. SUBJECTS/METHODS: BC was assessed with dual-energy X-ray absorptiometry in 28 NAFLD patients (19 men, 51±13 years, and 9 women, 47±13 years). BF depots ratios were calculated to assess BF distribution. Subjectsâ HRR was recorded 1 (HRR1) and 2 min (HRR2) immediately after a maximum graded exercise test.
RESULTS: BC and BF distribution were related to HRR; particularly weight, trunk BF and trunk BF-to-appendicular BF ratio showed a negative relation with HRR1 (r 1â4 0.613, r 1â4 0.597 and r 1â4 0.547, respectively, Po0.01) and HRR2 (r 1â4 0.484, r 1â4 0.446, Po0.05, and r 1â4 0.590, Po0.01, respectively). Age seems to be related to both HRR1 and HRR2 except when controlled for BF distribution. The preferred model in multiple regression should include trunk BF-to-appendicular BF ratio and BF to predict HRR1 (r2 1â4 0.549; Po0.05), and trunk BF-to-appendicular BF ratio alone to predict HRR2 (r2 1â4 0.430; Po0.001).
CONCLUSIONS: BC and BF distribution were related to HRR in NAFLD patients. Trunk BF-to-appendicular BF ratio was the best independent predictor of HRR and therefore may be best related to cardiovascular increased risk, and possibly act as a mediator in age-related cardiac autonomic control variation.info:eu-repo/semantics/publishedVersio
Multiple-length-scale elastic instability mimics parametric resonance of nonlinear oscillators
Spatially confined rigid membranes reorganize their morphology in response to
the imposed constraints. A crumpled elastic sheet presents a complex pattern of
random folds focusing the deformation energy while compressing a membrane
resting on a soft foundation creates a regular pattern of sinusoidal wrinkles
with a broad distribution of energy. Here, we study the energy distribution for
highly confined membranes and show the emergence of a new morphological
instability triggered by a period-doubling bifurcation. A periodic
self-organized focalization of the deformation energy is observed provided an
up-down symmetry breaking, induced by the intrinsic nonlinearity of the
elasticity equations, occurs. The physical model, exhibiting an analogy with
parametric resonance in nonlinear oscillator, is a new theoretical toolkit to
understand the morphology of various confined systems, such as coated materials
or living tissues, e.g., wrinkled skin, internal structure of lungs, internal
elastica of an artery, brain convolutions or formation of fingerprints.
Moreover, it opens the way to new kind of microfabrication design of
multiperiodic or chaotic (aperiodic) surface topography via self-organization.Comment: Submitted for publicatio
