    Safety of localizing epilepsy monitoring intracranial electroencephalograph electrodes using MRI: radiofrequency-induced heating

    Purpose: To investigate heating during postimplantation localization of intracranial electroencephalograph (EEG) electrodes by MRI. Materials and Methods: A phantom patient with a realistic arrangement of electrodes was used to simulate tissue heating during MRI. Measurements were performed using 1.5 Tesla (T) and 3T MRI scanners, using head- and body-transmit RF-coils. Two electrode-lead configurations were assessed: a standard condition with external electrode-leads physically separated and a fault condition with all lead terminations electrically shorted. Results: Using a head-transmit-receive coil and a 2.4 W/kg head-average specific absorption rate (SAR) sequence, at 1.5T the maximum temperature change remained within safe limits (<1°C). Under standard conditions, we observed greater heating (2.0°C) at 3T on one system and similar heating (<1°C) on a second, compared with the 1.5T system. In all cases these temperature maxima occurred at the grid electrode. In the fault condition, larger temperature increases were observed at both field strengths, particularly for the depth electrodes. Conversely, with a body-transmit coil at 3T significant heating (+6.4°C) was observed (same sequence, 1.2/0.5 W/kg head/body-average) at the grid electrode under standard conditions, substantially exceeding safe limits. These temperature increases neglect perfusion, a major source of heat dissipation in vivo. Conclusion: MRI for intracranial electrode localization can be performed safely at both 1.5T and 3T provided a head-transmit coil is used, electrode leads are separated, and scanner-reported SARs are limited as determined in advance for specific scanner models, RF coils and implant arrangements. Neglecting these restrictions may result in tissue injury

    Feasibility of simultaneous intracranial EEG-fMRI in humans: a safety study

    In epilepsy patients who have electrodes implanted in their brains as part of their pre-surgical assessment, simultaneous intracranial EEG and fMRI (icEEG-fMRI) may provide important localising information and improve understanding of the underlying neuropathology. However, patient safety during icEEG-fMRI has not been addressed. Here the potential health hazards associated with icEEG-fMRI were evaluated theoretically and the main risks identified as: mechanical forces on electrodes from transient magnetic effects, tissue heating due to interaction with the pulsed RF fields and tissue stimulation due to interactions with the switched magnetic gradient fields. These potential hazards were examined experimentally in vitro on a Siemens 3 T Trio, 1.5 T Avanto and a GE 3 T Signa Excite scanner using a Brain Products MR compatible EEG system. No electrode flexion was observed. Temperature measurements demonstrated that heating well above guideline limits can occur. However heating could be kept within safe limits (< 1.0 °C) by using a head transmit RF coil, ensuring EEG cable placement to exit the RF coil along its central z-axis, using specific EEG cable lengths and limiting MRI sequence specific absorption rates (SARs). We found that the risk of tissue damage due to RF-induced heating is low provided implant and scanner specific SAR limits are observed with a safety margin used to account for uncertainties (e.g. in scanner-reported SAR). The observed scanner gradient switching induced current (0.08 mA) and charge density (0.2 ΌC/cm2) were well within safety limits (0.5 mA and 30 ΌC/cm2, respectively). Site-specific testing and a conservative approach to safety are required to avoid the risk of adverse events

    Functional morphology of the forelimb of Early Miocene caviomorph rodents from Patagonia

    Caviomorph rodents (New World Hystricognathi) are represented in the Santa Cruz Formation of Patagonia (Early Miocene, Santacrucian) by four superfamilies. From this unit, excellent cranial and associated postcranial remains of Neoreomys, Eocardia, Schistomys (Cavioidea), Perimys (Chinchilloidea) and Steiromys (Erethizontoidea) are known. To interpret their use of substrate, a comparative description of limb bones (scapula, humerus, radius and ulna) of the five genera was performed within a taxonomic and ecologically diverse sample of extant rodents. Using palaeobiological inferences based on the habits and functional morphology of members of the extant sample, hypotheses were generated for the use of substrate by the Santacrucian taxa. Neoreomys would have had a marked flexo-extension capacity of the humerus and a moderate flexion of the manus. This genus and Eocardia would have had a complete and stabilized forearm extension, with wide stride and moderate extension of the forearm for the latter. Schistomys presents features similar to Eocardia. Perimys would have had strong external rotation of the humerus, moderate flexion of the manus and moderate, complete and stabilized extension of the forearm. Steiromys would have had good pronation/supination capacity, strong internal rotation of the humerus and flexion of the forearm and manus. Neoreomys would have been ambulatory and an occasional runner. Eocardia and Schistomys would have been good runners. Perimys would have been a digger and Steiromys a climber.     The VLT-FLAMES Tarantula Survey

