    Modelling the cooking process of a single cereal grain

    Four models are developed to assist with the uniform and accurate cooking of whole grains for Uncle Tobys breakfast cereals. 1. Heat satisfies a linear conduction equation and is found to rapidly penetrate the grain. 2. Moisture satisfies a non-linear diffusion equation, and is found to penetrate the grain more slowly than heat. The more sophisticated moisture diffusion model is solved by numerical and analytic techniques for spherical and ellipsoidal grains. A vital role is played in the moisture diffusion model by the concept of the mean action time for wetting a grain. These first two models are used to determine sensitivity to key cooking parameters, and to calculate the degree of over-cook in the existing batch steam process. Recommendations are made for improving and speeding up the cooking process. The last two models are modifications of the nonlinear moisture penetration model 2. above. The results of these improved models have the potential to provide finer adjustments to estimates of wetting times. 3. A cereal grain swells significantly during wetting. A model that takes this into account is developed and solved approximately. 4. Another wetting model describes the effect of the gelatinisation reaction, slowing moisture penetration, and leading to a sharp front entering the grain. The effect of gelatinisation on the speed of moisture penetration is expected to be more important for the present high-temperature cooking process, than when soaking a grain at a lower temperature.     Laboratory testing trends for respiratory syncytial virus, 2007–2011

    AbstractBackgroundAntigen detection tests have been the most common diagnostic assay used to detect and diagnose respiratory syncytial virus (RSV). The utility and increased sensitivity of polymerase chain reaction (PCR) tests have been reported; however, their use in US hospital laboratories is not well characterized.ObjectiveTo describe changes in RSV test types used by US hospital-affiliated laboratories, focusing on PCR testing prevalence.Study designData were collected from 480 to 666 laboratories each RSV season (2007–2008 through 2010–2011) across 50 states, the District of Columbia, and Puerto Rico. A descriptive analysis was conducted using this convenience sample of RSV tests conducted from November to April each season. Total numbers and types of RSV tests performed were reported weekly and weekly proportions by test type were calculated. Kendall τ rank correlation was used to quantify associations between time and proportions of each test type.ResultsPCR tests accounted for 2%, 3%, 16%, and 21% of weekly tests (total range, 381,068–481,654 over 4 seasons) conducted each season from 2007 to 2011, respectively. The proportion of laboratories reporting ≥1 PCR tests was 4%, 5%, 10%, and 16%, respectively. Decreases in antigen testing and viral culture were similarly observed.ConclusionsAlthough antigen detection was the predominant test type reported in the sample of US hospital laboratories for RSV testing, PCR use increased to >20% of tests reported. These results demonstrate the increasing contribution of PCR to RSV surveillance. RSV surveillance systems relying solely on antigen detection results will not capture an increasing proportion of RSV test results

    Chronic condition self-management support: proposed competencies for medical students

    Author version made available in accordance with the publisher's policy.Objective Governments and the medical profession are concerned that there continues to be less than optimal health outcomes despite escalating expenditure on health services from the effect of the ageing population with chronic illnesses. In this context, doctors will need to have knowledge and skills in effective Chronic Condition Management (CCM) and Chronic Condition Self-Management (CCSM). Method A national workshop of representatives of eight medical schools from the CCSM Special Interest Group (SIG) of the Australian and New Zealand Association on Medical Education (ANZAME) met in September 2004, to consider curriculum content in CCM and CCSM. Results The workshop recommended that the Committee of Deans of Australian Medical Schools (CDAMS) and the Commonwealth Department of Health and Ageing (DoHA) consider the identification and possible development of a specific curriculum for CCM and CCSM within the curricula of Australian Medical Schools. Discussion Consideration needs to be given to the changing nature of medical practice and that as part of this; doctors of the future will need skills in team participation, continuity of care, self-management support and patient-centered collaborative care planning. Doctors will also need skills to assist patients to better adhere to medical management, lifestyle behaviour change and risk factor reduction, if optimal health outcomes are to be achieved and costs are to be contained

    Cerebrospinal fluid cannot be used to distinguish inflammatory myelitis from congestive myelopathy due to spinal dural arteriovenous fistula: case series.

