    Uso de drogas entre crianças e adolescentes em situação de rua: o que ajuda?

    The aim of this study was to investigate factors associated to frequent and heavy drug use among street children and adolescents aged 10 to 18 years. A sample of 2,807 street children and adolescents from the 27 Brazilian state capital cities was analyzed. A World Health Organization questionnaire for non-students was adapted for use in Brazil. Data analysis was performed using logistic regression and decision tree models. Factors inversely associated with frequent and heavy drug use were: being age nine to 11 years (OR = 0.1); school attendance (OR = 0.3); daily time (one to five hours) spent on the streets (OR = 0.3 and 0.4); not sleeping on the streets (OR = 0.4); being on the streets for less than one year (OR = 0.4); maintenance of some family bonds (OR = 0.5); presence on the streets of a family member (OR = 0.6); not suffering domestic violence (OR = 0.6); being female (OR = 0.8). All of these variables were significant at the p < 0.05 level. The findings suggest that being younger, having family bonds and engagement in school are important protective factors that affect drug use among this population and should be considered in the formulation of public policies.O objetivo do estudo foi verificar fatores associados ao uso frequente e pesado de drogas entre adolescentes em situação de rua no Brasil. Estudo transversal com amostra representativa nacional de 2.807 crianças e adolescentes (10-18 anos). Foi usado um questionário da Organização Mundial da Saúde adaptado para o Brasil e análise dos dados, modelo de regressão logística. Fatores inversamente associados ao uso frequente e pesado de drogas: faixa etária entre 9-11 anos (OR = 0,1); frequentar escola (OR = 0,3); permanecer entre 1 e 5 horas na rua (OR = 0,3 e 0,4); não dormir na rua (OR = 0,4); estar na rua há menos de um ano (OR = 0,4); manter algum vínculo familiar (OR = 0,5); permanecer na rua com algum membro da família (OR = 0,6); não ter sofrido violência doméstica (OR = 0,6); gênero feminino (OR = 0,8). Todas essas variáveis apresentaram p < 0,05. Os achados sugerem que ser mais jovem, ficar menos tempo na rua e manter vínculos com escola e família são importantes fatores de proteção para essa população quanto ao uso frequente e pesado de drogas.     Uso recreacional de benzidamina como alucinógeno entre adolescentes em situação de rua no Brasil

    OBJECTIVE: To describe the recreational use of benzydamine, an anti-inflammatory drug, among street youth in Brazil. METHOD: Design: a descriptive, cross sectional survey. Setting: 93 welfare services for the street youth in 27 Brazilian capitals. Participants: 2807 street youth, 10 to 18 years old. Main outcome measures: demographic characteristics, drug use pattern (lifetime use, use in the past 30 days, frequency, and characteristics of use in the past month) and effects of benzydamine through the use of a questionnaire. RESULTS: 78 reported lifetime recreational benzydamine use (67 cases identified only in three capitals). Among the 30 respondents reporting drug use in the last month (the month preceding the survey), 66.7% (n = 20) used the drug on 4 or more days (in the month preceding the survey). The most frequently (50%) pleasure effects reported were hallucination and nonspecific sensory changes described as trips. Unwanted effects were reported by 75% of respondents, they were especially nausea and vomiting (21.4%). In the majority of the cases, drug was obtained from drugstores without a medical prescription. CONCLUSION: This study identifies the recreational use of benzydamine among street youth, mainly in the Northeast of Brazil, and also indicates the need for special controls on the dispensation of this substance.OBJETIVO: Descrever o uso recreacional de benzidamina, um medicamento antiinflamatório, entre adolescentes em situação de rua no Brasil. MÉTODO: Desenho: descritivo, transversal. Local: 93 instituições assistenciais para crianças e adolescentes em situação de rua nas 27 capitais do Brasil. Participantes: 2.807 crianças e adolescentes em situação de rua, com idade entre 10 e 18 anos. Principais medidas de interesse: características demográficas, padrão de uso (uso na vida, uso nos últimos 30 dias, frequência e características de uso no mês anterior à pesquisa) e efeitos da benzidamina usando um questionário. RESULTADOS: 78 relataram uso recreacional na vida de benzidamina (67 casos identificados somente em três capitais). Entre os 30 entrevistados que relataram uso recente da substância (no mês que precedia a investigação), 66,7% (n = 20) tomaram o medicamento em quatro dias ou mais no mês. Os efeitos desejados mais frequentemente reportados foram alucinações e alterações sensoriais inespecíficas como viagem (50%). Efeitos indesejados foram citados por 75% dos entrevistados, principalmente náuseas e vômitos (21,4%). Na maioria dos casos, o medicamento foi obtido de drogarias sem apresentação de prescrição médica.     Padrões intergeracionais de violência familiar associada ao abuso de bebidas alcoólicas: um estudo baseado em genogramas