    A spectroscopic analysis has been undertaken for the B-type multiple systems (excluding those with supergiant primaries) in the VLT-FLAMES Tarantula Survey (VFTS). Projected rotational velocities, vesini, for the primaries have been estimated using a Fourier Transform technique and confirmed by fitting rotationally broadened profiles. A subset of 33 systems with vesini ≀ 80 km s-1 have been analysed using a TLUSTY grid of model atmospheres to estimate stellar parameters and surface abundances for the primaries. The effects of a potential flux contribution from an unseen secondary have also been considered. For 20 targets it was possible to reliably estimate their effective temperatures (Teff) but for the other 13 objects it was only possible to provide a constraint of 20 000 ≀ Teff ≀ 26 000 K – the other parameters estimated for these targets will be consequently less reliable. The estimated stellar properties are compared with evolutionary models and are generally consistent with their membership of 30 Doradus, while the nature of the secondaries of 3 SB2 system is discussed. A comparison with a sample of single stars with vesini ≀ 80 km s-1 obtained from the VFTS and analysed with the same techniques implies that the atmospheric parameters and nitrogen abundances of the two samples are similar. However, the binary sample may have a lack of primaries with significant nitrogen enhancements, which would be consistent with them having low rotational velocities and having effectively evolved as single stars without significant rotational mixing. This result, which may be actually a consequence of the limitations of the pathfinder investigation presented in this paper, should be considered as a motivation for spectroscopic abundance analysis of large samples of binary stars, with high quality observational data

    Does It Really Work? Re-Assessing the Impact of Pre-Departure Cross-Cultural Training on Expatriate Adjustment

    Cultural adjustment is considered to be a prerequisite for expatriate success abroad. One way to enhance adjustment is to provide employees with knowledge and awareness of appropriate norms and behaviors of the host country through cross-cultural training (CCT). This article analyzes the impact of pre-departure CCT on expatriate adjustment and focuses on variations in participation, length and the comprehensiveness of training. Unlike previous research, the study focuses on the effectiveness of pre-departure CCT for non-US employees expatriated to a broad range of host country settings. Employing data from 339 expatriates from 20 German Multinational Corporations (MNCs) the study finds CCT has little if any effect on general, interactional or work setting expatriate adjustment. However, a significant impact of foreign language competence was found for all three dimensions of expatriate adjustment. We used interviews with 20 expatriates to supplement our discussion and provide further implications for practice

    ADAMTS13 regulation of VWF multimer distribution in severe COVID‐19

    Background Consistent with fulminant endothelial cell activation, elevated plasma von Willebrand factor (VWF) antigen levels have been reported in patients with COVID-19. The multimeric size and function of VWF are normally regulated through A Disintegrin And Metalloprotease with ThrombSpondin Motif type 1 motif, member 13 (ADAMTS-13)--mediated proteolysis. Objectives This study investigated the hypothesis that ADAMTS-13 regulation of VWF multimer distribution may be impaired in severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) infection contributing to the observed microvascular thrombosis. Patients and Methods Patients with COVID-19 (n = 23) were recruited from the Beaumont Hospital Intensive Care Unit (ICU) in Dublin. Plasma VWF antigen, multimer distribution, ADAMTS-13 activity, and known inhibitors thereof were assessed. Results We observed markedly increased VWF collagen-binding activity in patients with severe COVID-19 compared to controls (median 509.1 versus 94.3 IU/dl). Conversely, plasma ADAMTS-13 activity was significantly reduced (median 68.2 IU/dl). In keeping with an increase in VWF:ADAMTS-13 ratio, abnormalities in VWF multimer distribution were common in patients with COVID-19, with reductions in high molecular weight VWF multimers. Terminal sialylation regulates VWF susceptibility to proteolysis by ADAMTS-13 and other proteases. We observed that both N- and O-linked sialylation were altered in severe COVID-19. Furthermore, plasma levels of the ADAMTS-13 inhibitors interleukin-6, thrombospondin-1, and platelet factor 4 were significantly elevated. Conclusions These findings support the hypothesis that SARS-CoV-2 is associated with profound quantitative and qualitative increases in plasma VWF levels, and a multifactorial down-regulation in ADAMTS-13 function. Further studies will be required to determine whether therapeutic interventions to correct ADAMTS-13-VWF multimer dysfunction may be useful in COVID-microvascular thrombosis and angiopathy

    Emotional intelligence and British expatriates’ cross-cultural adjustment in international construction projects

