But the extent of the environmental damage caused by the military industry is becoming increasingly apparent. Keith Mc Loughlin writes that, to reduce defence spending, the left will have to win the argument that industrial conversion to a socially useful Green New Deal is possible without a loss to the economy or national security

    Converting the United States and United Kingdom defence sector to civil production: the views of defence workers

    There are many social, environmental, and economic reasons for converting from defence manufacturing to civil production. Importantly, such a transformation could support more peace in the world while still ensuring the secure jobs that the defence sector has historically provided. The views of defence workers on such a transition are important to understanding how this change could effectively and equitably occur. To capture some of these views, the research project on which this paper is based involved interviewing 58 former and current defence sector workers in the United States and the United Kingdom and convening key leadership focus groups which included their trade union representatives. Though these workers' opinions were not entirely polarized and some interviewees had mixed and nuanced views, they loosely fell into three categories (a) opposed to defence diversification, (b) supportive of defence diversification, and (c) supportive in principle but thought that it would be unlikely to happen. The (a) category of defence workers primarily based their view on the idea that the defence sector is necessary for security and a feeling of pride in supporting this endeavor. The (b) category views were built on the perceived harm of the sector, the potential ‘peace dividend’ that would arise from transition, and feelings of guilt about working in the sector. The (c) group of workers identified the profit motive, worker attachment to defence jobs, technological issues, and the economic and political power of the defence companies as the key barriers to change. Proposals for effectively converting included economic incentives, government regulation, and organizing trade unions to push for change. A unified view was that workers want to be involved in discussions about these issues in their workplace and unions

    The necessity of a transformational approach to just transition: defence worker views on decarbonisation, diversification and sustainability

    This paper highlights the perspectives of defence workers regarding a Just Transition of their industry, one of the most environmentally harmful sectors in terms of greenhouse gas (GHG) emissions, pollution and use of non-renewable resources. It is based on (i) interviews with defence sector workers in the United States and the United Kingdom (n58); and (ii) focus groups with key representatives of national and international labour unions, defence companies and relevant NGOs (n18). Some of these defence workers and their trade union representatives envisioned a transformational path towards sustainability, including converting the defence sector to more socially useful production. Drawing on Gramsci, we define these as ‘counter-hegemonic views’, since they challenge the hegemonic ‘growth coalition’ and ‘treadmill of destruction’ paradigms. Such views support and echo more radical interpretations of Just Transition, emphasising the necessity of a structural transformation of society to achieve a fair and effective transition to sustainability

    Pulmonary endothelial permeability and tissue fluid balance depend on the viscosity of the perfusion solution

    Fluid filtration in the pulmonary microcirculation depends on the hydrostatic and oncotic pressure gradients across the endothelium and the selective permeability of the endothelial barrier. Maintaining normal fluid balance depends both on specific properties of the endothelium and of the perfusing blood. Although some of the essential properties of blood needed to prevent excessive fluid leak have been identified and characterized, our understanding of these remains incomplete. The role of perfusate viscosity in maintaining normal fluid exchange has not previously been examined. We prepared a high-viscosity perfusion solution (HVS) with a relative viscosity of 2.5, i.e., within the range displayed by blood flowing in vessels of different diameters in vivo (1.5–4.0). Perfusion of isolated murine lungs with HVS significantly reduced the rate of edema formation compared with perfusion with a standard solution (SS), which had a lower viscosity similar to plasma (relative viscosity 1.5). HVS did not alter capillary filtration pressure. Increased endothelial shear stress produced by increasing flow rates of SS, to mimic the increased shear stress produced by HVS, did not reduce edema formation. HVS significantly reduced extravasation of Evans bluelabeled albumin compared with SS, indicating that it attenuated endothelial leak. These findings demonstrate for the first time that the viscosity of the solution perfusing the pulmonary microcirculation is an important physiological property contributing to the maintenance of normal fluid exchange. This has significant implications for our understanding of fluid homeostasis in the healthy lung, edema formation in disease, and reconditioning of donor organs for transplantation

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.     BHPR research: qualitative1. Complex reasoning determines patients' perception of outcome following foot surgery in rheumatoid arhtritis