    The objective of this study was to analyze intergenerational patterns of alcohol related violence (ARV). An intentional sample comprising 42 family members was selected according to a set of criteria, including history of ARV. A genogram based on anonymous semi-structured taped interviews was created. The Content Analysis pointed to different patterns of repetition of intergenerational ARV. The most recurrent ones were those of lineal consanguinity (father/son) and through marriage. We observed similarities over the generations of each family as regards the pattern of alcohol consumption; the type of violence; the family reaction and the family life cycle in which ARV was intensified. Our results confirm the intergenerational reproduction of ARV. In conclusion, it is important to create intervention strategies to prevent intergenerational repetition of this association of behaviors.O objetivo deste estudo foi analisar os padrões intergeracionais de violência familiar associada ao abuso de álcool (VAA). Foi composta uma amostra intencional por critérios, até a saturação teórica, com 42 familiares com histórico intergeracional de VAA, com os quais foi elaborado um genograma por meio de entrevistas semiestruturadas gravadas, individuais e anônimas. A análise de conteúdo indicou diferentes padrões de repetição intergeracional de VAA, sendo mais frequentes as recorrências em linha direta de parentesco (pai/filho) e por meio de casamento. Ao longo das gerações de cada família, foram observadas similaridades em relação ao padrão de consumo de álcool, tipo de violência, reação da família e etapas do ciclo vital familiar de intensificação da VAA. Os resultados confirmam a reprodução intergeracional de VAA.     Fluoxetine: indication of inadequate use

    OBJECTIVE: Investigate the use of fluoxetine in Santo André city, SP, by collecting and analyzing special prescriptions (SP) issued by physicians and filed in compounding pharmacies and drugstores of that city. METHODS: Special prescriptions were collected during August 2005 to July 2006 in 13 compounding pharmacies and 27 drugstores, in different regions of Santo André. Each prescription has been examined for the presence of fluoxetine, in combination or not with other active ingredients, and sex (a) patient was noted. RESULTS: We examined 39,782 SP; 16,124 of them were collected from compounding pharmacies and 23,658 from drugstores. Of these totals, 10,919 prescriptions contained fluoxetine as follows: 9,259 from the compounding pharmacies (84.8%) and only 1,660 (15.2%) from drugstores. Fluoxetine was manly prescribed for women (79.8%). In the vast majority of SP, fluoxetine was prescribed in combination with a large number of other active substances reaching more than ten others in almost half of the prescriptions. CONCLUSION: It is suggested that the large use of fluoxetine possibily aims to an aesthetic objective (to lose weight) and not as a therapeutic aim (treatment of depression). This work discusses the risk/benefit of this use which could be described as inappropriate, given the known adverse reactions of fluoxetine and its interference with the cytochrome P450 system.OBJETIVO: Investigar o uso da fluoxetina na cidade de Santo André, SP, por meio de coleta e análise das receitas especiais (RE) prescritas por médicos e arquivadas nas farmácias e drogarias daquela cidade. MÉTODOS: Foram coletadas as receitas especiais retidas durante os meses de agosto do ano de 2005 a julho de 2006, em 13 farmácias de manipulação e em 27 drogarias em diferentes regiões de Santo André. Cada receita especial foi analisada em relação à presença de fluoxetina, em associação ou não a outros princípios ativos, e o sexo do(a) paciente foi anotado. RESULTADOS: Foram analisadas 39.782 RE - 16.124 coletadas das farmácias magistrais e 23.658 das drogarias. Desses totais, 10.919 prescrições continham fluoxetina - 9.259 provenientes das farmácias magistrais (84,8%) e apenas 1.660 (15,2%) das drogarias. As prescrições de fluoxetina eram predominantemente destinadas a mulheres (79,8%). Na imensa maioria das RE, a fluoxetina foi prescrita em associação com um grande número de outras substâncias ativas, inclusive anfetaminas anoréticas, chegando a mais de dez outras em quase a metade das prescrições. Esse tipo de prescrição múltipla, principalmente para mulheres, é comparado com as prescrições de fórmulas magistrais para emagrecer, muito utilizadas no Brasil. CONCLUSÃO: Os dados obtidos sugerem que a utilização de fluoxetina teria um fim estético (perda de peso), e não um fim terapêutico (tratamento de depressão).     Impact of variation in functions and delivery on the effectiveness of behavioural and mood management interventions for smoking cessation in people with depression:Protocol for a systematic review and meta-analysis

