The prevalence and predictors of intimate partner violence among women attending a midwife and obstetrics unit in the Western Cape
Background: Intimate partner violence (IPV) during pregnancy is a common phenomenon across the world. The present study sought to determine the prevalence and predictors of intimate partner violence among pregnant women attending a midwife and obstetrics unit in the Western Cape. Methods: A convenience sample of a hundred and fifty pregnant women (n= 150) attending antenatal appointments at the Mitchell's Plain Midwife and Obstetrics Unit (MOU) were asked to participate in this study. Consenting women participated in an interview where they were asked questions concerning interpersonal violence and other psychosocial constructs, such as history of childhood trauma, exposure to community violence, depressive symptoms and alcohol use. Frequency distributions and descriptive statistics were calculated for categorical and continuous variables. Multivariable logistic models were developed to control for socio-demographics and psychosocial constructs. The first model was based on report of any form of IPV over the previous 12 months, while the remaining three models investigated the disaggregated forms of IPV: physical abuse, sexual abuse and emotional abuse. Results: Overall, the lifetime and 12-month prevalence rate for any IPV was 45% and 32%, respectively. For 12-month IPV, 32% reported general abuse, 29% physical and 20% reported being sexually abused. After adjusting for the effects of the other variables in the model, depressive symptoms, and reporting that this pregnancy was unplanned were significantly associated with the reporting of any IPV in the past 12 months. Looking specifically at 12 months general IPV, women who had depressive symptoms were more likely to experience some form of general IPV (OR= 6.42, CI 2.51-16.41) than women not at risk. Also, women of 'coloured' race were more likely to experience general IPV than Black African respondents (OR= 1.46, 95% CI 0.64-3.34). The model exploring associations for 12-month physical IPV found women who were at risk for depression were more likely to experience physical IPV (OR= 4.42, CI 1.88-10.41) than women not at risk, while the model exploring associations for 12-month sexual IPV found that women who reported experiencing community violence were more likely to report sexual IPV than women who reported no exposure to community violence (OR= 3.85, CI 1.14-13.08). Conclusion: This is the first study, which illustrates high prevalence rates of IPV among pregnant woman at Mitchells Plain MOU. A significant association was found between 12-month IPV and unintended pregnancy. Also, woman who are at risk for depression were found to have an increased chance of experiencing both general and physical IPV. Sexual IPV was associated with high levels of exposure to community violence. Further prospective studies in different centres are needed to address generalisability and the effect of IPV on maternal and child outcomes. Greater recognition of IPV in pregnancy could contribute to improved antenatal care, as well as enhanced policy development for appropriate intervention strategies. Anti-inflammatory and anti-endotoxin properties of peptides derived from the carboxy-terminal region of a defensin from the tick Ornithodoros savignyi
Antimicrobial peptides (AMPs) are small, cationic peptides that possess a large spectrum of bioactivities, including antimicrobial, anti-inflammatory and antioxidant activities. Several AMPs are known to inhibit lipopolysaccharide (LPS)-induced inflammation in vitro and to protect animals from sepsis. In this study the cellular anti-inflammatory and anti-endotoxin activities of Os and Os-C, peptides derived from the carboxy-terminal of a tick defensin (OsDef2), were investigated. Both Os and Os-C were found to bind LPS in vitro, albeit to a lesser extent than polymyxin B and melittin, known endotoxin-binding peptides. Binding to LPS was found to reduce the bactericidal activity of Os and Os-C against Escherichia coli confirming the affinity of both peptides for LPS. At a concentration of 25 μM, the nitric oxide (NO) scavenging activity of Os was higher than glutathione (GSH), a known NO scavenger. In contrast, Os-C showed no scavenging activity. Os and Os-C inhibited LPS/IFN-γ induced NO and TNF-α production in RAW 264.7 cells in a concentration-dependent manner, with no cellular toxicity even at a concentration of 100 μM. Although inhibition of NO and TNF-α secretion was more pronounced for melittin and polymyxin B, significant cytotoxicity was observed at concentrations of 1.56 μM and 25 μM for melittin and polymyxin B, respectively. In addition, Os, Os-C and GSH protected RAW 264.7 cells from oxidative damage at concentrations as low as 25 μM. Predictors of consent to cell line creation and immortalisation in a South African schizophrenia genomics study
Background
Cell line immortalisation is a growing component of African genomics research and biobanking. However, little is known about the factors influencing consent to cell line creation and immortalisation in African research settings. We contribute to addressing this gap by exploring three questions in a sample of Xhosa participants recruited for a South African psychiatric genomics study: First, what proportion of participants consented to cell line storage? Second, what were predictors of this consent? Third, what questions were raised by participants during this consent process?
Methods
760 Xhose people with schizophrenia and 760 controls were matched to sex, age, level of education and recruitment region. We used descriptive statistics to determine the proportion of participants who consented to cell line creation and immortalization. Logistic regression methods were used to examine the predictors of consent. Reflections from study recruiters were elicited and discussed to identify key questions raised by participants about consent.