    Patients with congestive myelopathy due to spinal dural arteriovenous fistula (SDAVF) typically present with progressive sensory and motor disturbance in association with sphincter dysfunction. Spinal MRI usually shows longitudinally extensive T2 signal change. Here, we report four patients with progressive myelopathy due to SDAVF who also presented with findings on cerebrospinal fluid (CSF) examination suggestive of an inflammatory aetiology. Such CSF findings in SDAVF are important to recognise, to avoid the erroneous diagnosis of an inflammatory myelitis and inappropriate treatment with immunosuppression. SDAVF can be difficult to detect and may require repeated investigation, with formal angiography as the gold standard

    K-shell photoionization of ground-state Li-like carbon ions [C3+^{3+}]: experiment, theory and comparison with time-reversed photorecombination

    Absolute cross sections for the K-shell photoionization of ground-state Li-like carbon [C3+^{3+}(1s2^22s 2^2S)] ions were measured by employing the ion-photon merged-beams technique at the Advanced Light Source. The energy ranges 299.8--300.15 eV, 303.29--303.58 eV and 335.61--337.57 eV of the [1s(2s2p)3^3P]2^2P, [1s(2s2p)1^1P]2^2P and [(1s2s)3^3S 3p]2^2P resonances, respectively, were investigated using resolving powers of up to 6000. The autoionization linewidth of the [1s(2s2p)1^1P]2^2P resonance was measured to be 27±527 \pm 5 meV and compares favourably with a theoretical result of 26 meV obtained from the intermediate coupling R-Matrix method. The present photoionization cross section results are compared with the outcome from photorecombination measurements by employing the principle of detailed balance.Comment: 3 figures and 2 table

    Linking Auxin with Photosynthetic Rate via Leaf Venation

    International audienceLand plants lose vast quantities of water to the atmosphere during photosynthetic gas exchange. In angiosperms, a complex network of veins irrigates the leaf, and it is widely held that the density and placement of these veins determines maximum leaf hydraulic capacity and thus maximum photosynthetic rate. This theory is largely based on interspecific comparisons and has never been tested using vein mutants to examine the specific impact of leaf vein morphology on plant water relations. Here we characterize mutants at the Crispoid (Crd) locus in pea (Pisum sativum), which have altered auxin homeostasis and activity in developing leaves, as well as reduced leaf vein density and aberrant placement of free-ending veinlets. This altered vein phenotype in crd mutant plants results in a significant reduction in leaf hydraulic conductance and leaf gas exchange. We find Crispoid to be a member of the YUCCA family of auxin biosynthetic genes. Our results link auxin biosynthesis with maximum photosynthetic rate through leaf venation and substantiate the theory that an increase in the density of leaf veins coupled with their efficient placement can drive increases in leaf photosynthetic capacity

    The Social, Educational and Market Scenario for nZEB in Europe

    Nearly Zero Energy Buildings (nZEB) are a significant part of the energy efficiency strategy of the European Union. As buildings represent approximately 40% of the final energy use in Europe, the reduction of their energy demand is key for a sustainable future. This paper takes a qualitative approach and presents data about professional and market barriers, as well as the educational market in relation to the implementation of nZEB policies for new and retrofit buildings in 11 European countries. Different levels of policy enactments and market penetration are reported and are generally found to be more advanced in western and central European countries. Furthermore, gender equality is examined in the building sector in relation to nZEB and presents significant gaps, with a more balanced situation reported in southern Europe. The accreditation and targeted education of nZEB experts is still almost non-existent in the examined countries, and the need for training of building professionals is highlighted as a critical missing component of current policy.     Mathematical models for estimating effective diffusion parameters of spherical drug delivery devices