    Tobacco is the world's leading preventable cause of disease and death. People with depression are twice as likely to smoke and are less responsive to standard tobacco treatments as compared with the general population. A Cochrane systematic review of randomised controlled trials of smoking cessation treatment for smokers with current or historical depression found that adding mood management to usual smoking treatment improved quit rates. However, the review did not examine if variation in intervention delivery or intervention functions impacted on treatment effectiveness.With the aim of providing information to develop tailored approaches to treating smoking for people with current depression, we will add-on to the Cochrane review in three ways: (1) use the Template for Intervention Description and Replication checklist to determine if variations in mood management delivery have impact on intervention effectiveness, (2) use the Taxonomy of Behaviour Change techniques for smoking cessation to examine which behaviour change functions are most effective for smoking cessation in people with current depression and (3) examine the difference in change in depression scores between intervention and control arms.We will include randomised controlled trials of smokers with current depression as identified by a previous Cochrane review and the in-progress update of this Cochrane review. We will use meta-regression to examine (1) if variations in delivery of mood management impact on smoking cessation intervention effectiveness, (2) determine which behaviour change functions are most effective for smoking cessation and (3) use meta-analysis of the difference in change in depression scores between treatment arms from baseline to follow-up to determine if offering smoking cessation treatment causes psychological harm.Ethical approval is not required for this study. We will disseminate the findings of this work at national and international conferences, and to relevant patient panels.CRD42017070741

    Smoking Cessation Quitlines in Europe: Matching Services to Callers' Characteristics

    <p>Abstract</p> <p>Background</p> <p>Telephone quitlines offer a wide range of services to callers, including advice and counsel, and information on pharmacotherapy for smoking cessation. But, little is known about what specific quitline services are offered to smokers and whether these services are appropriately matched to characteristics of smokers. This study examines how quitline services are matched to callers' level of addiction, educational level, stage-of-change with quitting, and whether they are referred by a doctor or other health professional.</p> <p>Methods</p> <p>Between February 2005 and April 2006, 3,585 callers to seven European quitlines responded to our survey. During the course of and immediately after the call, quitline counsellors collected descriptive data on callers' characteristics and the services they used. We then conducted four logistic regression analyses to examine the relationship between quitline services and the four caller characteristics.</p> <p>Results</p> <p>Forty three percent of all callers received information on pharmacotherapy - most often nicotine patches and nicotine gum - from the counsellor. As we predicted, these callers were the heavy smokers. There was a direct correlation between the length of the conversations between the counsellor and the educational level of the smoker: the lower the education of the smoker, the shorter the call. However, we found no significant association between any other type of service and the educational level of caller. We also found a correlation between the smoker's stage of quitting and the type of advice a counsellor gives. Smokers in the action stage of quitting were more likely to receive advice (in two quitlines) or counselling (in two quitlines) than those in the preparation stage, who were less likely to be referred (in three quitlines). Very few of the total number of calls (10.7%) were from referrals by health professionals. Referred callers were more likely to receive counselling, but this was found only in four of seven quitlines.</p> <p>Conclusion</p> <p>Most of the services quitlines offer to smokers favour heavy smokers and those at a more advanced stage of cessation, but not based on their educational level. Thus, we recommend that European quitlines extend and tailor their services to include less-educated smokers.</p

