    Anatomical variant origin of suprarenal arteries from coeliac trunk development, and its clinical significance

    Background: One of the most vascular organ in the body Adrenal gland being highly variant in vasculature and tough to approach, its knowledge needs to be updated regularly. Anatomists, Surgeons and Radiologists will be benefitted with this study and improves the quality of care provided to patients by reducing morbidity and mortality.Methods: This study was done on 48 formalin fixed cadavers (33 males, 15 females) in the department of anatomy, between 2014-2019. Coeliac trunk was skeletonized, and branches traced looking for suprarenal arterial branches.Results: Superior Suprarenal Artery originated from Inferior phrenic artery in 13(27%) cases, and these Inferior phrenic arteries were arising from the Coeliac trunk, and in one (2.03%) case Superior suprarenal artery was arising directly from Coeliac trunk and which had even replaced the Middle Suprarenal Artery. None of the Middle and Inferior Suprarenal Artery came from coeliac trunk.Conclusions: Knowledge of Superior Suprarenal Arterial variations while doing surgeries, and during radiological interventions in and around the lesser sac, and involving the Coeliac trunk helps the clinicians in reducing the morbidity and mortality

    Neurobrucellosis: a case report

    Neurologic involvement of brucellosis is common but exact prevalence of it is unknown due to difficulty in diagnosis and inadequate reporting all over the world. Neurologic involvement may manifest as chronic meningitis, lymphocytic meningoencephalitis, a variety of cranial nerve deficits, or ruptured mycotic aneurysms. We report a case of young male who had history of fever and headache on and off for 2 years and later developed signs of meningitis, lateral rectus palsy, altered sensorium and bilateral papilledema. He did not respond to anti-tubercular therapy and antibiotics. CT scan and MRI brain were normal. He had hepato-splenomegaly with small hypoechoic lesions in the spleen. CSF culture grew gram negative bacilli. Brucella IgG antibodies in serum by EIA 7, IgM negative, Brucella antigen titer was positive, 1:160. Patient received inj. streptomycin 0.75 g for 21 days and doxycycline 100 mg twice daily doxycycline for 6 weeks. The complete improvement in patient’s condition after a long lasting illness motivated authors to report this case. Delay or failure in diagnosis of this treatable disease may lead to significant morbidity and mortality so high index of suspicion should be kept in such cases

    Predictors of empathy in women social workers

    Summary This study explored the manifestation of reflective ability, emotional intelligence and empathy in 120 women social workers in Tiruchirappalli, India. A cross-sectional quantitative design was used and the Emotional Intelligence Scale, Groningen Reflective Ability Scale and the Empathy Assessment Index were administered to assess the manifestation of these dimensions. Multiple regression analysis was used to identify predictors of empathy. Findings Based on mean scores, the majority of respondents were classified as being ‘high’ in terms of emotional intelligence and reflective ability. Respondents of different age groups showed a significant difference in terms of the manifestation of empathy. Age, self-reflection and appraisal and expression of emotions were extracted as significant predictors of empathy. Application The article discusses the importance of these professional attributes for effective practice and the role of academic institutions and social work organisations in fostering the development of these dimensions in social work practitioners

    Cataract surgery at Aravind Eye Hospitals: 1988–2008

    In the 1980s, cataract was the major cause of blindness in India and was responsible for 80% of all blindness. This prompted the Indian government to launch a national cataract control programme, which succeeded in lowering the prevalence of blindness from 1.49% to 1.1%. In addition, by 2000, this programme had reduced the proportion of people blind due to cataract from 80% to 62%.Aravind Eye Hospitals contributed to a third of all cataract operations in the state of Tamil Nadu during the last two decades and played a major part in lowering the rate of blindness in that state. By 2000, the prevalence level of blindness was just 0.78%, compared to the national level of 1.11%

    Correlation of clinical and ultrasonographic features with histopathology in post-menopausal bleeding

    Background: Postmenopausal bleeding is generally regarded as an ominous alarm of genital pathologies which requires a thorough evaluation clinically and pathologically to exclude carcinoma as the cause and ensure a benign pathology. This study aims at finding out whether clinical diagnosis and ultrasonographic features can be reliable parameters for the diagnosis of causes and whether the findings correspond with histopathology reports.Methods: This observational study was conducted in a tertiary care centre in Pondicherry between January 2018 to August 2019. 114 women were enrolled for whom detailed history taking and clinical examination was done. All the patients were subjected to transvaginal ultrasonography. Patients with clinically visible lesions on cervix and vulva were subjected to biopsy and the rest underwent fractional curettage and the sample was sent for histopathological examination. Finally, histopathology report was compared with clinical and ultrasonographic findings.Results: With endometrial thickness cut off of 4 mm, the sensitivity, specificity, positive predictive value and negative predictive value in predicting malignancy by ultrasonography were 100%, 12.3%, 4.5% and 100%. Histopathology showed atrophic endometrium (43.8%), endometrial hyperplasia (8%), endometrial polyp (7.9%) and endometrial carcinoma (3%). Clinical and ultrasonographic findings did not show any statistical correlation with histopathology.Conclusions: Authors conclude that clinical findings and ultrasonographic features do not correlate with histopathology in cases of postmenopausal bleeding for which atrophic endometrium was the commonest etiology. However, ultrasound should be done routinely before endometrial sampling as the sensitivity for predicting malignancy was 100% for endometrial thickness cut off of 4 mm

    Phacomatosis pigmentovascularis (PPV) is a rare syndrome characterized by capillary vascular malformation and pigmentary nevus but with a wide variability in clinical presentation. A case of a 9-month-old patient is reported, who presented with capillary malformation and hemihypertrophy. These features typically are seen in Klippel–Trenaunay (KT) syndrome, a syndromic type of congenital vascular malformation. However, in addition, this child had large persistent Mongolian spots, the presence of which allowed us to classify this case as PPV, type II. This is a condition with a clinical course not always benign as KT syndrome
