    Connecting Environmental Humanities: Developing Interdisciplinary Collaborative Method

    There is now a consensus that the potential contribution of the humanities to wider environmental debate is significant, although how to develop it effectively is still unclear. This paper therefore focusses on realizing the potential of the environmental humanities through building interdisciplinary collaboration. A four-stage research model is outlined for areas where there is limited humanities scholarship, based on ongoing experience of the humanities in action in the Royal Society of Edinburgh Research Network in the Arts and Humanities,Connecting with a low-carbon Scotland. The model has two key objectives: (1) to enable humanities disciplines to articulate their own contributions to pre-identified environmental research issues; and (2) to develop interdisciplinary humanities collaboration on these issues. It can be adapted to develop understanding in local, national and international contexts, depending on the number of scholars involved and the available resources. The knowledge which emerges can facilitate further interdisciplinary working between the humanities, STEM subjects and social sciences, and be of value to environmental policy-makers

    Developing environmental law scholarship: Going beyond the legal space

    Elizabeth Fisher, Bettina Lange, Eloise Scotford and Cinnamon Carlarne have delivered a hard but justified message: environmental law scholarship is still perceived by many in the field as immature, and this is a reflection of the methodological challenges posed by its subject. This paper expands on their argument that scholars should think more closely about what can be learnt from debates about method in the wider legal mainstream and interdisciplinarity as part of the process of developing the discipline. It locates what is called 'classic' and 'novel' environmental law scholarship at the margins of the legal academy, which is conceptualised as an imagined legal 'space'. The paper then explores the ways in which insights derived from the environmental humanities and sciences can invigorate and mature environmental law scholarship by creating exciting new interdisciplinary contexts for the development of legal research method

    Collaborative Management of HIV Infection in the Community: An Effort to Improve the Quality of HIV Care

    Our hospital led a multidisciplinary community team to improve the quality of care delivered to HIV-infected clients utilizing a disease management approach in a US metropolitan community of 150,000 people. Community needs assessment and client and community surveys were used to define the problems. Patient care flowcharting and the creation of an electronic patient database facilitated patient tracking across the entire community. Clinical guidelines and a consultation and referral immunology clinic standardized care practices. Measurable improvements in the quality of care were noted in multiple areas. Flowchart completion rates rose from 44% to 100%; medication adherence assessment rose from 82% to 100%; immunization rates rose from a mean of 72% to a mean of 87%; PPD screening rose from a low of 35% to a high of 87%; perinatal transmission rates fell from 31% to 4%; and Emergency Department utilization decreased. Two essential components of the effort were the establishment of a full-time leadership position in the form of a clinical nurse practitioner and the creation of an electronic database with flowcharting to standardize the measurement, delivery and tracking of care. The programme has become an example of successful disease management through hospital-community collaboration

    State Mandated Prenatal Human Immunodeficiency Virus Screening at a Large Community Hospital

    Purpose: To describe the initial experience of state mandated prenatal HIV screening at a large community hospital.Methods: HIV screening was provided to all pregnant women as of October 1, 1999. All HIV-positive women identified received aggressive antiretroviral therapy to reduce the likelihood for vertical transmission. Neonates were screened for HIV at zero, six, and 12 months of age.Results: Seven pregnant women (0.3%) and two additional family members tested positive for HIV. All seven infants born to the identified HIV-positive women have tested negative for infection. We estimated that six of nine cases of HIV infection identified would have been missed under a policy of voluntary HIV screening.Conclusions: Universal screening for HIV in pregnancy is achievable and desirable and provides the best opportunity to minimize the number of new neonatal HIV infections

    Impact of an Adherence Program on the Health and Outlook of HIV-Infected Patients Failing Antiretroviral Therapy

    Background: We prospectively studied the impact of an adherence counselor on the outcome of patients failing antiretroviral therapy because of nonadherence. Methods: Forty-six patients, identified as chronically nonadherent were enrolled. Individual attention was provided using the information, motivation and behavioral methodology. HIV RNA (viral load, in copies/mL), CD4 count (in cells/[mm.sup.3]), and body weight before and after the adherence counselor were measured. Qualitative outcome and patient satisfaction were assessed by deidentified third-party interviews. Results: Over half completed at least 1 year; only 8 patients were lost to follow-up. Mean CD4 counts increased significantly (P \u3c .05) for completers at 6 and 12 months. Viral loads decreased between baseline and 6 months. Most clients reported subjective benefit from working with the adherence counselor. Conclusion: Although few clients showed complete virologic suppression, the value of an adherence counselor was validated. Longer term adherence programs should be evaluated