    Fatores associados ao uso pesado de álcool entre estudantes das capitais brasileiras

    OBJECTIVE: To evaluate the association between heavy use of alcohol among students and family, personal and social factors. METHODS: Cross-sectional study including public school students aged ten to 18 from 27 Brazilian capital cities in 2004. Data was collected using an anonymous, self-report questionnaire that was adapted from a World Health Organization instrument. A representative sample comprising 48,155 students was stratified by census tracts and clusters (schools). The associations between heavy alcohol use and the factors studied were analyzed using logistic regression at a 5% significance level. RESULTS: Of all students, 4,286 (8.9%) reported heavy alcohol use in the month prior to the interview. The logistic regression analysis showed an association between fair or poor relationship with the father (OR = 1.46) and the mother (OR = 1.61) and heavy use of alcohol. Following a religion (OR = 0.83) was inversely associated with heavy alcohol consumption. Sports practice and mother perceived as a "liberal" person had no significance in the model. However, a higher prevalence of heavy use of alcohol was seen among working students. CONCLUSIONS: Stronger family ties and religion may help preventing alcohol abuse among students.OBJETIVO: Analizar la asociación entre el uso pesado de alcohol entre estudiantes y los factores familiares, personales y sociales. MÉTODOS: Estudio transversal realizado con estudiantes de diez a 18 años de escuelas públicas de 27 capitales brasileras, en 2004. Los datos fueron colectados por medio de cuestionario anónimo, de auto-completación, adaptado de instrumento desarrollado por la Organización Mundial de la Salud. La muestra representativa, compuesta por 48.155 estudiantes, fue estratificada por sectores censitarios y por conglomerados (escuelas). Asociaciones entre el uso pesado de alcohol y los factores estudiados fueron analizadas por medio de regresión logística, considerando nivel de significancia de 5%. RESULTADOS: Del total de estudiantes, 4.286 (8,9%) hicieron uso pesado de alcohol en el mes anterior a la entrevista. El análisis por regresión logística mostró asociación entre relaciones malas o regulares con padre (OR= 1,46) y madre (OR =1,61) y uso pesado de alcohol. Seguir una religión (OR=0,83) se mostró inversamente asociado a este tipo de consumo de alcohol. La práctica de deportes y el hecho de que la madre se perciba como liberal, no mostraron significancia en el modelo. Hubo mayor prevalencia del uso pesado de alcohol entre los estudiantes que trabajaban. CONCLUSIONES: Uniones familiares más coherentes y seguir una religión pueden prevenir el uso abusivo de alcohol entre estudiantes.OBJETIVO: Analisar a associação entre o uso pesado de álcool entre estudantes e os fatores familiares, pessoais e sociais.     On the stability of tetrahedral relative equilibria in the positively curved 4-body problem

    We consider the motion of point masses given by a natural extension of Newtonian gravitation to spaces of constant positive curvature. Our goal is to explore the spectral stability of tetrahedral orbits of the corresponding 4-body problem in the 2-dimensional case, a situation that can be reduced to studying the motion of the bodies on the unit sphere. We first perform some extensive and highly precise numerical experiments to find the likely regions of stability and instability, relative to the values of the masses and to the latitude of the position of three equal masses. Then we support the numerical evidence with rigorous analytic proofs in the vicinity of some limit cases in which certain masses are either very large or negligible, or the latitude is close to zero.Comment: 32 pages, 6 figure