    Mathematical modeling of drug delivery is of increasing academic and industrial importance in manyaspects. In this paper, we propose an optimization approach for the estimation of the parameters characterizing the diffusion process of a drug from a spherical porous polymer device to an external finite volume. The approach is based on a nonlinear least-squares method and a novel mathematical model which takes into consideration both boundary layer effect and initial burst phenomenon. Ananalytical solution to the model is derived and a formula for the ratio of the mass released in a given time interval and the total mass released in infinite time is also obtained. The approach has been tested using experimental data of the diffusion of prednisolone 21-hemisuccinate sodium saltfrom spherical devices made of porous poly(2-hydroxyethyl methacrylate) hydrogels. The effectiveness and accuracy of the method are well demonstrated by the numerical results. The model was used to determine the diffusion parameters including the effective diffusion coefficient of the drug from a series of devices that vary in both the porous structure and the drug loading levels. The computed diffusion parameters are discussed in relation to the physical properties of the devices

    The General Practice Assessment Questionnaire (GPAQ) – Development and psychometric characteristics

    <p>Abstract</p> <p>Background</p> <p>Continual quality improvement in primary care is an international priority. In the United Kingdom, the major initiative for improving quality of care is the Quality and Outcomes Framework (QoF) of the 2004 GP contract. Although the primary focus of the QoF is on clinical care, it is acknowledged that a comprehensive assessment of quality also requires valid and reliable measurement of the patient perspective, so financial incentives are included in the contract for general practices to survey patients' views. One questionnaire specified for use in the QoF is the General Practice Assessment Questionnaire (GPAQ). This paper describes the development of the GPAQ (with post-consultation and postal versions) and presents a preliminary examination of the psychometric properties of the questionnaire.</p> <p>Methods</p> <p>Description of scale development and preliminary analysis of psychometric characteristics (internal reliability, factor structure), based on a large dataset of routinely collected GPAQ surveys (n = 190,038 responses to the consultation version of GPAQ and 20,309 responses to the postal version) from practices in the United Kingdom during the 2005–6 contract year.</p> <p>Results</p> <p>Respondents tend to report generally favourable ratings. Responses were particularly skewed on the GP communication scale, though no more so than for other questionnaires in current use in the UK for which data were available. Factor analysis identified 2 factors that clearly relate to core concepts in primary care quality ('access' and 'interpersonal care') that were common to both version of the GPAQ. The other factors related to 'enablement' in the post-consultation version and 'nursing care' in the postal version.</p> <p>Conclusion</p> <p>This preliminary evaluation indicates that the scales of the GPAQ are internally reliable and that the items demonstrate an interpretable factor structure. Issues concerning the distributions of GPAQ responses are discussed. Potential further developments of the item content for the GPAQ are also outlined.</p

    Evidence for Structural and Functional Damage of the Inner Retina in Diabetes With No Diabetic Retinopathy

    Purpose: To provide structural and functional evidence of inner retinal loss in diabetes prior to vascular changes and interpret the structure-function relationship in the context of an established neural model. Methods: Data from one eye of 505 participants (134 with diabetes and no clinically evident vascular alterations of the retina) were included in this analysis. The data were collected as part of a large population-based study. Functional tests included best-corrected visual acuity, Pelli-Robson contrast sensitivity, mesopic microperimetry, and frequency doubling technology perimetry (FDT). Macular optical coherence tomography volume scans were collected for all participants. To interpret the structure-function relationship in the context of a neural model, ganglion cell layer (GCL) thickness was converted to local ganglion cell (GC) counts. Results: The GCL and inner plexiform layer were significantly thinner in participants with diabetes (P < 0.05), with no significant differences in the macular retinal nerve fiber layer or the outer retina. All functional tests except microperimetry showed a significant loss in diabetic patients (P < 0.05). Both FDT and microperimetry showed a significant relationship with the GC count (P < 0.05), consistent with predictions from a neural model for partial summation conditions. However, the FDT captured additional significant damage (P = 0.03) unexplained by the structural loss. Conclusions: Functional and structural measurements support early neuronal loss in diabetes. The structure-function relationship follows the predictions from an established neural model. Functional tests could be improved to operate in total summation conditions in the macula, becoming more sensitive to early loss