Results
Approximately 40% of participants consented to cell line storage. The recruiter who sought consent was a strong predictor of participant’s consent. Participants recruited from the South African Eastern Cape (as opposed to the Western Cape), and older participants (aged between 40 and 59 years), were more likely to consent; both these groups were more likely to hold traditional Xhosa values. Neither illness (schizophrenia vs control) nor education (primary vs secondary school) were significant predictors of consent. Key questions raised by participants included two broad themes: clarification of what cell immortalisation means, and issues around individual and community benefit.
Conclusions
These findings provide guidance on the proportion of participants likely to consent to cell line immortalisation in genomics research in Africa, and reinforce the important and influential role that study recruiters play during seeking of this consent. Our results reinforce the cultural and contextual factors underpinning consent choices, particularly around sharing and reciprocity. Finally, these results provide support for the growing literature challenging the stigmatizing perception that people with severe mental illness are overly vulnerable as a target group for heath research and specifically genomics studies
Cognition and Behaviour in Sotos Syndrome: A Systematic Review
BACKGROUND:Research investigating cognition and behaviour in Sotos syndrome has been sporadic and to date, there is no published overview of study findings. METHOD:A systematic review of all published literature (1964-2015) presenting empirical data on cognition and behaviour in Sotos syndrome. Thirty four journal articles met inclusion criteria. Within this literature, data relating to cognition and/or behaviour in 247 individuals with a diagnosis of Sotos syndrome were reported. Ten papers reported group data on cognition and/or behaviour. The remaining papers employed a case study design. RESULTS:Intelligence quotient (IQ) scores were reported in twenty five studies. Intellectual disability (IQ < 70) or borderline intellectual functioning (IQ 70-84) was present in the vast majority of individuals with Sotos syndrome. Seven studies reported performance on subscales of intelligence tests. Data from these studies indicate that verbal IQ scores are consistently higher than performance IQ scores. Fourteen papers provided data on behavioural features of individuals with Sotos syndrome. Key themes that emerged in the behavioural literature were overlap with ASD, ADHD, anxiety and high prevalence of aggression/tantrums. CONCLUSION:Although a range of studies have provided insight into cognition and behaviour in Sotos syndrome, specific profiles have not yet been fully specified. Shorter telomere length - A potential susceptibility factor for HIV-associated neurocognitive impairments in South African women [corrected].
The neuropathogenesis of the human immunodeficiency virus (HIV) may manifest as various neurocognitive impairments (NCI). HIV-positive individuals also have significantly shorter telomere length (TL) in peripheral blood mononuclear cells (PBMCs) and CD8+ T cells compared to HIV-negative individuals. Additionally, reduced TL has been found to be associated with chronic psychological stress. This study focused on the effects of HIV-infection and chronic stress associated with childhood trauma on telomere length, and investigated whether leukocyte TL (LTL), in particular, represents a risk factor for NCI. Eighty-three HIV-positive and 45 HIV-negative women were assessed for childhood trauma and were subjected to detailed neurocognitive testing. Blood from each participant was used to extract Deoxyribonucleic acid (DNA). Relative LTL were determined by performing real time quantitative PCR reactions as described by Cawthon et al. (2002). As expected, relative LTL in the HIV-positive individuals was significantly shorter than that of HIV-negative individuals (F = 51.56, p = <0.01). Notably, a significant positive correlation was evident between relative LTL and learning performance in the HIV-positive group. In addition, a significant negative correlation was observed between relative LTL and verbal fluency, but this association was only evident in HIV-positive individuals who had experienced trauma. Our results suggest that reduced LTL is associated with worse learning performance in HIV-positive individuals, indicating that TL could act as a susceptibility factor in increasing neurocognitive decline in HIV-infected individuals
Appetitive and reactive aggression are differentially associated with the STin2 genetic variant in the serotonin transporter gene
Appetitive aggression is a sub-category of instrumental aggression, characterised by the primary intrinsic enjoyment of aggressive activity. Aggression is heritable, and serotonergic and monoaminergic neurotransmitter systems have been found to contribute to the underlying molecular mechanisms. The aim of this study was to investigate the role that genetic variants in the serotonin transporter (SLC6A4) and monoamine oxidase A (MAOA) genes play in the aetiology of appetitive aggression in South African Xhosa males (n = 290). SLC6A4 5-HTTLPR, rs25531, and STin2 variants, as well as MAOA-uVNTR were investigated for their association with levels of appetitive aggression using Poisson regression analysis. The STin2 VNTR12 allele was found to be associated with increased levels of appetitive aggression (p = 0.003), but with decreased levels of reactive aggression (p = 7 × 10-5). This study is the first to investigate genetic underpinnings of appetitive aggression in a South African population, with preliminary evidence suggesting that SCL6A4 STin2 variants play a role in its aetiology, and may also be important in differentiating between appetitive and reactive aggression. Although the results require replication, they shed some preliminary light on the molecular dichotomy that may underlie the two forms of aggression.publishe
