Procedurally, it discusses the emergence of the fourfold Gospel tradition, the Synoptic Problem and how it has been approached in history. The essay argues in favor of the Clementine or Two-Gospel Hypothesis position. An analysis of Matthew 10:5-23 functions as an illustration of the value of this Hypothesis for understanding the text and seeing its value for the life of faith

    Post-operative bilateral adrenal haemorrhage: A case report

    AbstractINTRODUCTIONBilateral adrenal haemorrhage is a rare, but serious, illness carrying an estimated 15% mortality.1,2 The majority of cases occur in patients with acute, stressful illness, however the exact mechanism underlying adrenal haemorrhage remains unclear. This medical emergency carries significant diagnostic difficulty4 with non-specific clinical symptoms and variations in electrolyte abnormalities. Timely treatment is important as it prevents both the acute and long-term sequelae of adrenal failure.PRESENTATION OF CASEThis report describes a medical emergency in a surgical patient following emergency surgery for intra-abdominal sepsis. The patient reported non-specific symptoms of confusion, mild pyrexia and vague abdominal pain during the post-operative phase, with subtle electrolyte abnormalities and a low serum cortisol suggestive of adrenal crisis. Timely medical treatment, with intravenous hydrocortisone and intensive monitoring, and appropriate medical follow-up with addition of long-term fludrocortisone resulted in a satisfactory outcome.DISCUSSIONThis report describes a potentially life-threatening complication of intra-abdominal sepsis with adrenal crisis secondary to bilateral adrenal haemorrhage. In particular, this case highlights the diagnostic difficulty in such surgical patients due to vague symptoms and, in this case, the presence of a presentation variant with acute hyponatraemia and normal potassium.CONCLUSIONThis case highlights the importance of awareness of both the symptoms and signs and variation in electrolyte profile when assessing surgical patients post-operatively. In addition, this case highlights the benefit of early recognition and initiation of treatment and the importance of follow-up as long-term medical management is often required to prevent further relapse

    The impacts of precision livestock farming tools on the greenhouse gas emissions of an average Scottish dairy farm

    Precision livestock farming (PLF) tools are increasingly used in daily herd management to improve health, welfare, and overall production. While not intended to reduce greenhouse gas (GHG) emissions on farm, PLF tools can do so indirectly by improving overall efficiency, thereby reducing the emissions per unit of product. This work modelled the potential effects of commercially available PLF tools on whole enterprise and product emissions of two average Scottish dairy farm systems (an 8,000ā€‰ L and 10,000ā€‰ L herd) using the Agrecalc carbon foot printing tool. Scenarios modelled included an improvement infertility and an improvement in fertility and yield from the introduction of an accelerometer-based sensor, and an improvement in health from introduction of an accelerometer-based sensor, with and without the use of management interventions. Use of a sensor intended to improve fertility had the large streduction in total emissions (kg CO2e) of āˆ’1.42% for a 10,000ā€‰ L farm, with management changes applied. The largest reduction in emissions from milk production (kg CO2e) of āˆ’2.31% was observed via fertility technology application in an 8,000ā€‰ L farm, without management changes. The largest reduction in kg CO2e per kg fat and protein corrected milk of āˆ’6.72% was observed from an improvement in fertility and yield in a 10,000ā€‰ L herd, with management changes. This study has highlighted the realistic opportunities available to dairy farmers in low and high input dairy systems to reduce their emissions through adoption of animal mounted PLF technologies

    Pharmacist Empathy in Smoking Cessation Counseling

    Cigarette smoking and nicotine addiction are prevalent in todayā€™s society. Approximately 19% of American adults (43.8 million people) smoke cigarettes. Smoking is associated with health risks such as Chronic Obstructive Pulmonary Disease (COPD), infertility, low birth weight, respiratory symptoms, heart disease, and lung cancer and is responsible for roughly $96 billion in health care costs. Provider empathy has been proven effective in other treatments like cold and cancer; however, its effectiveness in smoking cessation has not yet been studied. Empathy is defined in two realms: cognitive and affective. In the cognitive domain, individuals have the ability to understand and view the world from anotherā€™s perspective, and in the affective domain, they can connect to the experiences or feelings of others. The twofold objective of this study is (1) to validate a modified Kiersma-Chen Empathy Scale to measure patient perceptions of provider empathy and (2) to determine if patients who view their pharmacists as empathetic achieve a higher quit rate in smoking cessation. The original Kiersma-Chen Empathy Scale is a previously-validated measure of provider empathy. Patients who are utilizing smoking cessation clinical services and meet inclusion criteria and whose quit date was at least 6 months prior, will complete three surveys 6 months after their quit date. The surveys conducted will measure patient demographics, quit status, and the patientā€™s perceptions of their pharmacistā€™s empathy (modified KCES). The collected data will be analyzed via a psychometric test to show the validity of the modified empathy scale and via a Spearman correlation to demonstrate the association between provider empathy and smoking cessation rates. The results of this study will provide beneficial information about how pharmacistā€™s can best assist their patients in smoking cessation, specifically regarding the empathy shown to the patient

    This article reports on one part of an on-going project, the Near Future School, which aims to translate and explore the potential of participatory design fiction practices for use with young people and those that work with them to explore near future scenarios of education that open up alternative and plural futures in the context of processes of foreclosure in a neoliberalising society. The focus here is to explore the practical and ethical issues of developing a speculative form of governance, using the philosophy of Benedict de Spinoza, as an act of imaginative world building through participatory design fictions. The research raises a series of questions and issues relating to understanding how design fiction's multiple inheritances, from fiction and design or art and design, need to be better understood and enacted within participatory design fiction processes
