Olanzapine-Induced Hyperprolactinemia: Two Case Reports
Background: Hyperprolactinemia is a common consequence of treatment with antipsychotics. It is usually defined by a sustained prolactin level above the laboratory upper level of normal in conditions other than that where physiologic hyperprolactinemia is expected. Normal prolactin levels vary significantly among different laboratories and studies. Several studies indicate that olanzapine does not significantly affect serum prolactin levels in the long term, although this statement has been challenged. Aims: Our aim is to report two olanzapine-induced hyperprolactinemia cases observed in psychiatric consultations. Methods: Medical records of the patients who developed this clinical situation observed in psychiatric consultations in the Psychiatry Department of the Prof. Dr. Fernando Fonseca Hospital during the year of 2017 were analyzed, complemented with a non-systematic review of the literature. Results: The case reports consider two women who developed prolactin-related symptoms after the initiation of olanzapine. No baseline prolactinemia was obtained, and prolactin serum levels were only evaluated after prolactin-related symptoms developed: at the time of its measurement, both patients had been taking olanzapine for more than 24 weeks. Hyperprolactinemia was found to be present in Case 2, whereas Case 1 (a 49-year-old woman) had "normal" serum prolactin levels for premenopausal and prolactin levels slightly above the maximum levels for postmenopausal women. Both patients underwent similar pharmacological adjustments, which comprised switches from olanzapine to aripiprazole. After all pharmacological changes, prolactin serum levels decreased to normal range values and prolactin-related symptoms disappeared. Discussion/Conclusions: Laboratorial and literature prolactinemia values variability and discrepancies may make the management of borderline hyperprolactinemia clinical situations difficult. Baseline prolactin levels should have been obtained, as they help in the management of patients who develop neuroleptic-induced hyperprolactinemia. Prolactin-related symptoms can occur with borderline or normal standardized prolactinemia values. Olanzapine-induced hyperprolactinemia is a rare but possible event. Seroepidemiological survey of transmissible infectious diseases in a portuguese prison establishment
A cross-sectional study was conducted in 151 (71.6%) of 211 male inmates of a regional Portuguese prison in order to establish the seroprevalence for viral hepatitis (HAV, HBV, HCV), human immunodeficiency virus (HIV), syphilis and herpes simplex virus (HSV-1 and HSV-2) and to analyze some psychosocial and criminal characteristics. Mean age was 34 years. Anti-HAV was positive in 69.5% (n = 105) and in 34.4% (n = 52) for anti-HCV. One (0.7%) person had HBsAg and 29 (19.2%) had laboratory markers of past HBV infection. Non-immune inmates for HBV were 40.4% (n = 61). Syphilis was diagnosed in 6.0% (n = 9). The rate of HIV infection was 6.6% (n = 10; all HIV-1). The seropositivity of HSV-2 was 19.9% (n = 30) and of HSV-1 was 82.1% (n = 124). Alcohol dependence was reported by 26.5% (n = 40). Excluding tobacco and prescription medication, 73.5% (n = 111) reported drug use in prison. The most commonly used drugs were: cannabis (100%; n = 111) followed by heroin (56.7%; n = 63). Anti-HCV rate was noteworthy. The HIV infection rate (6.6%) in this regional prison is at least 13 to 22 times greater than in general population. As the inmate return to the community increases the risk of disease exposure for the general population, early detection and counseling is urgently needed for prisoners
Síndrome Inflamatória Multissistémica em Crianças Associada a COVID-19 num Hospital de Nível III em Portugal
Introduction: Multisystem inflammatory syndrome in children (MIS-C) is a rare and severe manifestation of coronavirus disease 2019 (COVID-19). The aim of this study was to describe the characteristics of children with MIS-C admitted to a pediatric tertiary hospital in Portugal.
Material and methods: Observational descriptive study of MIS-C patients admitted between April 2020 and April 2021. Demographic and clinical characteristics, diagnostic tests, and treatment data were collected. The diagnosis of MIS-C was based on the World Health Organization and Centers for Disease Control and Prevention criteria.