Today’s internationalized business demands global mindset, intercultural sensitivity and the ability to skilfully negotiate through cross-cultural interactions. Therefore, the overall aim was to investigate the influence of emotional intelligence (EI) on cross-cultural adjustment (CCA) of British expatriates working on International Architectural, Engineering and Construction assignments in Sub-Saharan Africa, China, Middle East and Indian Sub-Continent. Specifically, the causal relationship between EI and three facets of CCA i.e. work, general and interaction adjustment was explored. A sequential exploratory mixed methods design was adopted. These include extensive review of existing literature, eighteen unstructured interviews, and questionnaire survey of 191 British expatriates operating in 29 different countries from the four regions under investigation. Structural equation modelling was used to assess the causal relationship between EI and CCA. Results show that EI accounted for 91, 64 and 24% of the variance in work, interaction and general adjustment respectively. Overall, the model was able to explain 60% variance in CCA, suggesting that EI competencies play a huge role in facilitating an expatriate understand and adapt to host country culture. The findings would help decision-makers (HR managers) during expatriate selection process, in understanding that along with technical skills, it is the emotional competencies that are crucial in assisting expatriates adjust to foreign way of life

    The VLT-FLAMES Tarantula Survey XXVIII. Nitrogen abundances for apparently single dwarf and giant B-type stars with small projected rotational velocities

    Previous analyses of the spectra of OB-type stars in the Magellanic Clouds have identified targets with low projected rotational velocities and relatively high nitrogen abundances; the evolutionary status of these objects remains unclear. The VLT-FLAMES Tarantula Survey obtained spectroscopy for over 800 early-type stars in 30 Doradus of which 434 stars were classified as B-type. We have estimated atmospheric parameters and nitrogen abundances using tlusty model atmospheres for 54 B-type targets that appear to be single, have projected rotational velocities, ve sin i ≀ 80 km s−1 and were not classified as supergiants. In addition, nitrogen abundances for 34 similar stars observed in a previous FLAMES survey of the Large Magellanic Cloud have been re-evaluated. For both samples, approximately 75-80% of the targets have nitrogen enhancements of less than 0.3 dex, consistent with them having experienced only small amounts of mixing. However, stars with low projected rotational velocities, ve sin i ≀ 40 km s−1 and significant nitrogen enrichments are found in both our samples and simulations imply that these cannot all be rapidly rotating objects observed near pole-on. For example, adopting an enhancement threshold of 0.6 dex, we observed five and four stars in our VFTS and previous FLAMES survey samples, yet stellar evolution models with rotation predict only 1.25±1.11 and 0.26±0.51 based on our sample sizes and random stellar viewing inclinations. The excess of such objects is estimated to be 20-30% of all stars with current rotational velocities of less than 40 km s−1 . This would correspond to ∌2-4% of the total non-supergiant single B-type sample. Given the relatively large nitrogen enhancement adopted, these estimates constitute lower limits for stars that appear inconsistent with current grids of stellar evolutionary models. Including targets with smaller nitrogen enhancements of greater than 0.2 dex implies larger percentages of targets that are inconsistent with current evolutionary models, viz. ∌70% of the stars with rotational velocities less than 40 km s−1 and ∌6-8% of the total single stellar population. We consider possible explanations of which the most promising would appear to be breaking due to magnetic fields or stellar mergers with subsequent magnetic braking

    1000 Genomes-based meta-analysis identifies 10 novel loci for kidney function.

    HapMap imputed genome-wide association studies (GWAS) have revealed &gt;50 loci at which common variants with minor allele frequency &gt;5% are associated with kidney function. GWAS using more complete reference sets for imputation, such as those from The 1000 Genomes project, promise to identify novel loci that have been missed by previous efforts. To investigate the value of such a more complete variant catalog, we conducted a GWAS meta-analysis of kidney function based on the estimated glomerular filtration rate (eGFR) in 110,517 European ancestry participants using 1000 Genomes imputed data. We identified 10 novel loci with p-value &lt; 5 × 10(-8) previously missed by HapMap-based GWAS. Six of these loci (HOXD8, ARL15, PIK3R1, EYA4, ASTN2, and EPB41L3) are tagged by common SNPs unique to the 1000 Genomes reference panel. Using pathway analysis, we identified 39 significant (FDR &lt; 0.05) genes and 127 significantly (FDR &lt; 0.05) enriched gene sets, which were missed by our previous analyses. Among those, the 10 identified novel genes are part of pathways of kidney development, carbohydrate metabolism, cardiac septum development and glucose metabolism. These results highlight the utility of re-imputing from denser reference panels, until whole-genome sequencing becomes feasible in large samples