    Background: Foot surgery is common in patients with RA but research into surgical outcomes is limited and conceptually flawed as current outcome measures lack face validity: to date no one has asked patients what is important to them. This study aimed to determine which factors are important to patients when evaluating the success of foot surgery in RA Methods: Semi structured interviews of RA patients who had undergone foot surgery were conducted and transcribed verbatim. Thematic analysis of interviews was conducted to explore issues that were important to patients. Results: 11 RA patients (9 ♂, mean age 59, dis dur = 22yrs, mean of 3 yrs post op) with mixed experiences of foot surgery were interviewed. Patients interpreted outcome in respect to a multitude of factors, frequently positive change in one aspect contrasted with negative opinions about another. Overall, four major themes emerged. Function: Functional ability & participation in valued activities were very important to patients. Walking ability was a key concern but patients interpreted levels of activity in light of other aspects of their disease, reflecting on change in functional ability more than overall level. Positive feelings of improved mobility were often moderated by negative self perception ("I mean, I still walk like a waddling duck”). Appearance: Appearance was important to almost all patients but perhaps the most complex theme of all. Physical appearance, foot shape, and footwear were closely interlinked, yet patients saw these as distinct separate concepts. Patients need to legitimize these feelings was clear and they frequently entered into a defensive repertoire ("it's not cosmetic surgery; it's something that's more important than that, you know?”). Clinician opinion: Surgeons' post operative evaluation of the procedure was very influential. The impact of this appraisal continued to affect patients' lasting impression irrespective of how the outcome compared to their initial goals ("when he'd done it ... he said that hasn't worked as good as he'd wanted to ... but the pain has gone”). Pain: Whilst pain was important to almost all patients, it appeared to be less important than the other themes. Pain was predominately raised when it influenced other themes, such as function; many still felt the need to legitimize their foot pain in order for health professionals to take it seriously ("in the end I went to my GP because it had happened a few times and I went to an orthopaedic surgeon who was quite dismissive of it, it was like what are you complaining about”). Conclusions: Patients interpret the outcome of foot surgery using a multitude of interrelated factors, particularly functional ability, appearance and surgeons' appraisal of the procedure. While pain was often noted, this appeared less important than other factors in the overall outcome of the surgery.     Basic science232. Certolizumab pegol prevents pro-inflammatory alterations in endothelial cell function

    Background: Cardiovascular disease is a major comorbidity of rheumatoid arthritis (RA) and a leading cause of death. Chronic systemic inflammation involving tumour necrosis factor alpha (TNF) could contribute to endothelial activation and atherogenesis. A number of anti-TNF therapies are in current use for the treatment of RA, including certolizumab pegol (CZP), (Cimzia ®; UCB, Belgium). Anti-TNF therapy has been associated with reduced clinical cardiovascular disease risk and ameliorated vascular function in RA patients. However, the specific effects of TNF inhibitors on endothelial cell function are largely unknown. Our aim was to investigate the mechanisms underpinning CZP effects on TNF-activated human endothelial cells. Methods: Human aortic endothelial cells (HAoECs) were cultured in vitro and exposed to a) TNF alone, b) TNF plus CZP, or c) neither agent. Microarray analysis was used to examine the transcriptional profile of cells treated for 6 hrs and quantitative polymerase chain reaction (qPCR) analysed gene expression at 1, 3, 6 and 24 hrs. NF-κB localization and IκB degradation were investigated using immunocytochemistry, high content analysis and western blotting. Flow cytometry was conducted to detect microparticle release from HAoECs. Results: Transcriptional profiling revealed that while TNF alone had strong effects on endothelial gene expression, TNF and CZP in combination produced a global gene expression pattern similar to untreated control. The two most highly up-regulated genes in response to TNF treatment were adhesion molecules E-selectin and VCAM-1 (q 0.2 compared to control; p > 0.05 compared to TNF alone). The NF-κB pathway was confirmed as a downstream target of TNF-induced HAoEC activation, via nuclear translocation of NF-κB and degradation of IκB, effects which were abolished by treatment with CZP. In addition, flow cytometry detected an increased production of endothelial microparticles in TNF-activated HAoECs, which was prevented by treatment with CZP. Conclusions: We have found at a cellular level that a clinically available TNF inhibitor, CZP reduces the expression of adhesion molecule expression, and prevents TNF-induced activation of the NF-κB pathway. Furthermore, CZP prevents the production of microparticles by activated endothelial cells. This could be central to the prevention of inflammatory environments underlying these conditions and measurement of microparticles has potential as a novel prognostic marker for future cardiovascular events in this patient group.     Nigerian London: re-mapping space and ethnicity in superdiverse cities

    This paper explores the idea of ‘superdiversity’ at the city level through two churches with different approaches to architectural visibility: the hypervisible Universal Church of the Kingdom of God and the invisible Igbo Catholic Church, both in North London, guide our exploration of invisible Nigerian London. Although Nigerians have lived in London for over 200 years, they live beneath the radar of policy and public recognition rather than as a vital and visible element of superdiversity. This paper argues that we can trace the journeys composing Nigerian London in the deep textures of the city thus making it visible, but this involves re-mapping space and ethnicity. It argues that visibility is vital in generating more open forms of urban encounter and, ultimately, citizenship