Results: We reported 45 children with MIS-C. The median age was seven years (IQR 4 - 10 years) and 60.0% were previously healthy. SARS-CoV-2 infection was confirmed in 77.8% by RT-PCR or antibody testing for SARS-CoV-2, and in 73.3%, an epidemiological link was confirmed. All the patients had a fever and organ system involvement: hematologic (100%), cardiovascular (97.8%), gastrointestinal (97.8%), mucocutaneous (86.7%), respiratory (26.7%), neurologic (15.6%), and renal (13.3%) system. Neurological (p = 0.035) and respiratory (p = 0.035) involvement were observed in patients with a more severe presentation. There was a significant difference of medians when comparing disease severity groups, namely in the values of hemoglobin (p = 0.015), lymphocytes (p = 0.030), D-dimer (p = 0.019), albumin (p < 0.001), NT-proBNP (p = 0.005), ferritin (p = 0.048), CRP (p = 0.006), procalcitonin (p = 0.005) and IL-6 (p = 0.002). From the total number of children, 93.3% received intravenous immunoglobulin, 91.1% methylprednisolone, and one patient (2.2%) received anakinra. Thirteen patients (28.8%) required intensive care and there were no deaths. Of the 21 patients evaluated, 90.4% had reduction of exercise capacity and of the 15 patients who underwent cardiac magnetic resonance, 53.3% had sequelae of cardiac injury.
Conclusion: We observed a large spectrum of disease presentation in a group of patients where most were previously healthy. A small percentage of patients (28.9%) had a severe presentation of the disease. MIS-C is a challenge in current clinical practice and its diagnosis requires a high level of clinical suspicion as the timely initiation of therapy is essential to prevent complications. pessoais ou familiares até aos 52 anos, altura em que inicia ideias delirantes de conteúdo somático e hipocondríaco em que acredita ter ficado doente com um “problema na cabeça” após um pequeno traumatismo no local de trabalho. É internada pela primeira vez dois anos depois do referido traumatismo com o diagnóstico de perturbação delirante tipo somático, tendo tido alta com o delírio enquistado e sem crítica para o estado patológico.

Pediatria do Neurodesenvolvimento. Levantamento Nacional de Recursos e Necessidades
As perturbações do neurodesenvolvimento são das patologias crónicas mais frequentes da infância e com tendência a aumentar nas sociedades modernas. Têm na grande maioria dos casos um percurso crónico e com limitação da aprendizagem necessária
para a integração na sociedade de um modo autónomo. A Sociedade de Pediatria do Neurodesenvolvimento da Sociedade
Portuguesa de Pediatria procedeu em 2008 e 2009 ao levantamento de recursos, movimento e necessidades na área assistencial
do neurodesenvolvimento no universo de 49 hospitais portugueses com Pediatria, referente a 31 de Dezembro de 2007.
Responderam 42 (85.7%) hospitais. O número total de consultas de desenvolvimento representou 10.7% das de Pediatria, e foi-
-lhe imputada uma mediana de tempo de 20 horas por semana. Dedicavam-se ao desenvolvimento 82 pediatras, mas mais de
dois terços só o fazia a tempo parcial. Outros profissionais (fisiatras,
psicólogos, terapeutas da fala, terapeutas ocupacionais, fisioterapeutas, docentes e técnicos de Serviço Social) faziam parte
das equipas do desenvolvimento, mas em menor número que os pediatras, e de igual modo só raramente a tempo completo. Aguardava por consulta de desenvolvimento uma mediana de
185 crianças, e o tempo de espera variou entre um e 18 meses(mediana de seis).
No seu conjunto os hospitais a curto prazo recrutariam 34 Pediatras para se dedicarem à área do neurodesenvolvimento,metade em regime de tempo completo. Dos outros profissionais requisitados [psicólogos (21), terapeutas da fala (20), docentes (20), terapeutas ocupacionais (14), fisioterapeutas (8) e técnicos
do Serviço Social (6)], solicitavam-nos a tempo inteiro. Concluí-se que o movimento assistencial específico desta área
no contexto global da Pediatria representa já um número significativo de consultas. Ainda assim, a resposta na área do neurodesenvolvimento revelou-se insuficiente e as equipas não funcionavam na generalidade em trabalho multidisciplinar. Contudo, os pedidos solicitados de recursos humanos médicos e não médicos e a preferência de que a dedicação ao neurodesenvolvimento fosse a tempo completo reflecte uma evolução positiva
a curto prazo, caso estes recrutamentos se venham a concretizar
Clinicopathological features as prognostic predictors of poor outcome in papillary thyroid carcinoma