    Doing cold smarter

    Cold has been much neglected in the energy debate. Governments are developing strategies and policies to green everything from electricity to transport to heat, but the energy and environmental impacts of cooling have so far been largely ignored. This is a serious oversight, since making things cold is energy intensive and can be highly polluting, and demand for cooling in all its forms is booming worldwide – especially in developing countries. According to one projection, by the end of this century global demand for air conditioning alone could consume the equivalent of half our worldwide electricity generation today – and most of the increase will come in developing markets. The ‘greening’ of cold is clearly an urgent global problem – but it may also offer Britain a massive business opportunity. Cold may have been ignored but is vitally important to many aspects of modern life. An effective cold chain, for example, is essential for tackling problems such as food waste, food security, water conservation and public health. Cooling is also critical for many less obvious but essential functions: data centres couldn’t operate without it, nor for example MRI scanners in medicine or superconductors in power electronics. Cooling also provides modern levels of comfort in hot countries – and can make the difference between some regions being habitable or not. At the same time, vast amounts of cold are wasted – for instance during the regasification of LNG – which could in principle be recycled to satisfy some of this demand and start to reduce the environmental damage caused by cooling. Such a system-level approach – which starts by asking what energy services we need, and what is the least damaging way to provide them, rather than accepting existing practices as a fait accompli – has recently been coined the ‘Cold Economy’. It is clear the Cold Economy could unleash a wide range of innovative clean cold technologies and provide energy resilience, economic growth and environmental benefits, but there is an urgent need to develop a system-level analysis of this problem and the potential solutions to inform both industry and policymakers.     Not So Fast Kepler-1513: A Perturbing Planetary Interloper in the Exomoon Corridor

    Transit Timing Variations (TTVs) can be induced by a range of physical phenomena, including planet-planet interactions, planet-moon interactions, and stellar activity. Recent work has shown that roughly half of moons would induce fast TTVs with a short period in the range of two-to-four orbits of its host planet around the star. An investigation of the Kepler TTV data in this period range identified one primary target of interest, Kepler-1513 b. Kepler-1513 b is a 8.05−0.40+0.588.05^{+0.58}_{-0.40} R⊕R_\oplus planet orbiting a late G-type dwarf at 0.53−0.03+0.040.53^{+0.04}_{-0.03} AU. Using Kepler photometry, this initial analysis showed that Kepler-1513 b's TTVs were consistent with a moon. Here, we report photometric observations of two additional transits nearly a decade after the last Kepler transit using both ground-based observations and space-based photometry with TESS. These new transit observations introduce a previously undetected long period TTV, in addition to the original short period TTV signal. Using the complete transit dataset, we investigate whether a non-transiting planet, a moon, or stellar activity could induce the observed TTVs. We find that only a non-transiting perturbing planet can reproduce the observed TTVs. We additionally perform transit origami on the Kepler photometry, which independently applies pressure against a moon hypothesis. Specifically, we find that Kepler-1513 b's TTVs are consistent with an exterior non-transiting ∼\simSaturn mass planet, Kepler-1513 c, on a wide orbit, ∼\sim5%\% outside a 5:1 period ratio with Kepler-1513 b. This example introduces a previously unidentified cause for planetary interlopers in the exomoon corridor, namely an insufficient baseline of observations.Comment: 20 pages, 13 figures.     Distinct routes of lineage development reshape the human blood hierarchy across ontogeny

    In a classical view of hematopoiesis, the various blood cell lineages arise via a hierarchical scheme starting with multipotent stem cells that become increasingly restricted in their differentiation potential through oligopotent and then unipotent progenitors. We developed a cell-sorting scheme to resolve myeloid (My), erythroid (Er), and megakaryocytic (Mk) fates from single CD34+ cells and then mapped the progenitor hierarchy across human development. Fetal liver contained large numbers of distinct oligopotent progenitors with intermingled My, Er and Mk fates. However, few oligopotent progenitor intermediates were present in the adult bone marrow. Instead only two progenitor classes predominate, multipotent and unipotent, with Er-Mk lineages emerging from multipotent cells. The developmental shift to an adult ‘two-tier’ hierarchy challenges current dogma and provides a revised framework to understand normal and disease states of human hematopoiesis.This work was supported by Postdoctoral Fellowship Awards from Canadian Institute of Health Research (CIHR) to FN and SZ.
