    Turkey's Energy Demand, Production and Policies

    Turkey is currently one of the fastest growing economies in the world with the largest increase in the energy demand among the OECD countries. Since the country has limited reserves for oil, natural gas and coal, this increase in energy demand is not able to be met through domestic energy production alone, which possess a threat on the country's economic growth, national security and industrial well-being and therefore various regulations and policies are being developed to reduce the reliance on foreign energy sources. In this paper, the natural energy sources of Turkey are evaluated, current efforts to fully utilize these potential sources are examined and the governmental approaches and incentives to increase domestic production are studied in a systematic manner. The study concludes that even though tremendous efforts have been made to explore and efficiently utilize the domestic resources by the government through various legislations, guidelines and taxation; the domestic resources will not seem to be sufficient in the short run and energy imports will be necessary to meet the energy demand in the next decades.     Obesity is a common health problem with serious complications. Medical and surgical methods can be used in treatment. Laparoscopic sleeve gastrectomy (LSG) is a widely used surgical procedure. LSG is a high-risk surgery because of the effects of both existing obesity and the applied laparoscopic technique. Anesthesia plan from preoperative examination to perioperative management and postoperative care should be done carefully.     Evaluation of Subclavian Central Venous Port Catheters Placed by Anesthesiologists: Single-Center Experience

    Introduction:A Central Venous Port Catheter (CVPC) is a key part of the chemotherapy and palliative care of cancer patients. CVPC is placed by surgical clinics, oncologists, and anesthesia and reanimation specialists. In our study, we aimed to examine the complications related to subclavian CVPCs inserted by anesthesiology and reanimation specialists and to share the experiences obtained.Methods:The study included 1,805 cancer patients who underwent CVPC in the anesthesiology and reanimation clinic of the University of Health Sciences Turkey, İstanbul Training and Research Hospital. The medical records of the patients were reviewed retrospectively. The data obtained were analyzed and the mean age, gender distribution, and distribution of patients according to diagnoses, count, and percentages of early and late complications were calculated.Results:The records of 1,805 patients who underwent subclavian CVPC placement with the percutaneous method were reviewed retrospectively. Early complications occurred in 6 of our patients. Of these six, 3 patients had pneumothorax, (acute atrial fibrillation attack due to intervention in 1 patient, air embolism in 1, and cerebrovascular event complications in 1), whereas vessel perforation and hematoma were not seen in any patient. Considering the late complications, local port infection, port thrombosis, and skin necrosis were observed in 28, 50, and 26 patients, respectively. However, neither port dislocation or rupture, or catheter-related blood vessel infection, or pinch-off syndrome and nor reservoir access wounds were detected in the patients.Conclusion:Our data analysis showed that the complication rate in our study was similar or better than many other studies compared with the data of other clinics in the literature

    Glofitamab in relapsed/refractory diffuse large B cell lymphoma: Real world data

    Abstract INTRODUCT ̇ION Glofitamab is a T-cell-engaging bispecific antibody connecting CD20 on B cells and CD3 on T cells. Although, most of the patients with B-cell non-Hodgkin lymphoma (BNHL) achieve complete response (CR) following firstline treatment with rituximab and chemotherapy, about 40% of patients with diffuse large B-cell lymphoma (DLBCL) is refractory or relapse (R/R). Autologous stem-cell transplantation (ASCT) can cure some of these patients but many patients cannot undergo this procedure. CAR-T therapies are a significant advance but not available in many countries like Turkey. In Phase II expansion study, the overall response rate (ORR) was 51.6% and complete remission (CR) rate was 39.4% in R/R DLBCL patients (Dickinson er al. JCO 2022).     Hypercalciuria in Children with Monosymptomatic Nocturnal Enuresis

    Aim: Nocturnal enuresis is a common problem in childhood. We aimed to evaluate the role of urinary calcium excretion in children with monosymptomatic nocturnal enuresis (MNE). Methods: This study included 60 children with MNE and 30 healthy controls. In addition to routine tests, urine density and urinary calcium levels were measured. Hypercalciuria was defined as the excess urinary calcium excretion ≥4mg/kg/day or urinary calcium/creatinine ratio ≥0.21. Results: Urinary calcium excretion was significantly higher in the enuresis group compared with the control group and the prevalence of hypercalciuria was 25% in children with enuresis. In conclusion, our study showed that hypercalciuria is an important finding of nocturnal enuresis. Conclusion: However, further studies are needed to clarify this relationship.     Measurement of temperature and displacement with NiTi actuators under certain electrical conditions

    In this study, various mechanical behaviors of a shape memory alloy, depending on different thermal and electrical conditions to be used in areas such as biomedical, aerospace and aeronautics. Temperature changes and length change rates under different electrical current values of a shape memory alloy named nickel titanium, or nitinol (NiTi) has been observed. It has been seen that a 0.3 mm diameter wire can generate a force of 25 N while the material extends its linear measurement with a rate of 3.7 mm/s. It is observed that, under diverse constant electrical current values, the displacement and temperature relations of the nickel titanium wire is varying. It has empirically been seen that the nickel titanium alloy actuators are advantageous than their alternatives in terms of the generated strength to weight ratio and shape memory alloy materials can be used as actuators in industrial and biomedical applications

    Sedation in a Patient with Prader-Willi Syndrome: A Case Report

    Prader-Willi syndrome (PWS) is a rare disorder characterized by hypotonia, growth retardation, characteristic face shape, hypogonadism, hyperphagia and related morbid obesity. Anesthesia application in these patients has increased risk of peroperative complication due the central hypotonia, abnormal teeth structure, and limited neck mobility. Therefore, clinicians should be prepared for the worst scenario before anesthesia application in patients with PWS during general or out-patient surgery. Here, in this case report, outpatient anesthesia performed in a patient with PWS for diagnostic electromyography is presented with the literature review