Papillary thyroid cancer (PTC) has an indolent nature and usually excellent prognosis. Some PTC clinicopathological features may contribute to the development of aggressive metastatic disease. In this work, we want to evaluate PTC clinicopathological features that are presurgical prognostic predictors of patients’ outcomes and find which indicators are more adequate for tailoring surgical procedures and follow-up. We studied a series of 241 PTC patients submitted to surgery. All patients’ files and histological tumor samples were reviewed. The 8th edition AJCC/UICC (American Joint Committee on Cancer/Union for International Cancer) Controlstaging system and the 2015 American Thyroid Association risk stratification system were used. Total thyroidectomy was performed in 228 patients, lymphadenectomy in 28 patients. Gross extrathyroidal extension (ETE) was present in 10 patients and 31 tumor resection margins were incomplete. Cervical lymph node metastases (LNMs) were present in 34 patients and distant metastases at diagnosis in four patients. In multivariate analysis, male gender (OR = 15.4, p = 0.015), venous invasion (OR = 16.7, p = 0.022), and lateral compartment LNM (OR = 26.7, p = 0.004) were predictors of mortality; psammoma bodies (PBs) (OR = 4.5, p = 0.008), lymph vessel invasion (OR = 6.9, p < 0.001), and gross ETE (OR = 16.1, p = 0.001) were predictors of structural disease status; male gender (OR = 2.9, p = 0.011), lymph vessel invasion (OR = 2.8, p = 0.006), and incomplete resection margins (OR = 4.6, p < 0.001) were predictors of recurrent/persistent disease. Our study supports that the factors helping to tailor patient’s surgery are male gender, presence of PBs, gross ETE, and lateral compartment LNM. Together with pathological factors, lymph vessel invasion, venous invasion, necrosis, and incomplete surgical resection, should be taken into consideration regarding treatment and follow-up of patients.This study was supported by FCT, the Portuguese Foundation for Science and Technology through a PhD grant to E.T. Multiple-length-scale elastic instability mimics parametric resonance of nonlinear oscillators
Spatially confined rigid membranes reorganize their morphology in response to
the imposed constraints. A crumpled elastic sheet presents a complex pattern of
random folds focusing the deformation energy while compressing a membrane
resting on a soft foundation creates a regular pattern of sinusoidal wrinkles
with a broad distribution of energy. Here, we study the energy distribution for
highly confined membranes and show the emergence of a new morphological
instability triggered by a period-doubling bifurcation. A periodic
self-organized focalization of the deformation energy is observed provided an
up-down symmetry breaking, induced by the intrinsic nonlinearity of the
elasticity equations, occurs. The physical model, exhibiting an analogy with
parametric resonance in nonlinear oscillator, is a new theoretical toolkit to
understand the morphology of various confined systems, such as coated materials
or living tissues, e.g., wrinkled skin, internal structure of lungs, internal
elastica of an artery, brain convolutions or formation of fingerprints.
Moreover, it opens the way to new kind of microfabrication design of
multiperiodic or chaotic (aperiodic) surface topography via self-organization.Comment: Submitted for publicatio
Acute kidney injury, long-term renal function and mortality in patients undergoing major abdominal surgery: a cohort analysis
BACKGROUND: Acute kidney injury (AKI) is frequent during hospitalization and may contribute to adverse consequences. We aimed to evaluate long-term adverse renal function and mortality after postoperative AKI in a cohort of patients undergoing major abdominal surgery. METHODS: We performed a retrospective analysis of adult patients who underwent major non-vascular abdominal surgery between January 2010 and February 2011 at the Department of Surgery II of Hospital de Santa Maria-Centro Hospitalar Lisboa Norte, Portugal. Exclusion criteria were as follows: chronic kidney disease on renal replacement therapy, undergoing renal replacement therapy the week before surgery, death before discharge and loss to follow-up through January 2014. Patients were categorized according to the development of postoperative AKI in the first 48 h after surgery using the Kidney Disease: Improving Global Outcomes classification. AKI was defined by an increase in absolute serum creatinine (SCr) ≥0.3 mg/dL or by a percentage increase in SCr ≥50% and/or by a decrease in urine output to 6 h. Adverse renal outcomes (need for long-term dialysis and/or a 25% decrease in estimated glomerular filtration rate after hospital discharge) and mortality after discharge were evaluated. Cumulative mortality was analysed with the Kaplan-Meier method and log-rank test and outcome predictive factors with the Cox regression. Significance was set at P < 0.05. RESULTS: Of 390 selected patients, 72 (18.5%) developed postoperative AKI. The median follow-up was 38 months. Adverse renal outcomes and death after hospital discharge were more frequent among AKI patients (47.2 versus 22.0%, P < 0.0001; and 47.2 versus 20.5%, P < 0.0001, respectively). The 4 year cumulative probability of death was 44.4% for AKI patients, while it was 19.8% for patients with no AKI (log-rank test, P < 0.0001). In multivariate analysis, AKI was a risk factor for adverse renal outcomes (adjusted hazard ratio 1.6, P = 0.046) and mortality (adjusted hazard ratio 1.4, P = 0.043). CONCLUSIONS: AKI after major abdominal surgery was independently associated with the risk of long-term need for dialysis and/or renal function decline and with the risk of death after hospital discharge
